They influence accessibility of healthcare among the American citizens (Shi & Singh, 2015). The government has embarked on efforts to ensure that there is improved health access to its people and continually increase the good being of its people. The paper explores how various attributes related to the pillars in the delivery of healthcare in the United States of America. Health policy that the government employ foster on safety. The patients' safety is a major priority of the USA government (Shi & Singh, 2015). Safety in healthcare is patient-centered to ensure that there is proper flow of information between the various shareholders in the market. Proper flow of information ensures that the needs of the patients are addressed as well as better disease prevention methods (Shi & Singh, 2015) (Goodnough, 2019). Thus, there is little or no room for intolerance among the healthcare practitioners that fosters patient safety. People are assured of safety due to the high quality of services that the people provide (Goodnough, 2019). However, costs for diagnosis and medication of some diseases is high that lower access among the middle and poor working classes. Healthcare delivery is also efficient and effective. Efficiency in the United States is achieved through the utilization of technology. There is a continued use of technology to ensure that there are right diagnoses of diseases (Shi & Singh, 2015). Besides, the doctors and other shareholders within the healthcare sector use technology in the management of records. Management of records promotes the privacy of patients’ information. As a result, people have trust in healthcare providers. The government also improves the effectiveness of the healthcare system in the USA through increasing of insurance among the people (Goodnough, 2019). The USA has increased insurance covers among the people through the ACA act. The policy ensures that most of the people obtain insurance cover and consecutively increase access to the people (Shi & Singh, 2015). Another program is Medicare that provides insurance cover to the patients who are above 65 years. The programs cover patients who are above 65 years, those with disabilities and those with end-stage renal disease. Through the programs, the people obtain pharmaceuticals, and the healthcare providers are also better skilled that improve on the quality of services that they deliver to the American people. It is important as a healthcare consumer to understand the history of the U.S. healthcare delivery system, how it operates today, who participates in the system, what legal and ethical issues arise as a result of the system, and what problems continue to plague the healthcare system. We are all consumers of health care. Yet, in many instances, we are ignorant of what we are actually purchasing. If we were going to spend $1,000 on an appliance or a flat-screen television, many of us would research the product to determine if what we are purchasing is the best product for us. This same concept should be applied to purchasing healthcare services.
Increasing healthcare consumer awareness will protect you in both the personal and professional aspects of your life. You may decide to pursue a career in health care either as a provider or as an administrator. You may also decide to manage a business where you will have the responsibility of providing health care to your employees. And last, from a personal standpoint, you should have the knowledge from a consumer point of view so you can make informed decisions about what matters most—your health. On the other hand, those with low education have higher chances of doing risky jobs. Those individuals who are more educated are subjected to more earnings. Healthcare regulations, funds, workload, and technology continue to complicate and inconvenient the U.S healthcare system. However, the quality and value of care tops. In the United States of America, despite significant healthcare transformation efforts, poor care lingers a considerable concern.
America is second to none in terms of healthcare expenditure across the globe. Ironically, evidence shows that its citizens do not receive the most appropriate care, or at least, which they need. For instance, Graban (2018) documents that preventive care is underutilized in the country, which is escalating the budget of managing advanced diseases. On the other hand, patients of chronic ailments such as diabetes, hypertension, and cardiac complications, do not also usually get treatments that are proven and effective (Wiler, Pines, & Ward, 2019). According to Strome (2019), this case is particularly true and event rampant to the persons that insured, uninsured, or under-insured. Research depicts terrific variations in hospital inpatient lengths of stay, specialists' visits, testing and procedures, and costs — not just by United States' unalike geographic areas, but from one health institution to another in the same town (Wiler, Pines, & Ward, 2019). Abstract
The medical care system of the United States of America for the past years has been considered to be the most expensive in the world. The government of the United States has to spend huge amounts of money for medical care in relation to the gross domestic product and these sums are systematically increasing. Now many scholars came to the conclusion that it is the government programs, which held the responsibility for the growth of uncontrolled spending on medical care, with which such growth is a threat to the financial stability of the United States. The issue is worth-discussing, thus, the given work is devoted to the overview of the structure and the main issues of the US healthcare system to find the effective solution.
Unlike other developed countries the medical care system in the United States of America demands more and more funds while its quality remains the same. 1/3 of the US citizens are still uninsured and there is no future hope for improving the situation. People suffer from rapidly growing prices of medical services and slow growth of salaries inclement. Furthermore, the department of insurance loses its integrity and honesty; since they use such an opportunity to fraud money as well as not paying the workers. The risk of becoming bankrupt is very high in medical care system because of unplanned budget. The insurance programs, financed by the state, are also becoming more expensive, and the government is forced to pay more and more money, which later brings about increase in state financial expenditure that immensely contribute to the poor economy. Employees do not have the free will to change their job due to the high cost of insurance and the monopolization (Stone, et al., 2008,p.2-57). This paper will provide evidences by giving the most effective solution to control this problem and also encouraging people make decisive market decisions by finding new approaches.

Further, this sector consists of other participants, such as insurance and health information technology. Therefore, the need for the creation and implementation of laws that help the state maintain efficiency in the health sector in Colorado.
Problem Statement
The declining standards of medical care within the United States has caused significant concern in the world. Due to these rising concerns, there have been various policies implemented, leading to mixed reactions among the different states. Some of the active policies implemented offer a long-term solution to this problem including Medicaid and Medicare. After acquiring state control, the Republicans dismissed the idea to expand and create medical insurance for Medicaid in Colorado. Sustaining the structure of the health care payroll calls for the deductions from the employees and the employers, which may lead to loss of jobs and increased burden of expenditure (Garcia, 2019).

The main objective of this policy plan is to investigate the role of legislation in the management of the health care sector in the United States. Due to the need for achieving in-depth exploration, this paper uses a combination of both qualitative and quantitative methods of data collection by addressing both practical and theoretical aspects of the research. Based on the answers that the policy requires, choosing survey as the research design. This method involves collecting and analyzing data from a few people who represent the principal group within health care. However, the survey method faces some challenges such as attitudes and perception of the health workers leading to the delimitation of the study. the biggest challenges affecting the Health Care system in United States.
Methods of Examining the Problem
Both qualitative and quantitative research methods should be used to fully understand the
issue of high cost of care in the US. Quantitative methods like surveys and experimentations will
aid in estimating the prevalence, magnitude and frequency of the problem in different regions.
On the other hand, qualitative methods like case studies and observation will help describe the
extent and complexity of the issue. The two approaches need to work in complementation to
obtain a clear understanding of this menace.
Surveys, as a quantitative research method, is one of the most effective in the social
research and present a more viable method of examining the cost of health in the country. They
involve asking of questions in the form of questionnaires and interviews. Questionnaires are
written questions to which the response can be open ended or multiple-choice format. This
would be used to gain information about cost within determinants that are of
disagree/neutral/agree nature. An example is if patients are contented with the cost of services
they get or they deem the cost of cover worthy. Interviews, the researcher discussing issues with
the respondents, are to be used to gain more details on already known aspects of the system. This
may include gathering information to inform policies, administration and use of technology to

Economic Challenges of Healthcare Policy
The economics of the healthcare system in the United States is complex and fragmented. Costs associated with care and the prices healthcare consumers pay are determined by a wide variety of factors making it extremely difficult to find a one size fits all solution. Knickman and Kovner (2015) argue that healthcare is neither a commodity or a service, because of the high variabilities in need, cost, and consumption. The United States spends 80% of all healthcare revenue on just 20% of the population. Health insurance can be provided by the government, the private sector, or an employer. Some citizens do not have any form of healthcare coverage. Regardless of type, insurance coverage generally only pays a portion of the total healthcare cost leaving the consumer to pay the remaining balance. Moreover, reimbursement standards differ for facilities and providers. The government reimburses healthcare facilities, such as hospitals a fixed amount per patient, which creates a higher incentive to work efficiently. Healthcare providers, on the other hand, are reimbursed based on a fee for service model, meaning the more services they provide, the more money they are reimbursed (Knickman & Kovner, 2015).
Reindart (2010) maintains the passage of the Affordable Care Act (ACA) created more strain on the system by adding approximately 30 million uninsured Americans to the market. The projected cost to provide such coverage is around 8 billion to 1 trillion dollars over the next decade. However, the estimated expense of healthcare with no legislative intervention is 35 trillion dollars over the next decade (Reindart, 2010). Laureate Education (2012) contends that the human resources required to provide healthcare to an additional 30 million people is another consideration the ACA does not address. The United States is already experiencing staff shortages for key healthcare jobs like nurses and physicians. Additionally, with baby boomers retiring from the workforce and simultaneously needing more healthcare resources as they age will inevitably exacerbate the shortage of providers (Laureate Education, 2012).
Ethical Considerations
It is important to recognize that the private sector often follows the public sector when deciding what services will be covered (Knickman & Kovner, 2015). Therefore, the implications of Medicare deciding not to pay for a drug or service will likely affect the entire population. Stein (2010) asserts that cost should not be a consideration in determining if medications or services will be paid for by Medicare. Nonetheless, that is what happened with Provenge, a vaccine indicated for late stage prostate cancer patients. The drug prolonged the lifespan of patients by about four months. Provenge costs around 93,000 dollars per p.

Healthcare regulations, funds, workload, and technology continue to complicate and inconvenient the U.S healthcare system. However, the quality and value of care tops. In the United States of America, despite significant healthcare transformation efforts, poor care lingers a considerable concern.
America is second to none in terms of healthcare expenditure across the globe. Ironically, evidence shows that its citizens do not receive the most appropriate care, or at least, which they need. For instance, Graban (2018) documents that preventive care is underutilized in the country, which is escalating the budget of managing advanced diseases. On the other hand, patients of chronic ailments such as diabetes, hypertension, and cardiac complications, do not also usually get treatments that are proven and effective (Wiler, Pines, & Ward, 2019). According to Strome (2019), this case is particularly true and event rampant to the persons that insured, uninsured, or under-insured. The lack of proper coordination of chronic diseases patients' care would only source more or exuberate poor healthcare. The unsurprising healthcare system's underlying fragmentation only fuels the issue given that many health care providers hardly have the payment support such related gears, necessary for effective communication and coordination to improve patient care.
While a significant number of patients miss medically necessary care, other clients get unnecessary or even unsafe attention. Research depicts terrific variations in hospital inpatient lengths of stay, specialists' visits, testing and procedures, and costs — not just by United States' unalike geographic areas, but from one health institution to another in the same town (Wiler, Pines, & Ward, 2019). Though limited, evidence on the most effective treatments and procedures, on the best way of informing providers about the efficacy of different treatments, and on the failures of detecting and reducing errors further underwrite the gaps care's quality and effectiveness (Strome, 2019). The concerns are especially pertinent to the Americans of the lower social classes as well as to those from diverse demographic and ethnic groups are usually frequent victims of a lot of incongruences in health and health care.

Abstract
The medical care system of the United States of America for the past years has been considered to be the most expensive in the world. The government of the United States has to spend huge amounts of money for medical care in relation to the gross domestic product and these sums are systematically increasing. Now many scholars came to the conclusion that it is the government programs, which held the responsibility for the growth of uncontrolled spending on medical care, with which such growth is a threat to the financial stability of the United States. The issue is worth-discussing, thus, the given work is devoted to the overview of the structure and the main issues of the US healthcare system to find the effective solution.
Unlike other developed countries the medical care system in the United States of America demands more and more funds while its quality remains the same. 1/3 of the US citizens are still uninsured and there is no future hope for improving the situation. People suffer from rapidly growing prices of medical services and slow growth of salaries inclement. Furthermore, the department of insurance loses its integrity and honesty; since they use such an opportunity to fraud money as well as not paying the workers. The risk of becoming bankrupt is very high in medical care system because of unplanned budget. The insurance programs, financed by the state, are also becoming more expensive, and the government is forced to pay more and more money, which later brings about increase in state financial expenditure that immensely contribute to the poor economy. Employees do not have the free will to change their job due to the high cost of insurance and the monopolization (Stone, et al., 2008,p.2-57). This paper will provide evidences by giving the most effective solution to control this problem and also encouraging people make decisive market decisions by finding new approaches.

The declining standards of medical care within the United States has caused significant concern in the world. Due to these rising concerns, there have been various policies implemented, leading to mixed reactions among the different states. Some of the active policies implemented offer a long-term solution to this problem including Medicaid and Medicare. After acquiring state control, the Republicans dismissed the idea to expand and create medical insurance for Medicaid in Colorado. Sustaining the structure of the health care payroll calls for the deductions from the employees and the employers, which may lead to loss of jobs and increased burden of expenditure (Garcia, 2019).

The main objective of this policy plan is to investigate the role of legislation in the management of the health care sector in the United States. Due to the need for achieving in-depth exploration, this paper uses a combination of both qualitative and quantitative methods of data collection by addressing both practical and theoretical aspects of the research. Based on the answers that the policy requires, choosing survey as the research design. This method involves collecting and analyzing data from a few people who represent the principal group within health care. However, the survey method faces some challenges such as attitudes and perception of the health workers leading to the delimitation of the study. Both qualitative and quantitative research methods should be used to fully understand the
issue of high cost of care in the US. Quantitative methods like surveys and experimentations will
aid in estimating the prevalence, magnitude and frequency of the problem in different regions.
On the other hand, qualitative methods like case studies and observation will help describe the
extent and complexity of the issue. The two approaches need to work in complementation to
obtain a clear understanding of this menace.
Surveys, as a quantitative research method, is one of the most effective in the social
research and present a more viable method of examining the cost of health in the country. They
involve asking of questions in the form of questionnaires and interviews. Questionnaires are
written questions to which the response can be open ended or multiple-choice format. This
would be used to gain information about cost within determinants that are of
disagree/neutral/agree nature. An example is if patients are contented with the cost of services
they get or they deem the cost of cover worthy. Interviews, the researcher discussing issues with
the respondents, are to be used to gain more details on already known aspects of the system. This
may include gathering information to inform policies, administration and use of technology to

Economic Challenges of Healthcare Policy
The economics of the healthcare system in the United States is complex and fragmented. Costs associated with care and the prices healthcare consumers pay are determined by a wide variety of factors making it extremely difficult to find a one size fits all solution. Knickman and Kovner (2015) argue that healthcare is neither a commodity or a service, because of the high variabilities in need, cost, and consumption. The United States spends 80% of all healthcare revenue on just 20% of the population. Health insurance can be provided by the government, the private sector, or an employer. Some citizens do not have any form of healthcare coverage. Regardless of type, insurance coverage generally only pays a portion of the total healthcare cost leaving the consumer to pay the remaining balance. Moreover, reimbursement standards differ for facilities and providers. The government reimburses healthcare facilities, such as hospitals a fixed amount per patient, which creates a higher incentive to work efficiently. Healthcare providers, on the other hand, are reimbursed based on a fee for service model, meaning the more services they provide, the more money they are reimbursed (Knickman & Kovner, 2015).
Reindart (2010) maintains the passage of the Affordable Care Act (ACA) created more strain on the system by adding approximately 30 million uninsured Americans to the market. The projected cost to provide such coverage is around 8 billion to 1 trillion dollars over the next decade. However, the estimated expense of healthcare with no legislative intervention is 35 trillion dollars over the next decade (Reindart, 2010). Laureate Education (2012) contends that the human resources required to provide healthcare to an additional 30 million people is another consideration the ACA does not address. The United States is already experiencing staff shortages for key healthcare jobs like nurses and physicians. Additionally, with baby boomers retiring from the workforce and simultaneously needing more healthcare resources as they age will inevitably exacerbate the shortage of providers (Laureate Education, 2012).
Ethical Considerations
It is important to recognize that the private sector often follows the public sector when deciding what services will be covered (Knickman & Kovner, 2015). Therefore, the implications of Medicare deciding not to pay for a drug or service will likely affect the entire population. Stein (2010) asserts that cost should not be a consideration in determining if medications or services will be paid for by Medicare. Nonetheless, that is what happened with Provenge, a vaccine indicated for late stage prostate cancer patients. The drug prolonged the lifespan of patients by about four months. Provenge costs around 93,000 dollars per p.

Running Head: HEALTH POLICIES, POLITICS AND PERSPECTIVES 1 HEALTH POLICIES, POLITICS AND PERSPECTIVES . 4 Week #1 Discussion #2: Key Characteristics of the U.S. Healthcare System In the USA, the healthcare system has three pillars, quality, access and cost. They influence accessibility of healthcare among the American citizens (Shi & Singh, 2015). The government has embarked on efforts to ensure that there is improved health access to its people and continually increase the good being of its people. The paper explores how various attributes related to the pillars in the delivery of healthcare in the United States of America. Health policy that the government employ foster on safety. The patients' safety is a major priority of the USA government (Shi & Singh, 2015). Safety in healthcare is patient-centered to ensure that there is proper flow of information between the various shareholders in the market. Proper flow of information ensures that the needs of the patients are addressed as well as better disease prevention methods (Shi & Singh, 2015) (Goodnough, 2019). Thus, there is little or no room for
- 2. intolerance among the healthcare practitioners that fosters patient safety. People are assured of safety due to the high quality of services that the people provide (Goodnough, 2019). However, costs for diagnosis and medication of some diseases is high that lower access among the middle and poor working classes. Healthcare delivery is also efficient and effective. Efficiency in the United States is achieved through the utilization of technology. There is a continued use of technology to ensure that there are right diagnoses of diseases (Shi & Singh, 2015). Besides, the doctors and other shareholders within the healthcare sector use technology in the management of records. Management of records promotes the privacy of patients’ information. As a result, people have trust in healthcare providers. The government also improves the effectiveness of the healthcare system in the USA through increasing of insurance among the people (Goodnough, 2019). The USA has increased insurance covers among the people through the ACA act. The policy ensures that most of the people obtain insurance cover and consecutively increase access to the people (Shi & Singh, 2015). Another program is Medicare that provides insurance cover to the patients who are above 65 years. The programs cover patients who are above 65 years, those with disabilities and those with end-stage renal disease. Through the programs, the people obtain pharmaceuticals, and the healthcare providers are also better skilled that improve on the quality of services that they deliver to the American people. The major provider of healthcare services in the private sector since most hospitals are run by profit organizations (Goodnough, 2019). Healthcare providers firms compete over clients through the services that they offer. Competition fosters the quality of services that they provide as well as the rates that they offer to the general population. The healthcare system in the USA is also equitable by making it available to all the people. Providing people with equal medical care has been the major objective of American citizens (Shi &
- 3. Singh, 2015). Thee old, minority communities like African Americans as well as social-economic disparities. The affordable healthcare act ensures that all people have equal access to healthcare. The program lower pocket payoffs that people pay that lowers medical bills (Goodnough, 2019). People also easily access medical facilities that make people access facilities. These three pillars have been a focus in policy and healthcare reform in recent years and will continue to be in coming years. There are many attributes that contribute to these three pillars and their continued progress. Upon completing the readings by Shi and Singh (2014), some of these attributes include number of students entering medical school, prescription drug cost and the effects of research on healthcare. The number of students entering medical school, cost of prescription drugs and research are all related to the three pillars of quality, cost and access to healthcare.
- 4. The number of students entering medical school is determined by several variables. A major variable effecting the number of students entering medical school is their ability to obtain a government grant to help cover the cost of medical school (Shi & Singh, 2014). With the help of government grants, more students may be more likely to apply to a medical school. The increase in number of students going to medical school will result in more providers. Currently, there is great demand for primary care physicians and with more students attending medical school due to government grants, resultantly a greater number of providers will be available and will increase the populations access to healthcare (Shi & Singh, 2014). The attribute of the number of students going to medical school relates to the access to healthcare in the United States. The next attribute of healthcare identified it the readings of Shi and Singh (2014) is the cost of prescription drugs. The cost of prescription drugs impacts individuals overall cost of healthcare. The cost of prescriptions drugs can greatly increase in a short period, and in 2014 and 2015, there was a twelve- percent increase on overall spending on prescription drugs (Shi & Singh, 2014). The cost of prescription drugs has also been a hot topic in politics. The president of the United States has claimed that the cost of prescription drug prices went down last year for the first time in 51 years (Rizzo, 2019). In the report by The Washington Post, it was found that the statement and claims made by the president were skewed and in 2018, there was an increase in 4,412 brand-name drugs and a price cut to only 46 brand-name drugs in the first seven months of the year (Rizzo, 2019). The cost of prescription coverage effects the overall cost of healthcare to individuals and has been a topic of focus politically. The third attribute identified in the readings is the effect that research has on healthcare. Research is directly related to the quality of healthcare. The effect of research on healthcare includes reducing “the risk of harm from healthcare services by using evidence-based research and technology to promote the
- 5. delivery of best possible care” (Shi & Singh, 2014, p. 558). Research also transforms practice of healthcare by increasing access to effective services and “improves healthcare outcomes by encouraging providers, consumers and patients to use evidence-based information to make informed treatment decisions” (Shi & Singh, 2014, p. 558). Research is responsible for evidence-based practice and ultimately increasing the quality of healthcare in the United States. The combination of these three attributes of number of students entering medical school, cost of prescription drugs and the effects of research are all related to the three pillars of healthcare. The three pillars of healthcare provide a solid foundation to healthcare in the United States and will continue to be improved upon through policy and healthcare reform. This system avoids the use of global budgets, which is defined by Shi and Singh (2019) as “allocation of pre-established total expenditures for a health care system or subsystem”, and allows private businesses to finance health care expenditures independently from the government (p.11). Having no central agency also allows for lower taxes. Former Vice President Joe Biden, in the most recent democratic debate, argued that use of a central agency, or Senator Bernie Sanders proposal of “Medicare for All”, to control the healthcare system would cause taxes to “skyrocket” (Huetteman, 2019). While access is certainly a goal of the U.S health care system, there are other characteristics that could effect the American people as well to consider. Another characteristic of the U.S healthcare system is that access to health care services is based on insurance coverage (Shi & Singh, 2019, p. 10). There are multiple ways Americans can access health care services, however there are barriers if patients are uninsured. One of the main barriers to safe health care delivery to the uninsured is the lack of access to primary care services (Shi & Singh, 2019, p. 12). Because of these barriers, Americans are encouraged to insure themselves either through their employers or through a government health program. According to a report by Wolfson (2019), California will begin issuing tax penalties to those who are uninsured. Lastly, research and innovation in the medical field is one of the reasons the U.S has some of the best hospitals in the world and one of the characteristics that contributes to quality health care delivery (Shi & Singh, 2019, p. 16). However, balancing these new advances and providing cost-effective care can be a challenge (Mawali, Pinto, & Hinai, 2018, p. 442). According to Mawali, Pinto, and Hinai (2018), government systems must regulate “assessment, planning, and budgeting to formal evaluation, procurement, warehousing, distribution, utilization, maintenance, replacement, and disposal” in order to meet the needs of developing processes while also maintaining a budget (p. 442).
- 7. Providing high-quality, cost-effective and accessible care is a challenging task effected by multiple factors of the U.S healthcare system. This issue directly impacts every American and leaders are consistently presenting new ideas to effectively reach these goals to satisfy citizens. With diseases growing more complex and technology advancing to diagnose, treat and prolong the lives of many, the challenge of maintaining these pillars is becoming more of an issue.
