The 16-year-old male patient was admitted to the hospital with a frontal traumatic brain injury caused by being hit with a baseball bat. On admission, he had a Glasgow Coma Scale of 12, was confused and had left-sided weakness. A CT scan revealed a right frontal epidural hematoma, which was evacuated during craniotomy surgery. The patient also received the first treatment cycle of Renacenz. After 24 hours, his GCS improved to 15 and he was regaining mobility in his left side. By the end of the Renacenz treatment, the patient had made a full recovery without neurological impairments.

Resuscitation- what's the point.
Cardiopulmonary resuscitation (CPR) is unique as the only medical intervention performed on anyone without explicit contrary documentation. Therefore, CPR need to be understood in terms of societal expectations, legal mandates and professional duties. We also need to understand not just the the likelihood of survival, but also the likelihood of disability and the cost (both literally and figuratively) to patients, healthcare workers, and to an already stretched healthcare system. Even the term 'resuscitation' means different things to different people...and that's before we even wade into such terms as 'autonomy', 'paternalism' and 'patient-focused care'.
In short, doctors, nurses patients and families can no longer shy away from discussing CPR: it's time to talk. It can be a remarkable way to prevent premature death, it can also squander finite resources and be the beginning of a terrible ordeal for frail patients and frazzled families.

Background: The purpose of the study was to assess caregivers’ compliance with the management protocol for
chemical injury at St. John Eye Hospital, Jerusalem.
Materia l and methods: Charts of all new chemical injury patients who presented to St. John Eye Hospital,
Jerusalem, between January and December 2019 were retrospectively reviewed. Data categories collected included:
Presentation, age, sex, injury, irrigation, lids, visual acuity, slit-lamp examination (SLE), management plan, and
medications given. Data were stored and analysed using Excel.
Results: Patients’ presentation date and time, sex, and age were recorded in over 90% of cases. The mechanism of
injury and type of offending chemical were recorded in 65% of cases. The irrigating solution was identified in 50%
of cases. Corrected visual acuity was recorded in both eyes in almost 50% of cases. Limbal ischemia was documented
in 45% of cases, and intraocular pressure (IOP) was recorded in 25%. The management plan and explanation of the
condition to patients were documented in less than 50% of cases. Antibiotics and steroids (drops/ointment) were
prescribed in 92.5% of cases.
Conclusi ons: The results of this study reveal that our documentation needs improvement for several parameters.
Several recommendations were formulated:
1. Emphasize to caregivers that irrigation must be done first.
2. Corrected visual acuity should be attempted for both eyes in all cases, and reasons for not recording it should be
documented.
3. It is important to document and record limbus details, iris details, and IOP in all cases.

Background: The purpose was to describe the epidemiology of open-globe injury (OGI) in Palestine and identify
the prognostic factors associated with profound visual loss.
Materia l and methods: The current study is a retrospective review of hospital files for 83 consecutive patients
with OGI who presented to St. John Eye Hospital, Jerusalem, within 5 years, between 2009 and 2013. Demographic
details included age, gender, wound characteristics, and visual acuity (VA). The Ocular Trauma Classification Group
was used for wound location, classification, and scoring for each case.
Results: We identified 83 OGI that presented to St. John eye hospital. The study group included 62 males and
21 females. The mean age was 16.66 years ± 3.216. The most frequent injuries were playground injuries (59%),
followed by workplace injuries (26.5%). Penetrating injuries represented 45.8% of injuries, and rupture globes
occurred in 39.8% of cases. The most frequent objects causing injury were metal (31.3%) and stone (20.5%). Kinetic
impact projectiles were a statistically significant poor prognostic factor for the visual outcome. Variables that
were statistically significant poor prognostic factors for visual outcome included: retinal detachment, macular scar,
vitreous hemorrhage.
Conclusi on: This study showed that the act of demonstration, street injuries, kinetic impact projectiles, zone III
injuries, globe disruption, retinal detachment, vitreous hemorrhage, and a poor VA at the first visit are poor prognostic
factors for OGI. Recognition of these prognostic factors will help the ophthalmologist evaluate the injury
and its prognosis.

The PRAMI Trial was a randomized controlled trial that studied 465 patients with acute ST elevation myocardial infarction (STEMI) treated with percutaneous coronary intervention (PCI) of the infarct artery. Patients were randomized to either preventive PCI of significant stenoses in non-infarct arteries immediately during the initial procedure or to undergo PCI of non-infarct arteries only if refractory angina and objective evidence of ischemia developed. The primary outcome of death from cardiac causes, nonfatal myocardial infarction, or refractory angina occurred in 9% of patients who received preventive PCI versus 23% of patients who did not, demonstrating that preventive PCI of non-infarct arteries reduced adverse cardiovascular events.

Materia l and methods: Charts of all new chemical injury patients who presented to St. John Eye Hospital,
Jerusalem, between January and December 2019 were retrospectively reviewed. Data categories collected included:
Presentation, age, sex, injury, irrigation, lids, visual acuity, slit-lamp examination (SLE), management plan, and
medications given. Data were stored and analysed using Excel.
Results: Patients’ presentation date and time, sex, and age were recorded in over 90% of cases. The mechanism of
injury and type of offending chemical were recorded in 65% of cases. The irrigating solution was identified in 50%
of cases. Corrected visual acuity was recorded in both eyes in almost 50% of cases. Limbal ischemia was documented
in 45% of cases, and intraocular pressure (IOP) was recorded in 25%. The management plan and explanation of the
condition to patients were documented in less than 50% of cases. Antibiotics and steroids (drops/ointment) were
prescribed in 92.5% of cases.
Conclusi ons: The results of this study reveal that our documentation needs improvement for several parameters.
Several recommendations were formulated:
1. Emphasize to caregivers that irrigation must be done first.
2. Corrected visual acuity should be attempted for both eyes in all cases, and reasons for not recording it should be
documented.
3. It is important to document and record limbus details, iris details, and IOP in all cases.

This study compared intravenous levetiracetam (LEV) to intravenous phenytoin (PHT) for seizure prophylaxis in patients with severe traumatic brain injury (TBI) or subarachnoid hemorrhage (SAH) admitted to the neuroscience intensive care unit (NSICU). Fifty-two patients were randomized to receive either LEV (n=34) or PHT (n=18). Continuous EEG monitoring was performed for 72 hours. Long-term outcomes including Disability Rating Scale scores and Glasgow Outcome Scale scores were better in the LEV group compared to the PHT group at 3 and 6 months, respectively, after controlling for baseline severity. Background: The purpose was to describe the epidemiology of open-globe injury (OGI) in Palestine and identify
the prognostic factors associated with profound visual loss.
Materia l and methods: The current study is a retrospective review of hospital files for 83 consecutive patients
with OGI who presented to St. John Eye Hospital, Jerusalem, within 5 years, between 2009 and 2013. Demographic
details included age, gender, wound characteristics, and visual acuity (VA). The Ocular Trauma Classification Group
was used for wound location, classification, and scoring for each case.
Results: We identified 83 OGI that presented to St. John eye hospital. The study group included 62 males and
21 females. The mean age was 16.66 years ± 3.216. The most frequent injuries were playground injuries (59%),
followed by workplace injuries (26.5%). Penetrating injuries represented 45.8% of injuries, and rupture globes
occurred in 39.8% of cases. The most frequent objects causing injury were metal (31.3%) and stone (20.5%). Kinetic
impact projectiles were a statistically significant poor prognostic factor for the visual outcome. Variables that
were statistically significant poor prognostic factors for visual outcome included: retinal detachment, macular scar,
vitreous hemorrhage.
Conclusi on: This study showed that the act of demonstration, street injuries, kinetic impact projectiles, zone III
injuries, globe disruption, retinal detachment, vitreous hemorrhage, and a poor VA at the first visit are poor prognostic
factors for OGI. Recognition of these prognostic factors will help the ophthalmologist evaluate the injury
and its prognosis.

Journal Club 1: The Prami Trial
The PRAMI Trial was a randomized controlled trial that studied 465 patients with acute ST elevation myocardial infarction (STEMI) treated with percutaneous coronary intervention (PCI) of the infarct artery. Patients were randomized to either preventive PCI of significant stenoses in non-infarct arteries immediately during the initial procedure or to undergo PCI of non-infarct arteries only if refractory angina and objective evidence of ischemia developed. The primary outcome of death from cardiac causes, nonfatal myocardial infarction, or refractory angina occurred in 9% of patients who received preventive PCI versus 23% of patients who did not, demonstrating that preventive PCI of non-infarct arteries reduced adverse cardiovascular events.

A 18-year-old female patient was involved in a motor vehicle accident and sustained a right acetabular fracture. She presented with acute hip pain and inability to walk. Imaging revealed a minimally displaced low lying anterior column fracture. Due to her diabetes and the minimal displacement, she underwent percutaneous screw fixation. Post-operatively, she made good progress with physical therapy and weight bearing. At her 6-month follow up, she had good pain relief, minimal limping, and near full range of motion of the hip, though long term outcomes cannot be determined yet.

A 67-year-old man was referred to an eye clinic by his optometrist after noticing reduced vision in his right eye over recent months. Examination found his right eye vision was significantly worse than previous measurements while his left eye was normal. Imaging of the right eye showed changes to the retina including fluid accumulation and retinal detachment.
Out of hospital hypertonic saline
This randomized controlled trial evaluated whether pre-hospital administration of hypertonic saline or hypertonic saline plus dextran improved 6-month neurological outcomes in patients with severe traumatic brain injury without hypovolaemic shock compared to normal saline. Over 1300 patients were randomized to receive one of the three study fluids. The primary outcome of an extended Glasgow Outcome Scale score of ≤4 at 6 months was not significantly different between groups. Secondary outcomes including 28-day survival were also not significantly different between groups. The study was terminated early due to futility in showing benefit of the hypertonic fluids over normal saline.

A Strategic Approach: GenAI in Education
Artificial Intelligence (AI) technologies such as Generative AI, Image Generators and Large Language Models have had a dramatic impact on teaching, learning and assessment over the past 18 months. The most immediate threat AI posed was to Academic Integrity with Higher Education Institutes (HEIs) focusing their efforts on combating the use of GenAI in assessment. Guidelines were developed for staff and students, policies put in place too. Innovative educators have forged paths in the use of Generative AI for teaching, learning and assessments leading to pockets of transformation springing up across HEIs, often with little or no top-down guidance, support or direction.
This Gasta posits a strategic approach to integrating AI into HEIs to prepare staff, students and the curriculum for an evolving world and workplace. We will highlight the advantages of working with these technologies beyond the realm of teaching, learning and assessment by considering prompt engineering skills, industry impact, curriculum changes, and the need for staff upskilling. In contrast, not engaging strategically with Generative AI poses risks, including falling behind peers, missed opportunities and failing to ensure our graduates remain employable. - 5. Patient without any medical records before the accident. He is admitted at 5 am with a frontal traumatic brain injury, he was hit with a baseball bat.

- 9. Right Frontal Epidural Hematoma
- 10. Craniotomy and evacuation of the hematoma. Acute phase administration of 50 ml/day.
- 15. The patient was hit with a baseball bat in the right frontal region and admitted with a 12 GCS, confussion and left side hemiparesia. CT scan reported a right frontal epidural hematoma and a craniotomy and evacuation of hematoma was performed, also first treatment cicle of Renacenz is started. After 24 hours, the patient has 15 GCS, and he’s improving movility in the left side of the body.
- 16. At the end of the first cicle of Renacenz treatment the patient was fully recovered, without any neurological impairment. Renacenz has proven to be a helpful resourse to improve patients outcome, but we must not forget the role of decompressing surgery in the traumatic brain injury.
