Abstract
The practice problem identified is headache sufferers are not adequately getting their headaches controlled. They are utilizing the emergency room for pain control which only gives them immediate control but not long-term control. The emergency rooms are becoming overcrowded. Controlling headache patients pain will help keep them out of the emergency room and help to decrease the overcrowding of the emergency rooms. Controlling the headache pains will also help decrease costs to consumers and the healthcare system. Case managers can help not only educate these patients, but they can increase better continuity of care and be able to provide feedback to the physicians. The better educated these patients are, the less likely they are to go to the emergency room. The practice problem question to be answered is to examine if an education plan along with a plan of care with primary care provider and network, will assist in increasing headache control and ultimately keep these patients out of the emergency rooms. The purpose of this project will be to implement an education protocol for headache patients to evaluate usage of the emergency department for headache pain control. Models that will be useful for this project are Pender’s Health Promotion Model and the Health Belief Model. These models will help to evaluate individuals and how they interact and behave in order to be able to change their behavior and help them to control their headache pain. This protocol can be included for provider’s and case managers within a clinically integrated network so that a greater number of patients can be managed effectively to decrease emergency room visits. The expected results will be this educational protocol will help patients control their headaches better and will assist in decreasing emergency room visits. The implications of this project will include increased control on headaches, decrease emergency room visits from headaches, and better quality of life for headache patients.

Nurses have a responsibility to their patients to deliver quality healthcare and to keep patients safe. According to a report conducted by the American Nurses Association (2015) there is a new emphasis placed on cost containment and the first group being affected by cost-cutting measures is staffing (p. 4). This is not a very good idea since the majority of patient care falls on nurses and “appropriate nurse staffing levels are essential to optimizing quality of care and patient outcomes in the era of value-based healthcare (American Nurses Association [ANA], 2015, p. 4). This writer would like to explore how staffing affects patient care in regards to quality, outcomes and level of satisfaction. In addition, this writer would also like to explore the affect short-staffing has on the nurse and how the organization benefits financially from cutting staff. In the end, this writer would like to seek creative, cost-effective solutions that would benefit the nurse, the patient and the organization.
Impact of Staffing
Organizations believe that by cutting staff they are saving money to help their bottom-line, which is not unreasonable because everyone has a responsibility to be cost-conscious in today’s world. However, the reality may differ from what the organization believes that they are gaining. Lower staffing can have detrimental effects on patient care and outcomes and increase the risk of patient harm. When staffing is not adequate to care for patients it increases the risk for hospital acquired infections, medication errors, falls, missed treatment, and even death.
When staff levels are low and nurses are expected to perform too many tasks with too little time in a twelve-hour shift, staff burnout is sure to happen which will lead to high staff turnover. With the push to have more registered nurses with their Bachelor’s degree on the belief that they can deliver better more efficient care, will more likely open the door for the nurse to seek employment elsewhere. PICOT QUESTION: Does the required education on the Braden scale increase nursing interventions for Adult patients on an acute care floor at risk of developing pressure ulcers during hospitalization?
Currently, most hospitals are faced with a clinical problem of acquired pressure ulcers. According to Pittman et al (2015), hospital-acquired pressure ulcers remain one of the persistent and relevant issues that need to be addressed in long-term hospital stay patients. Health care is attempting to implement evidence-based protocols, though patients continue to suffer from this prevalent and preventable injury. Health care institutions are facing a big challenge for the patients with this acquired condition because hospital bills continue to balloon and at the same time insurance companies stopped paying for this condition. Research shows that pressure ulcer is preventable; though, in spite of hospitals striving to integrate evidence-based approaches to curb the issue, it continues to remain a serious issue for long-term hospital stay patients. This PICOT statement this research paper is proposing to use is a Braden Scale which can be used by nurses in their practice to reduce hospital-acquired pressure injuries which will reduce the patient stay in the hospital as well as the bills burden in the hospital.

According to Engels et al (2016), “the importance of using evidence-based practice in long-term care hospitals to reduce prevalent pressure ulcers is to promote a safe cost-effective outcome for our patients, families, and the healthcare group. Research needs to be conducted and qualitative data collected when designing an evidence-based solution to hospital-acquired injuries”. Despite a lot of research being conducted from the past years concerning acquired pressure ulcers, many patients continue to get the disease. Evidence-based practice allows the nurse to get pooled in a team of experts where interdisciplinary collaboration becomes the ultimate objective for nurses to practice autonomy that enhances change in the nursing field based on data. “The nursing research utilizes qualitative and quantitative logical methods and an EBP approach aimed at around the study and change of patient consideration, understanding consideration frameworks, and patient results” (Mervis & Phillips, 2019). This PICOT question will effectively apply the Braden Scale to see how it can positively impact long-term hospital in reducing pressure ulcers injuries.

Problem Statement
Standardized care plans can be described as the pre-determined menu of interventions which are used for different patient situations (Monsen, Swenson & Kerr, 2016). Evidence-based care is the conscientious use of the most recent evidence to make decisions on the care of individual patients or in the delivery of health care services (Murdaugh, Parsons & Pender, 2018). The current best evidence is the most recent information which has been obtained from valid and relevant research about the effects of different types of healthcare, the accuracy of diagnostic tests, the potential for harm from exposure to different agents, or predictive power of prognostic factor (Schmidt & Brown, 2017). Standardized care plans form the main basis for the implementation of evidence-based care directly in practice and for the improvement of patient outcomes (Nussbaum et al., 2015; Yehuda & Hoge, 2016). A health care facility recently transitioned to the use of a new and better electronic health record system. The facility also purchased standardized care plans to increase efficiency in their operations. However, the compliance with using the standardized care plans was only 40 percent among the inpatient nurses. Accordingly, the facility recently had a visit from the Joint Commission on Accreditation of Healthcare Organizations and received a negative rating because the nurses were not adding care plans based on the patients' primary problem or diagnosis in the patients' charts upon admission. This presents several specific problems in the healthcare facility. There is poor compliance from the nurses concerning the addition of standardized care plans to the charts of patients based on their diagnosis or primary problem(s). The system which the facility invested in was not being used for the improvement of patient outcomes and quality of care delivered. The focus of this project is the failure of inpatient nurses to make use of standardized care plans. The gap in nursing is the failure of delivery of evidence-based practice using the standardized care plans which result in poor patient outcomes and quality of life. One of the areas of knowledge that has not yet been explored is the cause of low rates of adoption of standardized care plans by nurses. Emergency Department overcrowding related to patients seeking care for non-emergent conditions is an increasing concern for hospitals across the country. In rural areas, this issue is of concern not only for patient care but also has an impact economically on hospital financial viability.
Current Situation
Emergency Departments are designed to provide expedient care for individuals with emergent, life-threatening situations. However, in the current state, emergency departments are increasingly serving as a source of providing primary care. The resulting inappropriate use of the emergency department for non-emergent visits has been shown to increases cost, impact patient safety and quality.
Healthcare organizations must find and development innovative methods to provide quality patient care while maintaining low cost and maximum efficiencies. While demand for Emergency Services grows in part due to an aging population, the volume also has grown due to lack of primary care physicians and patient preference. The financial pressures faced by hospitals due to reductions in reimbursement necessitate a restructuring of the standard model of healthcare care delivery.
Problem Statement
As the population continues to grow emergency departments will continue to see not only acute illness but more chronic illness. It is essential for health care systems to continue in developing new and innovative means related to optimization of care delivery. Specifically this will identify factors that affect overutilization of the emergency department by individuals that are more appropriately treated in the primary care setting. Therefore, the increasing use of emergency departments will impact overall patient care due to lack of continuity that is provided in the primary care setting for chronic illness.
Research Objective
This research proposal will evaluate the feasibility of incorporating a medical home into the emergency department setting, therefore, reducing overcrowding in the Emergency Department. This increased access to primary care will ultimately increase access to quality care in the most appropriate cost-effective setting
Research Question
The intended purpose of this research proposal will examine the concept volume and acuity of patients seen in the Emergency Department. The following questions will be addressed. What measures can be implemented to reduce the overutilization of the ED yet offer the appropriate level of care for the patient? What barriers are associated with accessing sustained primary care?

Introduction:
Stroke refers to a cardiovascular disease which has been one of the leading reasons for deaths and long term disability. A stroke is an abrupt onset of a neurological deficit led by a vascular rupture or blockage that reduces the blood flow to brain. Subsequently, causing death to the tissue in the brain region if interruption of the blood flow persists. The indications of stroke vary, but may include the loss of function to one side of the body, the inability to speak or talk, and reduced vision or severe headache (Poston, 2018).
Issue: Discovery Research
Over time, the financial penalties on readmissions to the hospital have been taking place, which is promoting hospitals to take measures to reduce the instance of readmissions. A variety of interventions are taking place on different levels to ensure that pre and post discharge care is in place to avoid readmissions. The efficacy of interventions is dependent on the variety of components. Single component interventions are least effective and tend to have no effect on readmissions to the hospitals. Patients that are discharged to post-acute care accommodations are subjected to multi-component interventions and readmissions have dropped drastically. These interventions work through communication, advanced planning of care, and training to tackle simple medical issues that might cause readmissions. The availability of risk stratification methods have made it easier for the hospitals to give more care and attention to the patients that are more likely to get readmitted. Home based services are provided to ensure proper medical care for the patients.
This capstone project attempts to discuss the factors causing the readmissions of stroke patients to the hospitals. The past 20 years have proven to be important in acute and inpatient stroke care however, quality of post-acute care varies specially for the patients that are discharged to home. (Condon, Lycan, & Duncan, 2016). Different reasons for stroke readmissions are to be examined in this capstone project. Expected Outcomes: Discovery Research
This project aims to take into account the reasons of stroke and readmissions after being treated for stroke. Stroke is the second primary reason of readmissions in the hospital. Major readmissions comprise of elderly people. 20-70% people who survive stroke are readmitted in the first year of their treatment (Bravata, Ho, Meehan, & Brass, 2006). The chronicity of COPD allows for self management by sufferers. (Spencer & Barcomb 2014). The self management goal is reduced hospital admissions and improved life quality (Bedra et al 2013). Sufferers should have access to a wide range of skills available from the multidisciplinary team. Those include exacerbation limitation, respiratory failure, chronic productive cough and anxiety and depression.
Symptom Recognition.
Patients discharged from hospital are susceptible to readmission (Bedra et al 2013). Understanding the condition and knowing when they are having an exacerbation is imperative for self management, and what to do in the given circumstances, and when and what medication to take, or realise they need hospital treatment.
Treatment.
The main form of treatments comes from inhaled therapies and explained below would be when they would be administered and their understandings are a major factor in self management.
For breathlessness and exercise limitations: A short acting Beta2 agonist (as required) or short acting muscarinic antagonist (as required).
For exacerbations or persistent breathlessness: A long acting beta2 agonist, long acting muscarinic antagonist, to – long acting beta2 agonist + inhaled corticosteroid (Combination Inhaler) OR a long acting muscarinic antagonist (must discontinue short acting antagonist once this is commenced).
(Remember if using Corticosteroids, this has no evidence of long terms benefits).
If experiencing persistent exacerbations or breathlessness. Long acting Muscarinic antagonist + long acting beta2 agonist and inhaled corticosteroid (combined inhaler).
Niesters et al, (2012) describe how oxygen therapy can also be used, but awareness of inappropriate oxygen therapy with COPD patients is imperative as this can cause respiratory depression.
Self Monitoring.
The British Thoracic Society (BTS) have identified five high impact actions that can improve outcomes for people being discharged after an acute exacerbation of COPD. The form is a quick way of identifying patients need for those interventions, ensuring their needs are met. The aim is for lessened hospital readmission rates with self monitoring patients. The five actions are;
Review of medication and demonstration of inhalers they will be using.
Provide a written Self Management plan and Emergency drug pack.
Asses and offer referral for smoking sensation.
Assess for suitability for pulmonary rehab.
Arrange a follow up call within 72 hours of discharge.
Educational Interventions.
Reardon et al, (2005) explain pulmonary rehabilitation as programs which work with patients to help manage their condition, muscle strength, ability to cope with their disease, help with social requirements as people can become quite isolated.

The paper will focus on pertinent and assigned sections of Tocqueville. The student will use parenthetical citations as needed, and will provide a bibliography in proper current Turabian format. Tocqueville must be the only source. The paper must begin with the student’s name and the date in the upper right hand corner, followed by a space, and then the beginning of the paper itself. Do not include a title page.
In the body of the paper, the student must key in on what he/she believes are the 2–3 most important issues in the assigned excerpts of Tocqueville. These issues will generally fall under the aegis of the size/scope/power of the central government. Although the image of the counseling field has changed dramatically over the past two or three decades, much of society still views counseling and therapy as a mystic process reserved for those who lack the ability to handle life issues effectively. Problem Statement
Standardized care plans can be described as the pre-determined menu of interventions which are used for different patient situations (Monsen, Swenson & Kerr, 2016). Evidence-based care is the conscientious use of the most recent evidence to make decisions on the care of individual patients or in the delivery of health care services (Murdaugh, Parsons & Pender, 2018). The current best evidence is the most recent information which has been obtained from valid and relevant research about the effects of different types of healthcare, the accuracy of diagnostic tests, the potential for harm from exposure to different agents, or predictive power of prognostic factor (Schmidt & Brown, 2017). Standardized care plans form the main basis for the implementation of evidence-based care directly in practice and for the improvement of patient outcomes (Nussbaum et al., 2015; Yehuda & Hoge, 2016). A health care facility recently transitioned to the use of a new and better electronic health record system. The facility also purchased standardized care plans to increase efficiency in their operations. However, the compliance with using the standardized care plans was only 40 percent among the inpatient nurses. Accordingly, the facility recently had a visit from the Joint Commission on Accreditation of Healthcare Organizations and received a negative rating because the nurses were not adding care plans based on the patients' primary problem or diagnosis in the patients' charts upon admission. This presents several specific problems in the healthcare facility. There is poor compliance from the nurses concerning the addition of standardized care plans to the charts of patients based on their diagnosis or primary problem(s). The system which the facility invested in was not being used for the improvement of patient outcomes and quality of care delivered. The focus of this project is the failure of inpatient nurses to make use of standardized care plans. The gap in nursing is the failure of delivery of evidence-based practice using the standardized care plans which result in poor patient outcomes and quality of life. One of the areas of knowledge that has not yet been explored is the cause of low rates of adoption of standardized care plans by nurses. Emergency Department overcrowding related to patients seeking care for non-emergent conditions is an increasing concern for hospitals across the country. In rural areas, this issue is of concern not only for patient care but also has an impact economically on hospital financial viability.
Current Situation
Emergency Departments are designed to provide expedient care for individuals with emergent, life-threatening situations. However, in the current state, emergency departments are increasingly serving as a source of providing primary care. The resulting inappropriate use of the emergency department for non-emergent visits has been shown to increases cost, impact patient safety and quality.
Healthcare organizations must find and development innovative methods to provide quality patient care while maintaining low cost and maximum efficiencies. While demand for Emergency Services grows in part due to an aging population, the volume also has grown due to lack of primary care physicians and patient preference. The financial pressures faced by hospitals due to reductions in reimbursement necessitate a restructuring of the standard model of healthcare care delivery.
Problem Statement
As the population continues to grow emergency departments will continue to see not only acute illness but more chronic illness. It is essential for health care systems to continue in developing new and innovative means related to optimization of care delivery. Specifically this will identify factors that affect overutilization of the emergency department by individuals that are more appropriately treated in the primary care setting. Therefore, the increasing use of emergency departments will impact overall patient care due to lack of continuity that is provided in the primary care setting for chronic illness.
Research Objective
This research proposal will evaluate the feasibility of incorporating a medical home into the emergency department setting, therefore, reducing overcrowding in the Emergency Department. This increased access to primary care will ultimately increase access to quality care in the most appropriate cost-effective setting
Research Question
The intended purpose of this research proposal will examine the concept volume and acuity of patients seen in the Emergency Department. The following questions will be addressed. Introduction:
Stroke refers to a cardiovascular disease which has been one of the leading reasons for deaths and long term disability. A stroke is an abrupt onset of a neurological deficit led by a vascular rupture or blockage that reduces the blood flow to brain. Subsequently, causing death to the tissue in the brain region if interruption of the blood flow persists. The indications of stroke vary, but may include the loss of function to one side of the body, the inability to speak or talk, and reduced vision or severe headache (Poston, 2018).
Issue: Discovery Research
Over time, the financial penalties on readmissions to the hospital have been taking place, which is promoting hospitals to take measures to reduce the instance of readmissions. A variety of interventions are taking place on different levels to ensure that pre and post discharge care is in place to avoid readmissions. The efficacy of interventions is dependent on the variety of components. Single component interventions are least effective and tend to have no effect on readmissions to the hospitals. Patients that are discharged to post-acute care accommodations are subjected to multi-component interventions and readmissions have dropped drastically. These interventions work through communication, advanced planning of care, and training to tackle simple medical issues that might cause readmissions. The availability of risk stratification methods have made it easier for the hospitals to give more care and attention to the patients that are more likely to get readmitted. Home based services are provided to ensure proper medical care for the patients.
This capstone project attempts to discuss the factors causing the readmissions of stroke patients to the hospitals. The past 20 years have proven to be important in acute and inpatient stroke care however, quality of post-acute care varies specially for the patients that are discharged to home. (Condon, Lycan, & Duncan, 2016). Different reasons for stroke readmissions are to be examined in this capstone project. Expected Outcomes: Discovery Research

The chronicity of COPD allows for self management by sufferers. (Spencer & Barcomb 2014). The self management goal is reduced hospital admissions and improved life quality (Bedra et al 2013). Sufferers should have access to a wide range of skills available from the multidisciplinary team. Those include exacerbation limitation, respiratory failure, chronic productive cough and anxiety and depression.
Symptom Recognition.
Patients discharged from hospital are susceptible to readmission (Bedra et al 2013). Understanding the condition and knowing when they are having an exacerbation is imperative for self management, and what to do in the given circumstances, and when and what medication to take, or realise they need hospital treatment.
Treatment.
The main form of treatments comes from inhaled therapies and explained below would be when they would be administered and their understandings are a major factor in self management.
For breathlessness and exercise limitations: A short acting Beta2 agonist (as required) or short acting muscarinic antagonist (as required).
For exacerbations or persistent breathlessness: A long acting beta2 agonist, long acting muscarinic antagonist, to – long acting beta2 agonist + inhaled corticosteroid (Combination Inhaler) OR a long acting muscarinic antagonist (must discontinue short acting antagonist once this is commenced).
(Remember if using Corticosteroids, this has no evidence of long terms benefits).
If experiencing persistent exacerbations or breathlessness. Long acting Muscarinic antagonist + long acting beta2 agonist and inhaled corticosteroid (combined inhaler).
Niesters et al, (2012) describe how oxygen therapy can also be used, but awareness of inappropriate oxygen therapy with COPD patients is imperative as this can cause respiratory depression.
Self Monitoring.
The British Thoracic Society (BTS) have identified five high impact actions that can improve outcomes for people being discharged after an acute exacerbation of COPD. The form is a quick way of identifying patients need for those interventions, ensuring their needs are met. The aim is for lessened hospital readmission rates with self monitoring patients. The five actions are;
Review of medication and demonstration of inhalers they will be using.
Provide a written Self Management plan and Emergency drug pack.
Asses and offer referral for smoking sensation.
Assess for suitability for pulmonary rehab.
Arrange a follow up call within 72 hours of discharge.
Educational Interventions.
Reardon et al, (2005) explain pulmonary rehabilitation as programs which work with patients to help manage their condition, muscle strength, ability to cope with their disease, help with social requirements as people can become quite isolated.

I am a scholar of the Revolution. That's the topic of my dissertation. Please believe me when I say that I know a lot about it.
I also happen to know--and this is well-supported by historians--that the Revolution was a civil war in which, for the first several years, Revolutionaries and Loyalists were evenly matched.
I will repeat that. Evenly matched. Loyalists were not merely too cowardly to fight, and they were not old fogies who hated the idea of freedom. Most had been in the Colonies for generations. Many of them took up arms for their King and their country. And when they lost, you confiscated their homes and they fled with the clothes on their back to Canada, England, and other places of the Empire. Now, a lot of you love to repeat some very fervent patriotic diatribe about how great the Revolution was. That's not history. That's propaganda. Know the difference.

So put yourself in those shoes. How many of you would have sided with the Empire (whether American or British) based on the fact that you don't know how this will shake out? Would you call someone who supports Calexit a Patriot? Revolutionary? Nutcase? Who gets to own that word, anyway?
You can choose that you would have supported the revolutionaries--but think. Think about the other side. They matter, and their experiences got to be cleansed out of history to make you feel better about the way the revolutionaries behaved during the War. Acknowledge that they are there, and that their point of view has merit, even if you not agree with it.

Although educational reform has had an effect on all children, it is most apparent in the early elementary years. Reform and change comes from a number of sources and the chapter begins by reminding you of this. Let’s examine a few of these sources...

Your text explains that the best way to think of a child’s development during this time is: slow and steady. During this stage, there is not much difference between boys and girls when it comes to physical capabilities. Although it is always important to not stereotype based on one’s gender, it is especially important during these years. These children are also entering into their "tween" years, thus; being sensitive to the children's and parents' needs in regards to such changes is important.

Typically, communication breakdowns result from lack of understanding without clarification; often, there wasn't even an attempt at clarification. If barriers to interpersonal communication are not acknowledged and addressed, workplace productivity can suffer.
Language Differences
Interpersonal communication can go awry when the sender and receiver of the message speak a different language -- literally and figuratively. Not everyone in the workplace will understand slang, jargon, acronyms and industry terminology. Instead of seeking clarification, employees might guess at the meaning of the message and then act on mistaken assumptions. Also, misunderstandings may occur among workers who do not speak the same primary language. As a result, feelings may be hurt, based on misinterpretation of words or of body language.
Cultural Differences
Interpersonal communication may be adversely affected by lack of cultural understanding, mis-perception, bias and stereotypical beliefs. Workers may have limited skill or experience communicating with people from a different background. Many companies offer diversity training to help employees understand how to communicate more effectively across cultures and relate to those who may have different background experiences. Similarly, gender barriers can obstruct interpersonal communication if men and women are treated differently, and held to different standards, causing interpersonal conflicts in the workplace.
Personality Differences
Like any skill, some people are better at interpersonal communication than others. Personality traits also influence how well an individual interacts with subordinates, peers and supervisors. Extraversion can be an advantage when it comes to speaking out, sharing opinions and disseminating information. However, introverts may have the edge when it comes to listening, reflecting and remembering. Barriers to interpersonal communication may occur when employees lack self-awareness, sensitivity and flexibility. Such behavior undermines teamwork, which requires mutual respect, compromise and negotiation. Bullying, backstabbing and cut throat competition create a toxic workplace climate that will strain interpersonal relationships.
Generational Differences
Interpersonal communication can be complicated by generational differences in speech, dress, values, priorities and preferences. For instance, there may be a generational divide as to how team members prefer to communicate with one another. If younger workers sit in cubicles, using social networking as their primary channel of communication, it can alienate them from older workers who may prefer face-to-face communication. Abstract
The practice problem identified is headache sufferers are not
adequately getting their headaches controlled. They are
utilizing the emergency room for pain control which only gives
them immediate control but not long-term control. The
emergency rooms are becoming overcrowded. Controlling
headache patients pain will help keep them out of the
emergency room and help to decrease the overcrowding of the
emergency rooms. Controlling the headache pains will also
help decrease costs to consumers and the healthcare system.
Case managers can help not only educate these patients, but
they can increase better continuity of care and be able to
provide feedback to the physicians. The better educated these
patients are, the less likely they are to go to the emergency
room. The practice problem question to be answered is to
examine if an education plan along with a plan of care with
primary care provider and network, will assist in increasing
headache control and ultimately keep these patients out of the
emergency rooms. The purpose of this project will be to
implement an education protocol for headache patients to
evaluate usage of the emergency department for headache pain
control. Models that will be useful for this project are Pender’s
Health Promotion Model and the Health Belief Model. These
models will help to evaluate individuals and how they interact
and behave in order to be able to change their behavior and help
them to control their headache pain. This protocol can be
included for provider’s and case managers within a clinically

Introduction
The healthcare field is forever changing; therefore, nurses need
to be on top of new and improved evidence-based practice.
Evidence-based research needs to be done to maintain patient
safety and best possible outcomes. One issue that should be
investigated is how to manage chronic headaches effectively.
Patients with chronic headaches take up resources, time and
space in the emergency room. According to Migraine Research
Foundation (2017) migraines are a prevalent neurological
disease which affects 38 million people in the United States and
one billion worldwide. These patients typically have a hard
time with the management of their headaches once they start.
Most patients will go through their plan of care, if they have
one, and then end up in the emergency department (ED) for pain
management when all else fails. Coming up with an educational
plan, to educate these patients will help decrease emergency
department visits and have better patient outcomes at home.
Background
In a review of emergency room visits within Sentara Quality
Care Network, for inappropriate emergency department visits,
headaches were noted as the number one reason for visits.
Decreasing the emergency department visits for headaches will
be a start to providing these patients with appropriate care for
better outcomes. The practice setting would take place with
case managers in an outpatient setting, over the phone or at the
6. patient’s home. This writers’ clinical setting for this project
takes place with Clinical Integrated Network of approximately
3,000 physicians. This network has contracts with several self-
ensured entities and an insurer in making sure that they are
giving the best care possible. This network has case managers
who call and visit with patients to make sure that they are
following their treatment plan. These case managers can look in
their database to see which patients have been discharged from
the emergency and for what. They can identify which patients,
once discharged, will benefit from this protocol. They can then
begin using this new educational protocol as part of their case
management and be able to track them to make sure they are
staying out of the emergency room. The trigger for the case
managers will be when the patients visit the emergency
department for a complaint of a headache. These case managers
will be able to poll those who have visited the emergency
department within the next six months with a questionnaire.
They will work closely with the patients as well as the providers
to make sure that the protocol is being followed in which they
will include the triple aim; right care, right place and right cost.
This education will help the patients to receive the best possible
evidence-based care and will have a better quality of life.
Problem Statement
Within the United States, there is an overuse of the emergency
department (ED). According to the Center for Disease Control
and Prevention (CDC) (2017), there are approximately 130.4
million emergency department visits per year. According to
Weiss, Wier, Stocks, and Blanchard (2014), there were over 131
million emergency department visits in 2008. This report shows
that there has not been much of a decrease in emergency
department visits over the last nine years. Patients are using the
emergency department for primary care issues which hinder the
emergency department’s ability to be available and ready for the
sick patients that truly need the emergency department care.
7. According to Lucado, Paez, and Elixhauser (2011), there were
over 3 million emergency department visits where patients
complain of headaches as the first line diagnosis; also, there
were 2.5 million emergency department visits where a headache
was the secondary diagnosis. Headaches are approximately 4.5
percent of all emergency department visits. According to
Minen, Loder, and Friedman (2014), there are over 800,000
emergency department visits for migraine patients annually.
According to Smitherman, Burch, Sheikh, and Loder (2013), the
third leading cause of emergency department visits was head
pain. This evidence shows that there has been a slight increase
in emergency department visits for headaches. According to
Skinner, Blanchard, and Elixhauser (2014), there was a one
percent increase from 2006-2011 in emergency department
visits for headaches. Headache patients are using the
emergency room more than they should. The emergency room
has a high number of emergency room visits each year which is
causing overcrowding in the emergency rooms. There needs to
be a way to get the non-emergent visits out of them and back to
their primary care or educate them to care for themselves at
home. Headaches are the number one diagnosis in the
population served by the Network. Getting these patients, the
right tools, education, and treatment plan will be essential to
keeping these patients out of the emergency room.
Purpose Statement
The purpose of this quality improvement project is to examine
the effect of a targeted intervention by a Nurse Care Manager
on the repeated use of the emergency department for treatment
of a headache. This doctoral project will aim to implement
structured patient counseling, based on a treatment protocol,
and measure the rate of return visits to the emergency
department for a headache. It is proposed that the number of
emergency department visits will decrease by ten percent for
headache/migraine patients in an insured population. A study
8. done by Martinez et al. (2015), found that migraine patients
who received a brochure and read the brochure have improved
migraine control. Education will be the most important to help
these patients manage their migraines at home. They need long-
term management, and the emergency departments’ primary
concern is to decrease a visit to the emergency department for
treatment of a headache. Developing an education plan that
works will be the primary goal.
Project Question
The guiding practice question that will be addressed to identify
the gap for headache patients. The question for this project will
be: For adult patients, aged 30-65, patients presenting to the
emergency department with headaches, will an education
protocol decrease return of emergency department visits by 10
percent within three months compared to the previous six
months? This project will be a quality improvement initiative
based on evidence focused on a specific issue or need. The issue
is that headache patients use the emergency department for
headaches, which is not an emergency. Patients do not have a
plan or may not be educated on managing their
symptoms/disease. This project aligns with other scholarly
project conducted by doctoral prepared nurses because it is an
application of evidence-based practice. This project will
evaluate current literature within the last five years, peer-
reviewed journal articles, case studies as well as randomized
control studies to support this change in practice. Institutional
Review Board approval will be obtained, data will be collected
from this health system using a retrospective chart review
regarding the number of patients that utilize the emergency
department for headaches.
Framework for the Project
The theoretical framework that was chosen for this project was
9. Pender’s Health Promotion by Nola Pender. This theory
supports a positive dynamic state rather than the absence of
disease. Pender’s Health Promotion Model will be able to
support and help change the patients’ behavior. According to
Khodaveisi, Omidi, Farokhi and Soltanian (2017) the Pender’s
Health Promotion Model was used to plan for and change
behaviors successfully for nutritional education. There will be
no way to get rid of headaches completely; we can effectively
manage them with appropriate behavior changes and education.
This model has four assumptions and thirteen theoretical
statements. According to Nursing Theory (2016), the major
concept of this theory is the individual characteristics and
experiences and prior behavior. Changing the current
educational level and behavior will help to control headaches.
Health Belief model is another theoretical framework in which
will help support this change because this model focuses on
behavior change as well. According to Jones et al. (2014), they
found that HBM will achieve optimal behavior change
successfully if perceived barriers, benefits, and self-efficacy
were targeted. Nursing Theories (2012) stated that the Health
Belief Model addresses someone’s behavior.
Implications for Social Change
The implications for social change that this project will prove
that headache patients who suffer from headaches, to become
better controlled so that they will be able to live a better quality
of life. Many of these patients who suffer from uncontrolled
headache pain miss many days of work and family time alike.
According to Serrano et al. (2013), they found that loss of
productive time was costly and increased more rapidly with the
patients that had a chronic migraine. Using structured education
can help improve headache control. Braschinsky et al. (2016),
found that general practitioners can improve headaches with
structured education. Utilizing education means that these
headache patients would have a chance to have better control,
10. better life outcome and overall better quality of life.
Definition of Terms
Chronic Headache is defined as headaches from time to time for
a long period of time (Mayo Clinic, 2017).
Cluster Headache is defined as a cyclical patterns or clusters
which awakens patient in the middle of the night with intense
pain in or around one eye on one side of the head (Mayo Clinic,
2017).
Education is defined as the process of receiving information.
(Merriam-Webster.com, 2017).
Emergency Department is defined as a department in the
hospital for the medical and surgical care of patients arriving at
the hospital in need of immediate care. Emergency department
is also called emergency room or ER. (MedicineNet.com, 2017).
Headache will include a chronic headache, migraine, tension-
type headache, and cluster headache.
Management is defined as the process of dealing with or
controlling things or people. (Merriam-Webster, 2017).
Migraine headache is defined by severe throbbing or pulsating
sensation and often accompanied by other symptoms, nausea,
vomiting, and extremely sensitivity to light and sound, which
can be disabling in nature. (Mayo Clinic, 2017).
Nurse Case Management is defined as a collaborative process of
assessment, planning, facilitation, care coordination, evaluation
and advocacy for options and serves to meet the individual’s
and family’s comprehensive health care needs that is led by a
nurse. (ANCC, 2017).
Tension-Type Headache is defined as a diffuse, mild to
11. moderate pain in your head that often described as a tight band
feeling around head. (Mayo Clinic, 2017).
Assumptions
The assumptions for this project are if patients receive
appropriate headache education, then headache patients will
have better pain control and will decrease the utilization of the
emergency departments. Headache education is defined as
headache diary, identifying triggers, when to use certain
medications, side effect, and purpose of the medication and
when to call your provider. The systematic literature review
indicates that there is a correlation between headache education
and better headache pain control. Martinez et al. (2015) found
that migraine patients who received an educational brochure and
read the brochure have improved migraine control. This
includes the concept of self-efficacy and will empower the
patient to control their disease.
Scope and Delimitations
The scope of this doctoral project will be to identify headache
patients through their visit to the emergency department. The
patients identified will be contacted by case managers; there
will be a questionnaire that will be used to gain information
regarding their management of their headaches. After the
completion of this questionnaire, the case managers will
implement a nurse-driven headache protocol which will help to
manage the patient’s headaches more effectively. Managing
these headaches more effectively will decrease Emergency
Department visits as well as allow these patients to have a
better quality of life. With these patients not using the
emergency department, this will also start to decrease the usage
of emergency departments leaving more space, time and
resources for those that need it.
12. Limitations
There are limitations for this project. The first limitation is that
there is no control group and may be prone to bias. The
evaluation of symptom improvement is completely subjective.
The researcher acknowledges that headache control can be
complex and include other associated factors. The control of a
headache as well can be associated with other factors which are
not taken into consideration in this study. Study participants
may or may not follow through the education given.
Significance
The significance of this issue is that patients are going to the
emergency departments and they are just providing a quick fix
for the moment. These patients need long term fixes and should
be able to control their headaches for a longer period.
Developing an educational plan for headache patients will be
the key to ensure that they are taught what steps they need to
take, what their plan of action is and therefore will be able to
control their headaches at home. While keeping these patients at
home for treatment will also help decrease emergency
department visits, it will also decrease insurance costs and any
potential out of pocket costs for the patient. According to
Stokes et al. (2011) found that chronic migraine had an
increased amount of total medical costs then episodic migraines
and that therapies that reduce headache frequency should be
important to reducing headache related costs.
Summary
When designing a Doctoral project, the first step is to outline
the problem statement, purpose, and question. One problem that
is identified was the lack of adequate headache control, which
leads to poor quality of life, increase in emergency rooms visits
and increase in medical costs. According to Stokes et al. (2011)
13. found that migraines had a total increase in medical costs.
Nature of the problem, significance to the nursing profession as
well as the implications for social change will also be
discussed. Overall, giving patients more control of their
headaches will lead to them having a better quality of life.
