Joint working in community teams has developed across Scotland and across a range of care groups over a number of years. This workshop shares the learning from an award winning integrated team and explore some of the key, common messages for practice. It highlights challenges in developing the workforce, mainstreaming the approach and spreading this to other localities. BAS Poster
The Behavioural Advisory Sessions (BAS) were developed to support staff teams working with individuals with learning disabilities and low-level challenging behavior. The BAS provides an opportunity for direct support staff to discuss concerns with a community nurse and receive advice and recommendations to create interventions. It aims to give staff ownership over behavior protocols and increase consistency. To make the most of BAS, staff must bring all relevant information about the individual's behavior, assessments, support plans, and previous interventions. Appointments will be no more than one hour with up to six sessions available over multiple months. The individual being discussed will not attend to minimize anxiety.

Mindful Service
The Mindful Service provides outreach mental health services for 16-25 year olds in Surrey who are not accessing traditional services. It is run by Surrey and Borders Partnership NHS Foundation Trust. The service aims to engage hard to reach youth, provide consultations and training, work with other agencies, and prevent long-term mental health issues. It operates flexibly in Guildford and Redhill to meet the needs of homeless, substance using, socially isolated, and transitioning youth. Challenges include managing a large geographic area with few staff, prioritizing work, and engaging those most in need of support.
Caring Together presentation
This document outlines plans to integrate health and social care services in Wirral, England to better serve an aging population with growing long-term health conditions. Key points include: forming multi-disciplinary integrated care coordination teams across organizations to provide more coordinated care; using risk stratification to identify those most in need of support; and promoting self-management and independent living through resources like an online patient portal. Resource Social Impact Seminars
As part of the Cultural Commissioning Programme, New Philanthropy Capital (NPC) delivered a series of Social Impact Seminars aimed at arts and cultural organisations. These seminars took place in Birmingham, Leeds, Exeter, London and Peterborough between October and November 2016.
Many arts and cultural organisations need to show how their work contributes to social outcomes. These may be outcomes required by public service commissioners or outcomes wanted by funders.
These seminars aimed to help participants to identify, collect and interpret evidence which:
• Could be used to inform and influence funders and commissioners of their social impact
• Was realistic to collect, in keeping with the individuals and communities they work with
• Focus on current best practice and make use of existing research.
The Cultural Commissioning Programme runs until June 2016 and is funded by Arts Council England. It is delivered by NCVO in partnership with NPC (New Philanthropy Capital) and nef (New Economics Foundation).
Housing and Health: Working in Partnership
Tony Powell discusses New Charter Housing Group's efforts to improve health and promote healthy communities. Some key initiatives discussed include developing a health and wellbeing strategy focused on involvement, employment, health, and partnership working; implementing local interventions through organizations like Active Tameside; and creating a successful Healthy Living Scheme placed in GP surgeries that engaged over 4,000 residents and saved nearly £400,000 for the health economy.
General Practice Transformation Champions: Collaborative Practice
This document describes a new collaborative care model used in 30 UK general practice clinics. It involves inviting local community members to volunteer alongside practice staff to meet patient needs. An evaluation found that the model improved patient and staff well-being, sustainability, and care outcomes. Housing and Health: Working in Partnership

A new model of care for general practice, pop up uni, 10am, 2 september 2015
Expo is the most significant annual health and social care event in the calendar, uniting more NHS and care leaders, commissioners, clinicians, voluntary sector partners, innovators and media than any other health and care event.
Expo 15 returned to Manchester and was hosted once again by NHS England. Around 5000 people a day from health and care, the voluntary sector, local government, and industry joined together at Manchester Central Convention Centre for two packed days of speakers, workshops, exhibitions and professional development.
This year, Expo was more relevant and engaging than ever before, happening within the first 100 days of the new Government, and almost 12 months after the publication of the NHS Five Year Forward View. It was also a great opportunity to check on and learn from the progress of Greater Manchester as the area prepares to take over a £6 billion devolved health and social care budget, pledging to integrate hospital, community, primary and social care and vastly improve health and well-being.

Mindful Service
The Mindful Service provides outreach mental health services for 16-25 year olds in Surrey who are not accessing traditional services. It is run by Surrey and Borders Partnership NHS Foundation Trust. The service aims to engage hard to reach youth, provide consultations and training, work with other agencies, and prevent long-term mental health issues. It operates flexibly in Guildford and Redhill to meet the needs of homeless, substance using, socially isolated, and transitioning youth. Challenges include managing a large geographic area with few staff, prioritizing work, and engaging those most in need of support.
Caring Together presentation
This document outlines plans to integrate health and social care services in Wirral, England to better serve an aging population with growing long-term health conditions. Key points include: forming multi-disciplinary integrated care coordination teams across organizations to provide more coordinated care; using risk stratification to identify those most in need of support; and promoting self-management and independent living through resources like an online patient portal. As part of the Cultural Commissioning Programme, New Philanthropy Capital (NPC) delivered a series of Social Impact Seminars aimed at arts and cultural organisations. These seminars took place in Birmingham, Leeds, Exeter, London and Peterborough between October and November 2016.
Many arts and cultural organisations need to show how their work contributes to social outcomes. These may be outcomes required by public service commissioners or outcomes wanted by funders.
These seminars aimed to help participants to identify, collect and interpret evidence which:
• Could be used to inform and influence funders and commissioners of their social impact
• Was realistic to collect, in keeping with the individuals and communities they work with
• Focus on current best practice and make use of existing research.
The Cultural Commissioning Programme runs until June 2016 and is funded by Arts Council England. It is delivered by NCVO in partnership with NPC (New Philanthropy Capital) and nef (New Economics Foundation).
Housing and Health: Working in Partnership
Tony Powell discusses New Charter Housing Group's efforts to improve health and promote healthy communities. Some key initiatives discussed include developing a health and wellbeing strategy focused on involvement, employment, health, and partnership working; implementing local interventions through organizations like Active Tameside; and creating a successful Healthy Living Scheme placed in GP surgeries that engaged over 4,000 residents and saved nearly £400,000 for the health economy.
General Practice Transformation Champions: Collaborative Practice
This document describes a new collaborative care model used in 30 UK general practice clinics. It involves inviting local community members to volunteer alongside practice staff to meet patient needs. An evaluation found that the model improved patient and staff well-being, sustainability, and care outcomes. Expo is the most significant annual health and social care event in the calendar, uniting more NHS and care leaders, commissioners, clinicians, voluntary sector partners, innovators and media than any other health and care event.
Expo 15 returned to Manchester and was hosted once again by NHS England. Around 5000 people a day from health and care, the voluntary sector, local government, and industry joined together at Manchester Central Convention Centre for two packed days of speakers, workshops, exhibitions and professional development.
This year, Expo was more relevant and engaging than ever before, happening within the first 100 days of the new Government, and almost 12 months after the publication of the NHS Five Year Forward View. It was also a great opportunity to check on and learn from the progress of Greater Manchester as the area prepares to take over a £6 billion devolved health and social care budget, pledging to integrate hospital, community, primary and social care and vastly improve health and well-being.

Louca-Mai Brady's background includes research on embedding children and young people's participation in health services and research. There is a lack of evidence on how to ensure participation is meaningful, effective and sustained. Barriers include understanding participation, power dynamics, and challenges recruiting and sustaining involvement of diverse groups of young people. Through action research case studies in an NHS trust and clinical trial, Brady is exploring how to define, operationalize and embed participation at different levels and settings to overcome barriers and be meaningful, effective and sustainable. Flexible, local, youth-centered engagement has proven more successful than traditional advisory groups.

CLAHRC NDL aimed to spread research findings into practice through "Diffusion Fellows", who were staff seconded from partner organizations to work on CLAHRC studies and disseminate results. However, the diffusion fellow approach did not always work as intended, as some fellows or study teams were not a good fit, or organizations did not provide enough support. Overall, the diffusion fellow role helped build research capacity and networks, but other supportive factors are also needed within organizations and the wider system for research to successfully impact practice.

Healthwatch Stoke Annual meeting 2019
Healthwatch Stoke-on-Trent held its annual meeting for 2018/19. The organization works to gather people's views on health and social care services, with a focus on those who find it hardest to be heard, to help shape support. In 2018/19, Healthwatch worked with others on an information card for homeless people accessing GPs. It also engaged people for the NHS Long-Term Plan and identified key themes around access, communication, and specific conditions. Healthwatch will focus areas of prescriptions/pharmacy and community mental health services to understand experiences and identify improvements.
Strategic Clinical Network - Youth Mental Health Network Participation
This document discusses early suggestions for patient and public engagement (PPE) in the South East Coast Strategic Clinical Networks. The National Falls Programme in Scotland aims to reduce falls and harm from falls by establishing integrated falls prevention and management pathways across all local health and social care partnerships by 2014.

Positive involvement is associated with children's cognitive, emotional, and social development as well as decreased negative outcomes.
