resource to guide in managing a specific
problem and sharing responsibility in patient

 Ideally, it would result in “a closed referral
loop”, in which the referral appointment is
completed and results are then shared with
the patient’s referring physician.
 Apart from consultants and hospitals, a
referral might be considered to family
physician colleagues with special interests or
expertise, and other members of the primary
health care team, such as physiotherapists,
dietitians, and social workers.
