Process Hazard Analysis (PHA)
The Process Hazard Analysis (PHA) is the core of a Process
Safety Management (PSM) program. It refers to a systematic
review of a process to identify potential hazards and operability
problems through the use of several different methodologies.
OSHA Process Safety Management (PSM) (29 C.F.R. §
1910.119(e)) and EPA Accidental Release Prevention
Requirements for Program 3 (40 C.F.R. § 68.67) require a hazard
analysis for all subject processes. PHA methodologies include:
 Hazard and Operability Study (HAZOP)
 What-If?/Checklist
 Layer of Protection Analysis (LOPA)
 Failure Modes and Events Analysis (FMEA)
 Bow-tie analysis
