Innovative Learning Grant Application – 2008 2009
Project Title: Student Success Center Successful Communication Initative
Project Description – Provide a brief description of your project (50 words or less). This
description will be used in grant program summaries and publicity.
The Student Success Center is a hub for all students interested in learning about future
educational and vocational choices. Students look to the SSC to learn who will be
visiting each day and for information about upcoming seminars, Pathway field trips,
summer camps, employment opportunities, scholarships, internship and service
learning.
Amount Requested: $ 2353.54
Number of Students to be served: 1850 Grade Level(s): 9-12
Funding Categories
For 2008-2009 applications, restricted and unrestricted grants will be offered.
Please indicate the appropriate category for your grant application. Choose the category
that most closely aligns with the goals of your project.
Restricted Grants - these projects must fall into one of the following categories. The
total amount of all combined grants will be at least $ 60,000. See Innovative Learning
Grant Guidelines for descriptions of these categories.

Project Narrative
Please use pages 4 & 5 to provide the information requested below. (Do not exceed
two pages)
1. Briefly describe your project. State clearly how the project relates to the funding
category you have chosen.
2. What are the goals and objectives of your project? List sequential activities and
approximate time frame for attaining the goals. How will you measure the
success of the project?
3. What is the relationship of your project to the curriculum? Reference PDE
academic standards when applicable. In what way is this an innovative and
enriching approach to teaching and learning?
4. How will you evaluate the impact and success of the project? What are the
follow-up activities for students? Describe the long term impact on teaching and
learning.
5. Describe all equipment (nonexpendable items over $500) requested for this
project. How is the equipment essential to the goals of your project?

Breakdown expenses by these categories:
Equipment – nonexpendable items that exceed a cost of $500 per item. Projects
that require equipment purchases must include a brief description of the equipment (in
the narrative) as well as supporting documents (catalogues, etc.).
Materials & Supplies – expendable/consumable items necessary for the project
Professional Development/Presenter Expenses – fees and travel expenses
related to bringing in outside professionals/ presenters. Projects that require
outside presenters or personnel must include resumes and/or organizational
information about the presenters.
Travel – student, teacher, and administrator related travel costs
Other expenses – everything else

Project Title: Student Success Center Successful Communication Initative
Project Narrative: (page 1)
The Student Success Center (SSC) is in a central location in our high school and is bustling with activity--
averaging 435 student visits per week. Each week we host multiple visits from colleges, the military,
professional guests for our Lunch & Learn series, hold informational seminars to share information about
career planning, course selection, college application processes, how to write a resume, how to find an
internship, job acquisition support services, etc. We also promote special opportunities for our students at
the SSC. Students learn about Pathways field trips to area businesses and to local colleges at the SSC.
They also learn about community service opportunities. The funding request for this project is from Media
Literacy. We expect to involve our students, staff and guests to interact and create using this medium.
The goals of this project are as follows: (1) to serve as a communication center, a smart board will be
placed in an area where students can see it and it's messages (2) the smart board will be avaliable to
guests to use as they talk to students (this will work especially well for our college representative visits
who can show students their websites and how to apply to college and our Lunch & Learn guests will be
able to use PPT presentations or navigate the web to show students current workplace projects (3) be a
tool for students to use as they present (4) be a tool for counselors as they work with small groups (i.e.
training for use of Bridges and Naviance Family Connection--see attached), (5) be avaliable to small
groups of students working on project planning. The platform will be used daily as an information source
for students to observe as they walk by the SSC and look into the space. It will be used by college reps,
professional speakers and staff presenting on an as needed basis. Success will be measured by
increased activity in the SCC (student attendance and number of functions hosted).
The functions and activities in the SSC help HHHS comply with the PA Career Education and Work
(CEW) standards (www.pacareerstandards.com). All functions and activities in the center support and
hold together the scope and sequencing of our CEW program. The SSC activities and programing
connect all of the pieces of our program from 9th
and 10th
grade CEW standards infusion to the 11th
grade
Pathways Seminar and 12th
grade community learning experiences such as Internship and Service
Learning (see attachments).
We use the Bridges program (www.bridges.com) for our students to build electronic portfolios. The
portfolio is started freshman year and is worked on every year through high school graduation. We expect
to see personal growth statements and reflections that will result, in part, to the exposures and activities
students have had in the SSC. Our students will all write a personal transition plan explaining their post-
high school plans and how those goals will be achieved. The plans will be presented to their teachers,
parents and school counselors.
For this project we are asking for: (1) NEC NP200 - DLP projector, (2) SMART Board 500 Interactive
Whiteboard and (3) Balt Rolz 2 Mobile Conference Center. This equipment will allow for the presentations
and uses described above. The moblie conference center will allow for movement of projection area and
provide security.

Grant Deadline – January 23, 2009 Innovative Learning Grant Application – 2008 2009 Project Title: Student Success Center Successful Communication Initative Project Description – Provide a brief description of your project (50 words or less). This description will be used in grant program summaries and publicity. The Student Success Center is a hub for all students interested in learning about future educational and vocational choices. Students look to the SSC to learn who will be visiting each day and for information about upcoming seminars, Pathway field trips, summer camps, employment opportunities, scholarships, internship and service learning. Amount Requested: $ 2353.54 Number of Students to be served: 1850 Grade Level(s): 9-12 Funding Categories For 2008-2009 applications, restricted and unrestricted grants will be offered. Please indicate the appropriate category for your grant application. Choose the category that most closely aligns with the goals of your project. Restricted Grants - these projects must fall into one of the following categories. The total amount of all combined grants will be at least $ 60,000. See Innovative Learning Grant Guidelines for descriptions of these categories. Hands-On Math & Science Media Literacy Environmental Education Staff Development Guest Professionals & Artists-in-Residence Unrestricted Grants – the total amount of unrestricted grant awards will be $6,500. Unrestricted Amount Requested: $ 2353.54 School year in which funds will be used 2008 - 2009 2009 - 2010 Please note that this is a competitive grant process, not all grants submitted will be funded. 2
  • 3. Grant Deadline – January 23, 2009 Project Title: Student Success Center Successful Communication Initative Project Narrative Please use pages 4 & 5 to provide the information requested below. (Do not exceed two pages) 1. Briefly describe your project. State clearly how the project relates to the funding category you have chosen. 2. What are the goals and objectives of your project? List sequential activities and approximate time frame for attaining the goals. How will you measure the success of the project? 3. What is the relationship of your project to the curriculum? Reference PDE academic standards when applicable. In what way is this an innovative and enriching approach to teaching and learning? 4. How will you evaluate the impact and success of the project? What are the follow-up activities for students? Describe the long term impact on teaching and learning. 5. Describe all equipment (nonexpendable items over $500) requested for this project. How is the equipment essential to the goals of your project? Project Budget Please provide a breakdown of expenses for your project on page 6. Breakdown expenses by these categories: Equipment – nonexpendable items that exceed a cost of $500 per item. Projects that require equipment purchases must include a brief description of the equipment (in the narrative) as well as supporting documents (catalogues, etc.). Materials & Supplies – expendable/consumable items necessary for the project Professional Development/Presenter Expenses – fees and travel expenses related to bringing in outside professionals/ presenters. Projects that require outside presenters or personnel must include resumes and/or organizational information about the presenters. Travel – student, teacher, and administrator related travel costs Other expenses – everything else 3

Grant Deadline – January 23, 2009 Project Title: Student Success Center Successful Communication Initative Project Narrative: (page 1) The Student Success Center (SSC) is in a central location in our high school and is bustling with activity-- averaging 435 student visits per week. Each week we host multiple visits from colleges, the military, professional guests for our Lunch & Learn series, hold informational seminars to share information about career planning, course selection, college application processes, how to write a resume, how to find an internship, job acquisition support services, etc. We also promote special opportunities for our students at the SSC. Students learn about Pathways field trips to area businesses and to local colleges at the SSC. They also learn about community service opportunities. The funding request for this project is from Media Literacy. We expect to involve our students, staff and guests to interact and create using this medium. The goals of this project are as follows: (1) to serve as a communication center, a smart board will be placed in an area where students can see it and it's messages (2) the smart board will be avaliable to guests to use as they talk to students (this will work especially well for our college representative visits who can show students their websites and how to apply to college and our Lunch & Learn guests will be able to use PPT presentations or navigate the web to show students current workplace projects (3) be a tool for students to use as they present (4) be a tool for counselors as they work with small groups (i.e. training for use of Bridges and Naviance Family Connection--see attached), (5) be avaliable to small groups of students working on project planning. The platform will be used daily as an information source for students to observe as they walk by the SSC and look into the space. It will be used by college reps, professional speakers and staff presenting on an as needed basis. Success will be measured by increased activity in the SCC (student attendance and number of functions hosted). The functions and activities in the SSC help HHHS comply with the PA Career Education and Work (CEW) standards (www.pacareerstandards.com). All functions and activities in the center support and hold together the scope and sequencing of our CEW program. The SSC activities and programing connect all of the pieces of our program from 9th and 10th grade CEW standards infusion to the 11th grade Pathways Seminar and 12th grade community learning experiences such as Internship and Service Learning (see attachments). We use the Bridges program (www.bridges.com) for our students to build electronic portfolios. The portfolio is started freshman year and is worked on every year through high school graduation. We expect to see personal growth statements and reflections that will result, in part, to the exposures and activities students have had in the SSC. Our students will all write a personal transition plan explaining their post- high school plans and how those goals will be achieved. The plans will be presented to their teachers, parents and school counselors. For this project we are asking for: (1) NEC NP200 - DLP projector, (2) SMART Board 500 Interactive Whiteboard and (3) Balt Rolz 2 Mobile Conference Center. This equipment will allow for the presentations and uses described above. The moblie conference center will allow for movement of projection area and provide security. 4
