BIOGRAPHY: Dr. Michael Diaz joined Florida Cancer Specialists & Research Institute (FCS) in 2011
and currently serves on the FCS Executive Board and as Director of Patient Advocacy for the practice. In
addition, he serves on the Board of Directors and as President for the Florida Society of Clinical Oncology
(FLASCO). Dr. Diaz also provides leadership on the Board of Directors for Community Oncology Alliance
(COA), serving on the Payment Reform Task Force working to champion Medical Oncology Health Care
Reform. He is active on the American Society of Clinical Oncology (ASCO) Payment Reform Work Group
and was recently appointed to ASCO's Clinical Practice Committee. Dr. Diaz donates many hours as an
active Medical Missionary and has participated in numerous medical missions to Kingston, Jamaica.

BIOGRAPHY: Bruce E. Johnson, MD is the Chief Clinical Research Officer at Dana-Farber Cancer
Institute, Institute Physician at Dana-Farber Cancer Institute and Brigham and Women’s Hospital, and
Professor of Medicine at Harvard Medical School. His laboratory-based research is devoted to testing
novel therapeutic agents for their efficacy against lung cancer and other thoracic malignancies.
Dr. Johnson is a member of the American Association of Physicians. He served on the American Society
of Clinical Oncology (ASCO) Board of Directors from 2008-2011, received the ASCO Cancer Foundation’s
Translational Research Professorship in 2008, and was selected as a Fellow of the American Society of
Clinical Oncology in 2012. Dr. Johnson is currently serving as ASCO President-Elect and will begin his
term as President in June of 2017. He also was awarded the International Association for the Study of
Lung Cancer (IASLC) 2010 Scientific Award, given to an IASLC scientist for “life-time scientific
contribution in thoracic malignancy research and who has also contributed to the organization’s
development.” He was one of the leaders of the team awarded the American Association for Cancer
Research (AACR) 2010 Team Science Award that “recognizes an outstanding interdisciplinary research
team for its innovative and meritorious science that has advanced or likely will advance our fundamental
knowledge of cancer or a team that has applied existing knowledge to advance the detection, diagnosis,
prevention, or treatment of cancer.”
Dr. Johnson received his Doctor of Medicine from the University of Minnesota, did postgraduate training
at the University of Chicago and the National Cancer Institute, and came to the Lowe Center in 1998 after
serving for six years as the head of the Lung Cancer Biology section of the National Cancer Institute’s
Medicinean

Biography: Dr. Raez is the Medical Director of Memorial Cancer Institute (MCI) at Memorial Health
Care System (MHS) and Director of the Thoracic Oncology Program. He is also Clinical Associate
Professor of Medicine at Florida International University (FIU) and Visiting Professor of Medicine at
Cayetano Heredia University in Peru. He is also an Affiliate Associate Professor of Clinical Biomedical
Science for Florida Atlantic University (FAU).
He designs phase I-III clinical trials with new chemotherapeutic agents and combinations. Dr. Raez does
translational research in the areas of cancer vaccines. He has been funded by NCI and the pharma
industry. He has given oral presentations and lectures in national and international meetings in the US,
Europe, Latin-America and Asia.
He has been the International Chair of the 2014 IASLC-Latin-American Meeting (LALCA); Chairman for
the "Miami Best of ASCO 2014", Chair and Founder of the "Miami Cancer Meeting (MCM)" since 2002,
Co-Chairman at the "Puerto Rico Fall Cancer Symposia (FCS)" since 2010.
He is American Board Certified in: Internal Medicine, Medical Oncology and Geriatric Medicine. He is
board eligible in Hematology. He is a member of AACR, ESMO, ASCO, IASLC, ALLIANCE, NCCTG,
ACCP, ACP, ACSG, and FLASCO among other institutions.

Eric Harris, DO
BIOGRAPHY: Dr. Harris is board certified in hematology/oncology. He trained at the University of
Florida in Gainesville where he served as Chief Fellow. He is currently employed by Florida Cancer
Specialists at a busy practice in the Daytona Beach area.
He has been very active in FLASCO serving initially as a fellow liaison during his training and currently
serves on the Board of Directors and on the Program Committee. He is a strong advocate for community
clinical research and is an active participant in the FCS-Sarah Cannon partnership where he is the
Principal Investigator on several clinical trials.

BIOGRAPHY: Edgardo S. Santos, MD, FACP, is a well-established thoracic surgeon with an impressive academic career and
a remarkably diversified professional journey. Educated in Panama and the United States, he is currently performing
at an outstanding level of quality in several key roles across Florida. Dr. Santos is the Medical Director of Cancer
Research and Co-leader of the Thoracic and Head & Neck Cancer programs at The Eugene M. & Christine E. Lynn
Cancer Institute of Boca Raton Regional Hospital in Boca Raton, Florida. Furthermore, he is a board certified
oncologist with the Center for Hematology-Oncology in Boca Raton, Florida, and serves on staff of several local
hospitals, including Bethesda Hospital in Boynton Beach, Florida, and Delray Medical Center in Delray Beach,
Florida. Established in 1995, The Center for Hematology-Oncology is on a mission to provide high quality, state-of-
the-art, medical care to patients with cancer and blood disorders. Dr. Santos also has an active academic life
through his roles as an Affiliate Associate Professor of Biomedical Clinical Medicine at Charles E. Schmidt College
of Medicine of Florida Atlantic University. He is a fellow of the American College of Physicians and has held
numerous and varied positions throughout his career with well-established education and healthcare providers,
such as the University of Miami Leonard M. Miller School of Medicine in Miami, Florida, Tulane University Health
Sciences Center - Tulane University School of Medicine in New Orleans, Louisiana, and Southeast Louisiana
Veterans Healthcare System, also in New Orleans. Dr. Santos areas of expertise are hematology-oncology, thoracic
oncology, and lung cancer. As a hematologist and oncologist, he deals with the diagnosis, prevention, and treatment
of various types of malignances, which entails therapy, pathology of cancer in a person, screening, and palliative

Edgardo S. Santos, MD, commenced his foray in medicine in 1994 when he acquired his doctorate of medicine from
the University of Panama School of Medicine in Panama City, Panama, his native country. He additionally
completed his first year of internship at Complejo Hospitalario Metropolitano in Panama City, second year of his
internship at Santiago Regional Hospital in Veraguas, Panama, and his internal medicine residency at Complejo
Hospitalario Metropolitano. In 1998, Dr. Santos relocated to the United States where he served additional internal
medicine internship and residency training at Jackson Memorial Medical Center in Miami, Florida. He also
completed fellowship training in hematology and medical oncology at the University of Miami & Jackson Memorial
Medical Center, Sylvester Comprehensive Cancer Center. Dr. Edgardo S. Santos is certified in internal medicine and
holds sub-specialty certifications in hematology and medical oncology from the American Board of Internal
Medicine. A distinguished member of the International Association for the Study of Lung Cancer, he is also a
prolific author with many of his works published. Dr. Santos attributes his success to his unwavering dedication and
hard work. Furthermore, he has recently been selected by The International Association of HealthCare
Professionals (IAHCP) to be published in the premier publication of The Leading Physicians of the World. An avid

Michel Velez, MD
BIOGRAPHY: Dr. Michel Velez is a board-certified hematologist/oncologist. Dr.
Velez earned his medical degree from the Universidad del Norte, Colombia, after
which he became involved in breast cancer research as a research associate at the
Cancer Research Network and, subsequently, at Memorial Cancer Institute. He then went on to complete
his internal medicine internship and residency at the University of Miami/Miller School of Medicine at
JFK Medical Center and Veterans Affairs Medical Center Regional Campus, West Palm Beach. He
completed his hematology-oncology fellowship at Jackson Memorial Hospital/University of
Miami/Miller School of Medicine in 2014, where he served as chief fellow.
Dr. Velez serves as Principal Investigator in multiple clinical trials for the treatment of breast cancer,
which some have ultimately led to the approval of new therapies that have helped change the treatment
of this disease.
“Current Management of Axillary Disease in Breast Cancer”

John Kiluk, MD, FACS
Moffitt Cancer Center
BIOGRAPHY: Dr. John Kiluk is a surgical oncologist specializing in breast cancer at the
Moffitt Cancer Center in Tampa, Florida, where he has practiced for the last 10 years. He was
voted by his patients and peers as Moffitt's Physician of the Year in 2011. From an early age, Dr.
Kiluk accompanied his physician father on hospital rounds and learned the rewards of helping
patients during a difficult time in their lives. Now, as a Moffitt breast surgeon, he values the
opportunity to guide and support patients through every step of the process, from initial diagnosis through
recovery.

“Advances in Radiation Therapy in Breast Cancer”
OBJECTIVES:
1. To review role of radiation in breast conserving therapy.
2. To discuss who may and may not benefit from radiation therapy.
3. To discuss future implications and forward directions of radiation therapy in breast conserving therapy.
Julie Greenwalt, MD
Baptist Health-MD Anderson
BIOGRAPHY: Dr. Greenwalt was born and raised on the Space Coast and has lived
in Florida all of her life. She is blessed with a wonderful, supportive husband and a
son who brings joy to her life daily. Her husband is a dentist and owns his own
practice in Jacksonville. Being a doctor has been her dream since she was a child. Dr.
Greenwalt has always been passionate about helping patients and felt oncology was
the perfect place to do this, as it allows her to support and encourage her patients on this journey. Being a
radiation oncologist allows her to combine two of her greatest interests; research and technology, to drive
medicine forward in hopes of helping patients live longer cancer-free lives. She is excited about the future
of her field and the methods which will be used to see more patients cured.
Dr. Greenwalt is blessed to work at Baptist MD Anderson where the mindset is on giving back to their
patients and their community. She is personally passionate about giving back, both locally and globally.
She and her husband volunteer at their church and participate in annual medical and dental mission
trips to countries such as Nicaragua. They feel honored to use their professional skills in order to help
those in less fortunate situations.

Ana Cristina Sandoval-Leon, MD

BIOGRAPHY: Dr. Sandoval-Leon earned her medical degree Summa Cum Laude from
Universidad Internacional del Ecuador. She then completed her Internal Medicine Residency at
Jackson Memorial Hospital/University of Miami. Dr. Sandoval continued her education there for
her Hematology-Oncology fellowship and is now the Chief fellow.

“Recent Advances in the Understanding and Management of Patients
with Colorectal Cancers: A Medical Oncologist’s Perspective”

2. To narrate the array of options now available for patients with advanced/metastatic colorectal cancers.
3. To emphasize the importance of genetic testing of the patient, and the genetic testing of the tumors.
Pashtoon Murtaza Kasi, MD

BIOGRAPHY: Dr. Kasi is an Assistant Professor in Oncology at the Mayo Clinic,
Jacksonville, Florida with a strong interest gastrointestinal oncology, immuno-oncology
and cancer genetics. He is a medical oncologist by training. Within GI oncology, Dr.
Kasi’s focus is more on patients with colon and rectal cancers. His goal is to continue working on the
clinical as well as research aspects of gastrointestinal oncology in order to bring new treatments to the
clinic for patients with colorectal cancers; with the goal of providing better clinical care to these patients.
“Surgical Management of Rectal Cancer: Are we there yet?”
Othon Wiltz, MD
Medical Director and Chief, Colorectal Surgery Program
BIOGRAPHY: Dr. Othon Wiltz is the director of Colon and Rectal Surgery at Memorial
Healthcare System. Through minimally invasive and innovative surgical techniques, he
treats patients with a variety of conditions, including colon cancer, hemorrhoids and fissures,
inflammatory bowel disease, diverticulitis, and accidental bowel leakage.
Dr. Wiltz decided to follow in the footsteps of his father, who also was a colon and rectal surgeon, after
seeing a patient thank him for saving her life. Now, Dr. Wiltz is carrying out his father’s legacy by
changing the lives of patients at Memorial Healthcare System.
While Dr. Wiltz treats patients with a variety of conditions – from life-threatening cancers to others that
simply cause discomfort – his approach to patient care is always the same. He treats patients as he would
like to be treated and works with them to find out how he can make a difference in their lives.
Dr. Wiltz earned his medical degree at Columbia University College of Physicians and Surgeons, and
completed specialized training at Deaconess Hospital in Boston and University of Minnesota. Prior to
joining Memorial Healthcare System, Dr. Wiltz was chief of Colorectal Surgery at Virginia Hospital
Center, where he created a successful Colon and Rectal Surgery program and mentored future surgeons.

“RECTAL CANCER: The Radiation Oncology Perspective”
OBJECTIVES:
1. Understand how radiotherapy is integrated into the multidisciplinary treatment of rectal cancer.
2. Review data regarding neoadjuvant chemo-radiotherapy for rectal cancer.
3. Explore new directions and uses of radiotherapy for rectal cancer.
Brendan Prendergast, MD
Health First Cancer Institute
BIOGRAPHY: Dr. Prendergast is a radiation oncologist at Health First Cancer
Institute (formerly Space Coast Cancer Center), a multi-specialty oncology group
serving all of Brevard County. Dr. Prendergast joined the practice in July 2013 and
treats patients at both the Viera Cancer Center and the Titusville Cancer Center.
Dr. Prendergast is a Phi Beta Kappa graduate of the University of Notre Dame in 2004. He then
graduated from the University of Florida, College of Medicine in 2008, where he was selected to the
Alpha Omega Alpha Medical Honor Society. He completed his internship at Presbyterian- St. Luke’s
Medical Center in Denver Colorado in 2009, followed by a Clinical and Translational Science Program at
the University of Alabama, Birmingham (UAB) in 2011. Dr. Prendergast completed his residency in
Radiation Oncology at UAB in July 2013.
Currently, Dr. Prendergast is a member of the Florida Society of Clinical Oncology (FLASCO) where he
serves on the Board of Directors, American Society of Therapeutic Radiation Oncology (ASTRO), and the
American College of Radiation Oncology (ACRO), where he was appointed to the Resident Committee
from 2011- 2013.
“Case Presentation: Rectal Cancer”
Aaron C. Denson, MD

BIOGRAPHY: Dr. Denson is from Mobile, Alabama. Dr. Denson went to college and
medical school at Tulane University in New Orleans, Louisiana. He completed his
internal medicine residency at University of South Florida in Tampa, Florida, where he
continued his education and is currently a hematology/oncology fellow. Dr. Denson’s research interests
include phase 1 clinical trials and novel therapeutic development.

Throughout the year FLASCO offers the opportunity for FLASCO member Oncology
Focused Trainees to submit applications for Travel Awards. Thank you for your desire
to learn! It is our sincere hope that FLASCO is providing a great service to FLASCO’s

Society of Gynecologic Oncology Annual Meeting
Amanda Stephens, DO
Florida Hospital Gynecologic Oncology
Dr. Amanda Stephens is a 1st-year GYN oncology fellow at Florida Hospital in
Orlando. She received her OB/GYN training in Farmington Hills, Michigan at
Beaumont Hospital, Botsford Campus, medical degree at Lincoln Memorial University
DeBusk College of Osteopathic Medicine, and Bachelor of Science degree in psychology at the University
of Michigan. Dr. Stephens discovered her passion for GYN oncology late in her residency when she
completed her 4th-year rotation and was exposed to complicated, high-risk surgical cases, innovative
research in women's health, and possibly the most appreciative patient population that exists. The
connections made with these patients are what continue to drive her in her training and pursuit of
advancements in identifying and treating gynecologic cancers. Dr. Stephens, her husband, and two
children very much enjoy sunny Florida and hope to stay for years to come. She is very grateful to
FLASCO for the financial assistance to attend the annual meeting of the Society of Gynecologic Oncology!
Ketura Wisner, DO
Florida Hospital Gynecologic Oncology
Preya Wisner, D.O is in her third year in the Florida Hospital Gynecologic Oncology
Fellowship. She is from Oregon, completed medical school in California, and
completed her Ob/Gyn residency in Chicago.
Dr. Wisner is dedicated to the Gynecologic Oncology profession. She feels attending
the SGO Annual Meeting on Women's Cancer was invaluable explaining, “The SGO meeting had many
informative presentations about the latest science technology in the field and treatment in the areas of
chemotherapy, radiation therapy, radical surgeries, and palliative care.”
Dr. Wisner is dedicated to participating in the Society of Gynecologic Oncology and the Florida Society of
Clinical Oncology organizations throughout her career. She thanks the FLASCO members for the
generous Travel Award to attend this meeting.

Society of Surgical Oncology Annual Meeting
Sonia Orcutt, MD
Dr. Orcutt is a second year fellow in the Complex General Surgical Oncology program
at the Moffitt Cancer Center. She completed her general surgery training at Baylor
College of Medicine in Houston, TX, prior to moving to Tampa, FL. She has been
interested in surgical oncology since medical school, and is happy to be nearing the end
of her training and about to embark on the next phase of her life. Dr. Orcutt is joining a practice with the
University of Illinois at Peoria in a few months and will be the director of Surgical Oncology there. She
will have a broad-based practice, managing everything from cutaneous malignancies to hepatobiliary
malignancies to sarcoma. She appreciates the education she has received while in Tampa, and thank the
patients at Moffitt Cancer Center for their education.
The primary conference for her specialty is the Society of Surgical Oncology Annual Cancer
Symposium. Each year, top academic surgeons from around the world meet to present groundbreaking
research in the field. The meeting provides a forum to discuss new ideas and innovations in surgical
oncology, and this year was no exception. New techniques and new therapies for managing patients with
cancer were presented, and researchers presented their ideas on how to adapt to changing fields. Dr.
Orcutt believes, “the ability to meet giants in the field and network with other surgical oncologists at this
meeting, including meeting next year's president, will be very helpful for future collaborations and
creating further opportunities.”

Cesar E. Ochoa Perez, MD
Mayo Clinic Florida
Dr. Ochoa is a Hematology/Oncology Fellow at Mayo Clinic. He received his medical
degree from Tecnologico de Monterrey School of Medicine in Monterrey, Mexico. His
postgraduate training includes an Internal Medicine Residency at Wayne State
University – Henry Ford Health System in Detroit, MI and a Postdoctoral Research
Fellowship at MD Anderson Cancer Center in Houston, TX where he studied the innate
immune signaling pathways and focused on the role of IL-6/STAT3 on a KRAS-mutant
model of lung carcinogenesis.
Dr. Ochoa is currently involved in identifying markers of response to immunotherapy in NRAS-mutant
melanoma and the development of a model that incorporates immune checkpoint blockade to
intraperitoneal chemotherapy for the treatment peritoneal carcinomatosis from colorectal and ovarian
cancer. His career interests include translational research and solid tumor immunotherapy.

the Associate level of FLASCO membership.

Peter Adeosun

Peter Adeosun is a PD 1 student at the University of Florida, Orlando Campus. He has a
B.Sc. degree in Chemistry from University of Ife, Nigeria and MS in Environmental
Science from the Florida Institute of Technology, Melbourne. He was a high school
Chemistry, Physics and Mathematics teacher in Fort Myers until recently when he
rediscovered the passion that he has always had for healthcare since he was a child. He hopes to be
involved in direct patient care as a clinical pharmacist in the state of Florida after graduation.
Mr. Adeosun enjoys research and was involved in research activities both at undergraduate and graduate
degree levels. He is currently at the literature review stage of researching possible impact of genetic
polymorphism on drug selection and dosage in black mCRPC patients.

Kevin Corneille. I am currently a first-year student attending Nova Southeastern
University’s College of Pharmacy. Like many, my first introduction to cancer was a
melancholy one. However, I began to develop a marked interest in the field while taking
a course entitled “Biology of Cancer” taught by Dr. James X. Hartman. While thinking
about the various fields of Pharmacy, I recalled how interesting and challenging cancer and thus
Oncology can truly embody. I am sure that I want to pursue a career in Pharmacy academia. Moreover,
my goal is to pursue a PGY-1 and 2 residency, become board certified in Oncology and work in a setting
where my unique talents and skills are both needed and appreciated.

Aixa Guadarrama
Pharmacy
Aixa Guadarrama’s passion for the field of oncology began early in her college
experience. During her undergraduate studies, she worked for over 3 years in cancer
research. She had the privilege of presenting some of the research findings she worked
on at a poster presentation for the 2013 American Association for Cancer Research
(AACR) symposium for undergraduate students. As she continued on her journey toward her career in
pharmacy, she knew she wanted to continue in the field of oncology. Aixa completed her health system
rotations during her third year in a specialized oncology hospital. As a 4th year student, she is currently
completing a 3 month block rotation in a specialized oncology hospital setting as well. Ms. Guadarrama
has the privilege of working in rotations in pediatric oncology, oncology clinical research, and oncology
administration. She is looking forward to graduating in May with her Pharm. D. and pursuing her goal as
a Doctor in Pharmacy in oncology. Knowing that she has such a great passion in the field for clinical
oncology, she joined FLASCO to be able to connect with fellow health professionals that share in the
same desire to lead and advance the field of clinical oncology. Building a strong network in a field that
she feels committed to supporting as a pharmacist will bring great opportunities to communicate and
collaborate with the clinical oncology community.
Andrea Mezentsef
Palm Beach Atlantic University
Andrea Mezentsef is a fourth year student at Palm Beach Atlantic University’s Lloyd L.
Gregory School of Pharmacy (GSOP) in West Palm Beach, Florida. She was born in
Miami, Florida but has had the opportunity to live in various countries including Costa
Rica, Argentina and Brazil.
Prior to pharmacy school, Andrea worked as a pharmacy technician at Walgreen’s and
that is when she decided to pursue a career in pharmacy due to enjoying interacting with patients and
learning about various medications and their uses. Since starting pharmacy school, leadership, serving in
the community and advocating for the pharmacy profession have been her main priorities. She has been
involved in participating and organizing various community outreach and health fair events and has also
been involved in medical missions where she served during her first year pharmacy school in Honduras
and in Uganda during her second and third year of pharmacy school.
In the past, Andrea served as the chapter president of the Academy of Managed Care Pharmacy
organization and currently is this year’s Christian Fellowship Pharmacist International’s national student
retreat coordinator. Andrea is also engaged in multiple professional organizations such as, American
Society of Health-System Pharmacists and American College of Clinical Pharmacy, both of which she has
made poster presentations at national conferences.
In addition to extracurricular activities, due to her academic success she has also made Dean’s List
throughout pharmacy school and was inducted into the Rho Chi pharmaceutical honor society and the
Phi Lambda Sigma Pharmacy Leadership Society.
Andrea interns and works weekends at Memorial Regional Hospital and is currently completing her
advanced pharmacy practice experience rotations. She is looking forward to graduation and plans to
pursue a residency and to continue being involved in the growth of the pharmacy profession.

Racquel Reese, Pharm. D.
Racquel Reese, Pharm. D., obtained a Bachelor of Science in Physiology and
Neurobiology from the University of Maryland College Park in 2008. She went on to
earn her Doctor of Pharmacy from the University of Maryland Eastern Shore in May
2015. Racquel completed a PGY-1 Pharmacy Practice Residency at Peninsula Regional
Medical Center in Salisbury, MD in June 2016. She is the current PGY-2 Oncology Pharmacy Resident at
Memorial Regional Hospital/Memorial Cancer Institute.
Nelly Ruiz
Pharmacy
Nelly Ruiz is part of the Larkin University College of Pharmacy Class of 2019 in Miami,
FL. She is a determined first generation student. Prior to commencing pharmacy
school, she was a pharmacy technician at a Specialty Pharmacy with a focus in oncology
patients and services. Since then, her goal to be an oncology pharmacist was set.
Having completed her Introductory to Pharmacy Practice rotation at the UM Lennar
Foundation Medical Center, a part of the Sylvester Comprehensive Cancer Center, has further cemented
that goal.

FLASCO consists of over 2,500 members that include physicians and their
supporting staff, allied healthcare, and industry representatives that represent
the oncology professionals of Florida. FLASCO will continue to be recognized as a
nationally respected state professional society as we stand together as “the
Voice of Oncology in Florida”.
