This document provides an agenda and speaker biographies for the "Rapid Integration Course" event hosted by the Florida Society of Clinical Oncology on September 30, 2017 at the Mayo Clinic Florida. The one-day course aims to offer relevant and practical training to help new physician assistants and nurse practitioners more quickly integrate into the oncology practice setting. The agenda includes sessions on hematology/oncology basics, pharmacology, pathology, symptom management, emergencies and other topics. Presenters are physicians, nurse practitioners and other experts from Mayo Clinic, Moffitt Cancer Center, Cleveland Clinic and other institutions. The event chairman is Dr. Winston Tan from Mayo Clinic Florida and the vice chairman is Terry Gruchow from Moffitt Cancer Center

The mission of the Rapid Integration Course is to offer a course that is relevant,
practical and patient centered that could be used as a durable and long term project that
will positively impact the speed in which entry level oncology physician assistants and
nurse practitioners integrate into the oncology practice setting.

BIOGRAPHY: Winston Tan, M.D., is focused on translating basic science principles into tangible
treatment options for patients with cancer. He is interested in the importance of molecular pathways, such as
those related to the estrogen receptor, vascular endothelial factor and HER-2 receptor, in the pathogenesis
and therapy of cancer. He has focused on developing novel hormonal strategies to abrogate the pathway of
resistance in breast cancer, prostate cancer and bladder cancer, identifying molecular markers that will
predict response and toxicity to targeted treatment, and identifying and developing novel targets for kidney
cancer, including predictors of treatment response and resistance. The development of novel targets of
therapy would lead to oral and less toxic treatments for cancer. These treatments would also be able to lead to
meaningful improvement of quality of life, prolongation of the time to cancer progression and hopefully
improved survival. Dr. Tan’s professional highlights are his leadership rolls as Board Member, Chair of the
Membership Committee, and member of Program Committee in the Florida Society of Clinical Oncology. He
is a Fellow within the American College of Physicians, and Member and a part of the Membership and
Survivorship Committees of the American Society of Clinical Oncology.

BIOGRAPHY: Terry Gruchow, PA-C, joined Moffitt Cancer Center as an Advanced
Practice Professional with Department of Internal and Hospital Medicine in 2006. A
graduate of the University of Wisconsin and Lock Haven University (Pa), his clinical
responsibilities include the Moffitt Direct Referral Center (the Center’s Urgent Care clinic) and the Inpatient
Wards. His other responsibilities include Administrator of the Moffitt PA Student Mentoring Program and
the Moffitt Student Shadowing Program. He is Adjunct Faculty at Barry University and South University. A
member of The American Academy of Physician Assistants, Association of Physician Assistants in Oncology
and Society of Hospital Medicine, he is the West Central Region Director for the Florida Academy of Physician
Assistants. Terry is also on the Moffitt APP Council, serves as the PA Liaison to the FLASCO Board of
Directors and is a member of the South University PA Program Advisory Board.

BIOGRAPHY: Dr. Jessica McMillan is an Advanced Registered Nurse Practitioner at Mayo Clinic
Florida in the Division of Hematology/Oncology. She has over 19 years of experience as a leader,
educator and patient care advocate. She also works for Brooks Rehabilitation Hospital caring for
patients multiple disabilities incurred by motor vehicle accidents, traumatic brain injuries, spinal cord
injuries, and an array of other debilitating illnesses that require rehabilitation and care. In addition to
working as a NP at Mayo Clinic and Brooks Rehabilitation Hospital, she continues to teach for University
of North Florida, empowering new graduate nurses to provide patients with care, compassion and expert
care always putting the needs of the patient first. She obtained her Nurse Practitioner Certificate at
University of North Florida and is expecting to graduate with her Doctorate of Nursing Practice, with a
sub-specialty of Palliative Care on July 24, 2015. She transitioned from caring to patients in the
Intensive Care Unit to Oncology 8 years ago because she wanted to develop relationships with her
patients and have a positive impact on their lives as they dealt with life limiting illness in the oncologic
setting.

BIOGRAPHY: Lindsay Williamson is board certified as an Advanced Oncology
Certified Nurse Practitioner as well as an Adult Nurse Practitioner. She received her BSN
at West Chester University in West Chester, Pennsylvania and her MSN at La Salle
University in Philadelphia, Pennsylvania. Lindsay has been an Oncology Nurse for 17
years with 9 of those years in the role of Nurse Practitioner. She has worked in a variety of settings
including inpatient and outpatient as well as community based and academic based. Also, she has worked
in a variety of roles including Oncology Staff Nurse, Infusion Nurse, and ARNP in an academic setting
and community practice, Pharmaceutical Sales Representative and Clinical Operations Manager of the
Lab Draw and Infusion areas at Moffitt Cancer Center. She is currently working with Florida Cancer
Specialists in New Port Richey, Florida as an Oncology Nurse Practitioner. Lindsay is a member of the
Advanced Practice Society for Hematology and Oncology and the Oncology Nursing Society.

Kathrine Tzou, MD

BIOGRAPHY: Dr. Katherine Tzou is an Assistant Professor of Radiation Oncology at
Mayo Clinic Florida. She specializes in radiation therapy for genitourinary cancers,
gynecologic cancers, breast cancer, and skin cancer. Dr. Tzou completed her Radiation
Oncology Residency in 2012, at the Mayo Clinic College of Medicine. In 2008, Dr. Tzou
completed her Preliminary Internal Medicine Year at Fletcher Allen Health Care,
University of Vermont. Dr. Tzou attended medical school at The Ohio State University College of
Medicine. She serves as a panel member of the National Comprehensive Cancer Network Testicular
Cancer Guidelines Committee, chair of the Mayo Clinic Florida Radiation Oncology Research Executive
Committee, Co-chair of the Mayo Clinic Genitourinary Disease Site Group, member of the American
Society for Radiation Oncology (ASTRO) Membership Committee, and associate residency direction for
Radiation Oncology at Mayo Clinic Florida.

BIOGRAPHY: Dr. Shahla Masood is a Persian born physician, who currently holds
the positions of Professor and Chair of the Department of Pathology at University of
Florida College of Medicine – Jacksonville and Chief of Pathology and Laboratory Medicine at UF Health
Jacksonville. She is also the Director of the Cytopathology and Breast Pathology Fellowship Training
Program. In addition, Dr. Masood is the Medical Director of UF Health Breast Center. An internationally
recognized expert in breast cancer diagnosis and prognosis, Dr. Masood has fostered the concept of an
integrated multidisciplinary approach in breast cancer care, research, and education. She is the founder
and Editor-in-Chief of The Breast Journal, the founder and past president of the “International Society of
Breast Pathology,” the Director of the “Annual Multidisciplinary Symposium on Breast Disease”, and
“The Breast Cancer Public Forum”.
Dr. Masood is heavily involved in the study of minimally invasive procedures such as fine needle
aspiration biopsy and ductal lavage in providing diagnostic and prognostic information in high risk and
breast cancer patients. She defined the cytomorphology of high-risk proliferative breast disease in the
early 1990s and has pioneered the concept of cytomorphology as a breast cancer predictor.
Dr. Masood is the author of several textbooks, book chapters, and numerous publications. She is a
frequent speaker at national and international symposiums and consensus meetings. She is also a
member of the board of trustees of several prestigious scientific societies and organizations at local,
regional, national and international levels.
Dr. Masood has received numerous awards and recognition for her scientific work, her contribution to
advancing global breast health education and her efforts to improve the quality of breast health care. She
is the recipient of “The 2010 Breast Health Global Initiative Award,” “Courage to Teach Award” from
National Accreditation Council for Graduate Medical Education, and Florida Times Union “Eve Award,”
to name a few. She has been named as one of the Top Doctors in America, Top Doctors in Cancer and one
of the 20 Top Most Influential Professors in Oncology at an international level. In 2015, Dr. Masood was
honored in Rome, Italy by Susan G. Komen Italia, a partnership between Susan. G. Komen and the
Catholic University in Rome for her widespread contributions to breast cancer advocacy, and initiation of
the Italian “Race for the Cure” in 2000.
Dr. Masood is a patient advocate, a partner in community affairs and an accomplished artist and gourmet
chef. She is the mother of two accomplished physicians, Dr. Ali Kasraeian, urologist, and Dr. Sina
Kasraeian, orthopedic surgeon.

BIOGRAPHY: Dr. Chakra Chaulagain completed his Hematology/Oncology
Fellowship at Tufts University, New England Medical Center in Boston, MA in 2014. In
2008, Dr. Chaulagain completed his Internal Medicine residency at John H. Stroger Jr.
Hospital of Cook County in Chicago, IL. Dr. Chaulagain attended medical school at
Tribhuvan University in Maharajgunj, Kathmandu. Dr. Chaulagain is board certified in
Internal Medicine, Medical Oncology, and Hematology. Dr. Chaulagain is an active member of the
Florida Society of Clinical Oncology, serving on the Board of Directors and the Membership and Program
Committees.

BIOGRAPHY: Dr. Ana Rosa Espinosa is the Administrative Director of the Oncology
Clinical Services at Memorial Cancer Institute. In her role she oversees the practice of all
nursing and ARNP’s. She is an exceptional Nursing Director with extensive education in
nursing as wells as business development skills. She has been in the oncology field for
most of her long nursing career, and is Oncology Certify. She has a Doctor of Nursing Practice from the
University of Miami, a Masters in Business from Nova Southeastern University and Bachelors in Nursing
from Barry University. She is a speaker, has published several nursing articles and has an interest in
nursing research. She is promoter of higher education and supporter of nursing research and evidence-
based practice. Prior to Joining Memorial Cancer Institute, she was the Nursing Director for the
Outpatient Clinics at Sylvester Comprehensive Cancer Center. She is a member of the Florida Nursing
Association and the Oncology Nursing Society. She is an active member in her community and is
involved in many cancer events.

Research at Memorial Healthcare System in Hollywood, FL. She received her BSN from
Mount Mercy College in Cedar Rapids, Iowa and MSN from Chamberlain College of
Nursing. She has been in research for the past 5 years working on studies related to all
cancers including but not limited to Lung, Breast, GI, GYN, GBM, Leukemia, Multiple
Myeloma, Lymphoma, Head and Neck, as well as Sarcomas and Melanomas.

BIOGRAPHY: Dr. Beau Toskich is a part of the radiology department at Mayo Clinic in
Jacksonville, Florida. Dr. Toskich has an interest in interventional oncology with an
emphasis on management of primary and metastatic liver cancer, portal hypertension,
central venous disease, and vascular malformations. Dr. Toskich earned his doctorate
from Florida State University College of Medicine. He continued his education at Shands Hospital at the
University of Florida completing his General Surgery Internship in 2008, Residency in Diagnostic
Radiology in 2012, and his fellowship in Vascular and Interventional Radiology in 2013.

The Florida Society of Clinical Oncology
FLASCO was formed as a statewide 501(c)(3) nonprofit organization committed to facilitating and
promoting multidisciplinary efforts to improve patient care in Florida. FLASCO continues to make
strides in promoting excellence in care of patients afflicted with cancer and hematologic disorders and
leads efforts to respond to policies and legislation that threaten to compromise patient access to quality
cancer care. FLASCO is gaining momentum as more professionals use us as the catalyst to develop
partnerships, share knowledge, and stand together to demonstrate strength and determination to move
forward in the battle against cancer.
FLASCO consists of over 2,800 members that include physicians and their supporting staff, allied
healthcare, and industry representatives that represent the oncology professionals of Florida. FLASCO
will continue to be recognized as a nationally respected state professional society as we stand together as
“the Voice of Oncology in Florida”.
