From Silos to Legos presents a new healthcare delivery paradigm that moves away from the traditional siloed approach. The current system operates like separate silos that don't communicate, but the new paradigm envisions a more integrated approach where all parts work together like interlocking Lego blocks. This aims to improve care coordination and reduce waste. The presentation discusses how policies and programs can help dismantle silos and build more connections between different areas of the healthcare system.
DirectTrust.org is an independent non-profit organization created by and for participants in Direct exchange to develop and promote rules and best practices to maintain trust. Its goal is to foster widespread confidence in Direct exchange of health information. The organization has over 80 members from healthcare and technology organizations. It has working groups focused on security, compliance, and certificate policy. Direct exchange allows secure transmission of health information like test results, referrals, and clinical summaries to support meaningful use. Security, privacy, and trust are essential for health information exchange to succeed. Digital certificates are important for verifying identity and encrypting messages in Direct exchange. Issues around who can be certificate authorities and what identity verification is required still need resolution.
The document discusses the technical details of direct trust infrastructure, which is built on public key infrastructure (PKI). PKI uses public and private key cryptography, digital certificates, encryption, and digital signatures to enable authentication, secure messaging, electronic signatures, and data encryption. It describes how certificates bind a public key to identity information and are issued by a certification authority (CA) with the help of a registration authority (RA) that verifies identity documentation.
The Direct Project aims to create standards and services to enable secure and meaningful health information exchange over the internet between known participants. It addresses current issues with communication relying mainly on mail and fax by providing a secure electronic alternative. The Direct Project specifications are developed collaboratively by over 50 organizations and 200 participants through weekly calls and meetings. The end goal is widespread adoption of the Direct standards by late 2012 to facilitate health information exchange and help meet meaningful use requirements.
The document discusses how the cloud can serve as a platform for better health through collaboration on data, clinical research, and data protection. Some key points:
1) The cloud allows for data collaboration through storage and transfer services, identity and access management, and encryption features.
2) It enables clinical research through services that power applications for patient-specific education, clinical knowledge systems, and personalized medicine.
3) The cloud provides data protection and disaster recovery using continuous online backup, multiple regions and availability zones, and dedicated instances for compliant workloads.
4) It can extend existing corporate datacenters and applications through services like Direct Connect.
The document summarizes that health plans are well positioned to improve healthcare through technology and payment evolution. It discusses how IT-enabled care collaboration can deliver higher value for purchasers by reducing resources, substituting lower cost options, and avoiding complications. It suggests health plans can support providers and value-based care by changing reimbursement models and leveraging plan assets to partner with providers through tools, analytics and shared care management services.
This document discusses the evolution of e-health in Medicaid managed care. It describes how e-health has progressed from mainly administrative information exchange between payers and providers through portals, to including more health information like care gaps, medications and clinical summaries. The next phase involves bi-directional health information exchange between payers and electronic health records. The document provides an example of how discharge information from hospitals could be intelligently routed from the HIE to primary care providers and care managers' EHRs or portals.
The document discusses modernizing health care through synchronization. It proposes personalized tools and resources to help move from reactive to proactive care by tracking patients' progress towards clinical and economic goals. This includes a nurse desktop technology for engagement, an eHealth portal with intuitive design, and mobile health coaching. Physician-driven population management is also discussed as a way to significantly improve risk factor reduction through data exchange, referrals to coordinators, and pay for outcomes programs.
The document discusses health information technology and the potential for an open source platform approach. It notes that an open environment could allow for faster innovation, higher quality products, and lower costs. An open platform with reusable components could enable users to self-configure integrated experiences. However, platforms alone are not enough and cultural and market changes may also be needed to fully realize this vision of a more open and learning-oriented health system.
This document discusses substitutable medical apps and reusable technologies. It describes how a blood test app was overhauled by one developer in one week using an open API, documentation, and example data. The app integrated data authentication and user interfaces. The document advocates for open formats and unrestricted, realistic example data to promote innovation. It notes that while CCR and CCD formats are licensed for temporary storage and single printing, a blog later highlighted a lack of freely available examples. Overall innovation in healthcare apps requires open platforms, documentation, and access to sample clinical data.
This document discusses the importance of collaborative care and care coordination for healthcare delivery systems. It notes that solo practice is no longer a sustainable business model and that fee-for-service payments have limitations. The document provides evidence that care coordination can reduce costs through fewer hospital admissions and readmissions without worse health outcomes. It also shows that patients experience a lack of communication and information sharing between their different doctors. To improve care coordination, mobile access to patient data and collaborative workflows are seen as critical, as mobile devices are increasingly how physicians access information. The right devices and secure mobile computing are needed to enable these new care coordination models.
The document discusses the future of platforms in healthcare. It argues that platforms are necessary, not just nice to have, to address the complexity of healthcare delivery. Platforms can unlock patient value and competitive advantage by connecting different parts of the healthcare system. The document provides recommendations to healthcare organizations, including promoting platform neutrality, improving data storage and liquidity, and reviewing contracts to ensure interoperability and data ownership.

Effective Application Security in Software Delivery lifecycle using Deployment Firewall and DBOM
The modern software delivery process (or the CI/CD process) includes many tools, distributed teams, open-source code, and cloud platforms. Constant focus on speed to release software to market, along with the traditional slow and manual security checks has caused gaps in continuous security as an important piece in the software supply chain. Today organizations feel more susceptible to external and internal cyber threats due to the vast attack surface in their applications supply chain and the lack of end-to-end governance and risk management.
The software team must secure its software delivery process to avoid vulnerability and security breaches. This needs to be achieved with existing tool chains and without extensive rework of the delivery processes. This talk will present strategies and techniques for providing visibility into the true risk of the existing vulnerabilities, preventing the introduction of security issues in the software, resolving vulnerabilities in production environments quickly, and capturing the deployment bill of materials (DBOM).

As machine learning integrates deeper into human-computer interactions, the concept of epistemic interaction emerges, aiming to refine these interactions to enhance system adaptability. This approach encourages minor, intentional adjustments in user behaviour to enrich the data available for system learning. This paper introduces epistemic interaction within the context of human-system communication, illustrating how deliberate interaction design can improve system understanding and adaptation. Through concrete examples, we demonstrate the potential of epistemic interaction to significantly advance human-computer interaction by leveraging intuitive human communication strategies to inform system design and functionality, offering a novel pathway for enriching user-system engagements.

This talk will present strategies and techniques for providing visibility into the true risk of the existing vulnerabilities, preventing the introduction of security issues in the software, resolving vulnerabilities in production environments quickly, and capturing the deployment bill of materials (DBOM).

zkStudyClub - Reef: Fast Succinct Non-Interactive Zero-Knowledge Regex ProofsAlex Pruden
This paper presents Reef, a system for generating publicly verifiable succinct non-interactive zero-knowledge proofs that a committed document matches or does not match a regular expression. We describe applications such as proving the strength of passwords, the provenance of email despite redactions, the validity of oblivious DNS queries, and the existence of mutations in DNA. Reef supports the Perl Compatible Regular Expression syntax, including wildcards, alternation, ranges, capture groups, Kleene star, negations, and lookarounds. Reef introduces a new type of automata, Skipping Alternating Finite Automata (SAFA), that skips irrelevant parts of a document when producing proofs without undermining soundness, and instantiates SAFA with a lookup argument. Our experimental evaluation confirms that Reef can generate proofs for documents with 32M characters; the proofs are small and cheap to verify (under a second).
