Reinventing Rehabilitation Delivery Using Big Data and Mobile Technology

People who suffer a stroke, brain injury, onset of dementia or learning disorders often have trouble with learning, cognition, speech, and language. Traditionally, their rehabilitation is long-term, cumbersome and expensive. We demonstrate a software platform that provides remote rehabilitation with or without a clinician’s assistance and is accessible to the patient 24/7. The program uses smart algorithms to deliver and monitor therapy, monitors patient engagement and uses machine learning to provide personalized rehabilitation. It has already delivered over 40 million therapy exercises to patients across the US and 10 countries globally.

Expo 15 returned to Manchester and was hosted once again by NHS England. Around 5000 people a day from health and care, the voluntary sector, local government, and industry joined together at Manchester Central Convention Centre for two packed days of speakers, workshops, exhibitions and professional development.
This year, Expo was more relevant and engaging than ever before, happening within the first 100 days of the new Government, and almost 12 months after the publication of the NHS Five Year Forward View. It was also a great opportunity to check on and learn from the progress of Greater Manchester as the area prepares to take over a £6 billion devolved health and social care budget, pledging to integrate hospital, community, primary and social care and vastly improve health and well-being.

Abstract
Effective patient adherence management strategies require better understanding of patient-generated data, including patient-reported data and measurements from devices and sensors, as key to assisting providers in learning more about their patients’needs and enhancing patient centric care. Gaining “meaningful use” of patient-generated data could ultimately lead to improvements in patient safety and outcomes. In this workshop, we review proof of concept studies using technology to assess patient health literacy and self-efficacy with the goal of providing timely intervention, remedy, and improvements in cost and quality of care. In particular, we focus on engagement-enabling technolgoies that can leverage non-clinical information sources and reflect patient activities in the “wild”. We look into barriers to adherence, patients and providers roles in improving adherence, and the use of technology to assist patients in staying on track. The speakers will address the issues related tothe integration of patient-generated data into everyday life and clinical practice and share lessons learned from implementing these designs in practice. This workshop aims to share requirements gathered for the design of next-generation healthcare systems, especially in areas where the explosive availability of patient-generated data is expected to make impacts.

Personalization is key to the delivery of wellness care including preventive measures and disease management regimes, where patients take on increased responsibility for
their own health. While personalized care has already taken a giant leap through genomics, it remains a challenge to understand how individual differences play a role in patient adherence and manage recommended changes accordingly.
Practical methods of creating and evaluating personalized
systems have not been fully established. In particular, the role of data-driven analytics in producing actionable insights for practitioners is unclear, and the use of behavioral data has created additional challenges to the understanding of patient adherence for effective care delivery.
The panel will discuss the challenges that face many countries around personalized care from various perspectives. These range from behavioral aspects such as maintaining good practices, cognitive aspects such as how do individuals make decisions in the lights of good evidence, social aspects such as how to engage patients in sustaining adherence behavior, to technological aspects such as how to evaluate individual applicability of data-driven analytics and personalized technological systems.
The panel is expected to contribute to the global community by presenting lessons learned from
existing pilot designs and a collective list of recommendations for pilot design of personalized services at the conclusion of this panel.

The Whole System Demonstrator trial aimed to test whether new telehealth technologies could help people stay healthy at home. It involved 6000 patients across three sites including Cornwall. The trial provided patients with devices to monitor health readings like blood pressure and weight at home, which were transmitted to nurses. Initial concerns from doctors about increased workload were alleviated as the technology allowed remote monitoring and helped prevent unnecessary visits. Patients found the systems easy to use and that it increased their independence and empowerment.
Wundrsight Healthcare
Digital therapeutics startup Wundrsight is developing mixed reality platforms to address mental healthcare issues through evidence-based and standardized interventions. They have developed ReliefXR for stress and relaxation and ReviveXR for substance abuse rehabilitation. They plan to partner with clinics and hospitals to offer their digital therapeutics and establish physical-digital "brain gyms". Wundrsight aims to treat over 10,000 patients across India by 2023 through these offerings.

Abstract
The practice problem identified is headache sufferers are not adequately getting their headaches controlled. They are utilizing the emergency room for pain control which only gives them immediate control but not long-term control. The emergency rooms are becoming overcrowded. Controlling headache patients pain will help keep them out of the emergency room and help to decrease the overcrowding of the emergency rooms. Controlling the headache pains will also help decrease costs to consumers and the healthcare system. Case managers can help not only educate these patients, but they can increase better continuity of care and be able to provide feedback to the physicians. The better educated these patients are, the less likely they are to go to the emergency room. The practice problem question to be answered is to examine if an education plan along with a plan of care with primary care provider and network, will assist in increasing headache control and ultimately keep these patients out of the emergency rooms. The purpose of this project will be to implement an education protocol for headache patients to evaluate usage of the emergency department for headache pain control. Models that will be useful for this project are Pender’s Health Promotion Model and the Health Belief Model. These models will help to evaluate individuals and how they interact and behave in order to be able to change their behavior and help them to control their headache pain. This protocol can be included for provider’s and case managers within a clinically integrated network so that a greater number of patients can be managed effectively to decrease emergency room visits. The expected results will be this educational protocol will help patients control their headaches better and will assist in decreasing emergency room visits. The implications of this project will include increased control on headaches, decrease emergency room visits from headaches, and better quality of life for headache patients.

This webinar discussed implanted neural prosthetics for restoring function. It defined neural prosthetics as devices that connect directly to the nervous system to replace or supplement function. The webinar outlined therapeutic applications that aim to restore voluntary motor control through temporary treatment versus prosthetic applications that replace lost function with an implanted device. Examples of neurostimulation applications included bladder control, breathing, hand function, and more. Clinical trials were discussed as the process for testing these devices, and resources were provided to learn about eligibility and participation. The webinar concluded by discussing how the Institute for Functional Restoration aims to create a sustainable commercialization model for neuromodulation systems to restore function long-term.
Evaluating the Integrated Approach to Chronic Care Management
This webinar will provide an overview of the evaluation study being done at the Durham Clinic, an integrated health home run by Cherry Street Health Services in Grand Rapids, Michigan. ALIVIO is developing a mental health app called MHA that connects to EEG devices to measure brain activity and mental health conditions. The app will allow users to (1) do self-assessments of their mental condition, (2) record measurements and analyze results to determine their mental state, and (3) connect to and share data with healthcare providers if needed. For healthcare providers, the app provides access to patient data to assess mental health before appointments and allows constant monitoring of patient data to recommend treatment plans. The app collects daily mental activity data from brain stimulation devices that can be shared with healthcare providers like psychiatrists and neurologists for consultation and medical help.

Abstract
The practice problem identified is headache sufferers are not adequately getting their headaches controlled. They are utilizing the emergency room for pain control which only gives them immediate control but not long-term control. The emergency rooms are becoming overcrowded. Controlling headache patients pain will help keep them out of the emergency room and help to decrease the overcrowding of the emergency rooms. Controlling the headache pains will also help decrease costs to consumers and the healthcare system. Case managers can help not only educate these patients, but they can increase better continuity of care and be able to provide feedback to the physicians. The better educated these patients are, the less likely they are to go to the emergency room. The practice problem question to be answered is to examine if an education plan along with a plan of care with primary care provider and network, will assist in increasing headache control and ultimately keep these patients out of the emergency rooms. The purpose of this project will be to implement an education protocol for headache patients to evaluate usage of the emergency department for headache pain control. Models that will be useful for this project are Pender’s Health Promotion Model and the Health Belief Model. These models will help to evaluate individuals and how they interact and behave in order to be able to change their behavior and help them to control their headache pain. This protocol can be included for provider’s and case managers within a clinically integrated network so that a greater number of patients can be managed effectively to decrease emergency room visits. The expected results will be this educational protocol will help patients control their headaches better and will assist in decreasing emergency room visits. The implications of this project will include increased control on headaches, decrease emergency room visits from headaches, and better quality of life for headache patients.

This webinar discussed implanted neural prosthetics for restoring function. It defined neural prosthetics as devices that connect directly to the nervous system to replace or supplement function. The webinar outlined therapeutic applications that aim to restore voluntary motor control through temporary treatment versus prosthetic applications that replace lost function with an implanted device. Examples of neurostimulation applications included bladder control, breathing, hand function, and more. Clinical trials were discussed as the process for testing these devices, and resources were provided to learn about eligibility and participation. The webinar concluded by discussing how the Institute for Functional Restoration aims to create a sustainable commercialization model for neuromodulation systems to restore function long-term.

This webinar will provide an overview of the evaluation study being done at the Durham Clinic, an integrated health home run by Cherry Street Health Services in Grand Rapids, Michigan.
