The Suriname Men United program aimed to reduce HIV and STI transmission among gay men and men who have sex with men (MSM) in Paramaribo, Suriname. The program distributed condoms and lubricant, provided information about HIV/STIs, and assessed attitudes towards condom use. They engaged with openly gay men as well as closeted bisexual men. The program organizers were proud that they could directly engage with the target group to learn about their attitudes and involve them in program planning.

The PRISM Programme was established in Botswana to develop and implement an HIV/STI prevention and human rights initiative for sexual minorities (MSM/WSW communities). The rationale is that involving at-risk communities in prevention programs empowers them to make informed choices. The objectives are to support public health efforts to prevent new HIV infections by 2016 through protecting health rights of sexual minorities and promoting non-discrimination and dignity. Mainstreaming efforts include sensitization and training of partner organizations, NGOs, journalists, police and prison officers to address issues like discrimination faced by sexual minorities.

The survey aimed to understand the risk factors threatening the sexual health of lesbians and women who have sex with women (WSW) in Durban, South Africa. It interviewed 60 lesbians and WSW aged 15-35, with 75% Black African and 25% Indian/Asian. Many respondents had come out to friends and siblings but not parents. Reasons for not coming out varied and included fear of rejection. Health issues found included alcohol use by over 68% and drug use like marijuana. Rates of STIs and violence in relationships were low but underreporting was an issue. Access to sexual healthcare faced challenges like stigma and lack of lesbian-friendly services.

A structural intervention was implemented at public sex venues in Amsterdam starting in 2000 to address rising STI rates among men who have sex with men. There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
