Normal defecation begins with movement in the left colon, moving stool toward the anus. When stool reaches the rectum, the distention causes relaxation of the internal sphincter and an awareness of the need to defecate. At the time of defecation, the external sphincter relaxes, and abdominal muscles contract, increasing intrarectal pressure and forcing the stool out
The Valsalva maneuver exerts pressure to expel faeces through a voluntary contraction of the abdominal muscles while maintaining forced expiration against a closed airway. Patients with cardiovascular disease, glaucoma, increased intracranial pressure, or a new surgical wound are at greater risk for cardiac dysrhythmias and elevated blood pressure with the Valsalva maneuver and need to avoid straining to pass the stool.
Normal defecation is painless, resulting in passage of soft, formed stool
CONSTIPATION
Constipation is a symptom, not a disease. Improper diet, reduced fluid intake, lack of exercise, and certain medications can cause constipation. For example, patients receiving opiates for pain after surgery often require a stool softener or laxative to prevent constipation. The signs of constipation include infrequent bowel movements (less than every 3 days), difficulty passing stools, excessive straining, inability to defecate at will, and hard feaces
IMPACTION
Fecal impaction results from unrelieved constipation. It is a collection of hardened feces wedged in the rectum that a person cannot expel. In cases of severe impaction the mass extends up into the sigmoid colon.
DIARRHEA
Diarrhea is an increase in the number of stools and the passage of liquid, unformed feces. It is associated with disorders affecting digestion, absorption, and secretion in the GI tract. Intestinal contents pass through the small and large intestine too quickly to allow for the usual absorption of fluid and nutrients. Irritation within the colon results in increased mucus secretion. As a result, feces become watery, and the patient is unable to control the urge to defecate. Normally an anal bag is safe and effective in long-term treatment of patients with fecal incontinence at home, in hospice, or in the hospital. Fecal incontinence is expensive and a potentially dangerous condition in terms of contamination and risk of skin ulceration

Hemorrhoids are dilated, engorged veins in the lining of the rectum. They are either external or internal.
FLATULENCE
As gas accumulates in the lumen of the intestines, the bowel wall stretches and distends (flatulence). It is a common cause of abdominal fullness, pain, and cramping. Normally intestinal gas escapes through the mouth (belching) or the anus (passing of flatus)

6. Objective of Hospital Training
Hospital training is an observational oriented procedure in which
a person is able to learn practically from their theoretically
knowledge.
Hospital training helps to study closely the ground level problems
regarding their job profile.
Hospital training provides practical knowledge to the students.
Hospital training promotes an environment in which student are
induced to adapt themselves quickly to changed circumstances.
Hospital training puts the students in real life situations.
Hospital training removes the hesitation of the students
regarding their working skills and personality development.

The Tajusharia Hospital, Bareilly is very good faculties and doctors well
graduated in medicine.and the hopital facilities are good and they treat the
patient as good friend and the doctors have more patience and they are
specilists in every sector.it means which are related to medicine.the
hospital have more beds and have more medical facilities.and the
environment and atmosphere is very clean.and hospital look like a god
temple.very neat clean.over all these hospital doctors are secondary god

9. Outpatient Department (OPD)
An outpatient department is the part of hospital designed for the
treatment of outpatients. This department provides diagnosis and care
for patients that do not need to stay overnight. It is an important part of
the overall running of the department. The outpatient department will
usually be on the ground floor of the hospital. Wheel chairs and
stretchers are available for non-ambulatory patients. Close at hand will
be X-rat facilities, laboratories, the medical record office and a
pharmacy. In the main waiting area there are a range of facilities for
the patients and their families including toilets, public telephones.
canteen, and water dispenser. Some of the hospitals have no
separate department for outpatients, so they may be treated in same
department as patients that stay overnight. In Tajusharia Hospital,
Bareilly , there is separate outpatient department. Timing for OPD is
10:00 AM to 2:00 PM &ss 03:00 to 06:00.
10. Pharmacy Department
Hospital pharmacy can usually be found with in the premises of
the hospital.
Hospital pharmacy usually stock a large range of medications
including more specialized and investigational medications
(medicines that are being studied but have not yet been
approved), then would be feasible in the community setting.
Hospital pharmacies typically provide medications for the
hospitalized patients only and are not retail establishments.
They typically do not provide prescription service to the public
Some hospitals do have retail pharmacies with in them, which
sell over the counter as well as prescription medications to the
public, but these are not the actual hospital pharmacy.4
11. First Aid Treatment
First aid refers to medical attention that is usually administered immediately
after the injury occurs and at the location where it occurred. It often consists of
a one-time, short-term treatment and requires little technology or training to
administer. First aid can include cleaning minor cuts, scrapes, or scratches;
treating a minor burn; applying bandages and dressings; the use of non-
prescription medicine; draining blisters; removing debris from the eyes;
massage; and drinking fluids to relieve heat stress. OSHA's revised
recordkeeping rule, which went into effect January 1, 2002, does not require
first aid cases to be documented. For example: A worker goes to the first-aid
room and has a dressing applied to a minor cut by a registered nurse.
Although the registered nurse is a health care professional, the employer does
not have to report the accident because the worker simply received first aid.

12. Injections
An injection is a way of administering a sterile liquid form of
medication into tissues of the body beneath the skin, usually using a
sharp, hollow needle or tube. Most people with access to healthcare
services experience injections at some point in their lives, such as
childhood immunisations or medical treatment.
The four most frequently used types of injection are:
Intravenous (IV) injections. An IV injection is the fastest way to inject
a medication and involves using a syringe to inject a medication
directly into a vein. ...
Intramuscular (IM) injections. ...
Subcutaneous (SC) injections. ...
Intradermal (ID) injections.
13. Prescription
A prescription is an order for medicine which a doctor writes, and
which is given to a pharmacist to prepare and administer the
medicine. The new drug does not require a physician's
prescription - it can be bought over the counter. The pharmacist
cannot dispense certain medicines without a doctor's prescription.
Dispensing Procedure
Dispensing refers to the process of preparing and
supplying medicines to a named person together with
clear instructions, advice and counselling where
necessary on the use of those medicines
14. Summary
After 45 days of hospital training I came to learn about how to
dispense medicines to the patient, how to inject injections to
them, how to handle trauma and emergency cases. I also kam
about dealing with hospital conditions like diseases of the
patients, wards, staff members, different departments, etc.
Almost 2000 of prescriptions were received by the dispensary
and we have to treat them with full hospitality services.
Observations
Thus I observed that the hospital is a place where people of all
kinds come with their problems which they believe to be solved
by the medical staff. The working in the hospital takes place by
maintaining proper cleanliness in the environment. The staff and
the doctors are all bostic and good-natured towards the patients
and listen to their problems. Each and every department has its
15. own way of working and at the end of the day, all of the work is
finished by a. There is no carelessness towards the patients for
their drugs or injections and they are treated on time. The
nursing staffs are present at all times for their care. This type of
methodology should really be applicable in all hospitals so that
the public may get treated once and for all to maintain a healthy
country.

During training procedures I have got lot of knowledge about
following:-
Stated project a training regarding each and every first aid
procedures. It includes checking the symptoms and treating at
small scale in first aids and later transferring for surgical
procedures.
I got known regarding artificial respiration process and wound
dressing.
Sites of injection which includes knowledge of syringes, routes of
injections. Routes of injections such as I.V., I.M., I.D.,
Subcutaneous etc.
In Prescription reading, its parts and the abbreviations used are
studied by me in this project it’s truly a scandalous matter for
pharmacists study.
16. Later the dispensing procedure is stated therefore which was
practiced by me all around the training at regular intervals.
I also learn about patient observation chart and how to fill it, use
it.
In simple diagnostic reports that are easy to study in case of
pathological reports but a bit of difficulty arises in reading
radiological reports.
Therefore I have got a marvelous experience by this training.
