The document discusses limitations of continuous glucose monitors and how patient communities worked to develop solutions like Nightscout and OpenAPS. It then talks about efforts in the type 1 diabetes prevention community, including discussing diet, supplements, and current research. The author expresses a willingness to participate in home testing and trials to help researchers, and asks about recommended genetic tests, inflammation markers, and prevention "cocktails".

Inviza® Health's Founder and CEO, Dr. Robert Andosca's invited presentation and live demonstration given to SEMI MEMS & Sensors Executive Congress (MSEC 2022) in San Diego, CA on Tuesday, October 11, 2022.
Our Inviza® Sole 1.0 smart remote patient monitoring insoles planned to be launched in Q1 2023 self-charge the wearable technology's battery via steps ... walking, running, exercise, limping, crawling, and more. Our smart insoles distinguish the difference between step types as well. All vital signs measured are categorized by step type. Vital signs measured together are heart rate, resting heart rate, respiratory rate, body temperature, and % blood oxygen saturation "SpO2". Blood pressure, body weight, and blood glucose (for type 2 diabetic patients) sensors will be added to Inviza® Sole 2.0 targeted to be launched in 2024. All sensors are clinically, highly accurate and 100% commercial -off-the-shelf (COTS). In addition, our patents pending piezoelectric energy harvesters, power management electronics, wireless charging, and thin rechargeable battery, and LTE cellular with GPS, Bluetooth, Wi-FI chips, and antenna are 100% COTS as well.
Our Inviza® Sole 1.0 comes with our Inviza® Life 1.0 digital health mobile app for patients-users, and our Inviza® Care 1.0 digital health / telehealth Inviza Cloud dashboard.
There are 18+ smart health, smart fitness (e.g. highly accurate SmartSteps™ and SmartCalories™) and smart safety (e.g. fall detection, GPS location) features. Please review the presentation to find out what they all are.

Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol
that is rapidly distributed in the body and brain. Ethanol alters many
neurochemical systems and has rewarding and addictive properties. It
is the oldest recreational drug and likely contributes to more morbidity,
mortality, and public health costs than all illicit drugs combined. The
5th edition of the Diagnostic and Statistical Manual of Mental Disorders
(DSM-5) integrates alcohol abuse and alcohol dependence into a single
disorder called alcohol use disorder (AUD), with mild, moderate,
and severe subclassifications (American Psychiatric Association, 2013).
In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,
comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and
pharmacotherapies for AUD.

5. CGM BROUGHT INSIGHT, BUT WAS LIMITED
• Distance: you can never be more than 30 feet away from child
• Problematic for:
• School
• Sports
• Sleep overs
• Life
• Only one person with the receiver can know what is going on
• People sleep through alarms
• Diabetes fatigue / burn out
#WeAreNotWaiting was born when
parents created solutions for these
limitations, and started sharing them
with others
6. COWBOY HACKERS TO THE RESCUE
• Genius hackers reverse engineer existing medical equipment to build better tools
for consumers
• Picking up signal / data from receiver
• Decoding secure communication
• Raising the ire of device makers and one of their CEOs who labeled them “Rogue
Cowboy Hackers”
• Upload the data to the Cloud (MongoDB)
7. Nightscout – 4+ years
WHAT #WEARENOTWAITING LOOKS LIKE
OpenAPS (2.5 years ahead)
Pebble watchfaces
(2 years ahead)
Android APS & Loop Apps
(1.5+ years ahead)Multi-child monitoring in Romania
Reports with charts and
predicted A1C (3+ years)
Autotune – analyzes settings,
recommends changes (1+ years)
10. • Knowing you have no hope?
• Knowing you are going to have multiple kids with T1D?
• Knowing in advance?
• Angry, sad, fearful, cursed, depressed, like we are failing our kids
REALITY SINKS IN FOR US
15. WHAT HAS MY FAMILY BEEN WILLING TO DO
• Home CGM testing for Julie and I (with
our own OGTT)
• Vitamins/supplements
• LDN
• Probiotics
• No Dairy (or A2 only)
• No gluten
• Lowish carb
• Food testing
• Anything else you geniuses could
recommend
16. GOALS
• What can the patient community do or provide to help researchers?
• Consensus on safe and effective and decently affordable components
• Affordable is debatable for people that are desperate, FYI
• What tests should we be doing to set baseline and monitor progress
• Genetic testing. What is the specific test? Something about Class 2.
• What tests should we be doing for inflammation?
• Find out what should be in the “cocktail” and dosing levels
Editor's Notes
jjj
I’m Matt…
More of a geek…less of a presenter.
Daughter was diagnosed 4 year ago
Beat the Bridge / JDRF event 3 months before diagnosis
On vacation…wasn’t herself
We are an adventurous family
3 weeks after diagnosis we were backpacking hours from medical attention but I felt confident in our abilities.
Anyone see the pink camo case?
As we acclimated, we did some of the normal things
- Got a CGM
- Our endo mention the NightScout folks aka the #WeAreNotWaiting folks
(next)
CGM in the Cloud
Now 26K members
Was started by folks that wanted to be able share with other families how you could get number for your child anywhere in the world...more than the 20 feet the commercially available products allowed
Geeks that do this in their spare time because they wanted to give their children more freedom (sleepovers / parties) and safety (knowing what your kids' BG is all the time is peace of mind)
NS – Allows ANYONE in the world to track / backbone of Artificial Pancreas - around for 4+ years now
* My daugthers graph – remotely control her pancreas
OpenAPS – Closed loop artificial pancreas system that Ella is on
* She was number 28 in the world and there are over 700 folks on it now
* 2.5 years before first commercially available AP...helped pave way for Medtronic 670
* APS named after StarWars characters
Pebble - Two years before commercially available product around
* This iteration allow parents to not burnout from being always on…has all the info, will buzz when you need to address issues
* Even when system loses contact, it will buzz you
Temp Targets / Remote control
Custom alerting that makes predictions way out to avoid highs or low
Actual carb or insulin recommendations
This is because the system is built with safety in mind and won’t take drastic measures
Multi-child monitoring in Romania
* Built on NS but the OpenSource / #WeAreNotWaiting community built it
Android APS
* Another looping alternative
Loop or LoopKit
* Mac friendly Artificial Pancreas
Visualizations
*NS reports showing time in range and predicted A1C
Autotune - around for 1+ years
* OpenAPS folks built this
* nothing commercially available
So switching back to our timeline / story
- Part of acclimating meant T1D camp and participating in providing data to TrialNet
- Little did we expect to get a call telling me that Julie had 4 AB and I had 3.
- So we participated even more
- (slide)
- And they clearly are willing to work with all sorts of people
Told we have antibodies (slide)
- can infer timeline
- learn there is nothing we can do for Julie based on her age and she didn't test positive for the marker needed for Oral Insulin...neither did I.
- willing to do anything diet / vitamins / supplement wise...even if only best guesses / prevailing wisdom
- told nothing we can do except look at vitamin D levels
What does this do you?
- Knowing you have no hope?
- Knowing you are going to have multiple kids with T1D?
- Knowing in advance?
- Angry, sad, fearful, cursed, depressed, like we are failing our kids
- What is the point of knowing when you can do nothing?
* Pictures of adorable Julie
Worked with Dr Jody, a T1D naturapath who’s work definitely help lower our titres but based on OGTT figured we were still tipping
I was looking for answers and help so:
- translated to research
- used my Rogue Cowboy community to put out the call
- found Sonia
- got cocktail / had hope
* The cocktail of vitamins we take have been winnowed down to deal with taking handfuls of vitamins a day (picture)
- saw titres getting better
- Julie dropped an AB
- we were super strict with diet, low carb, taking every vitamin under the sun
- slacked off a bit
Wanted to help other families that were desperate
- Need to do something
- kills me to see posts like this
So Sonia and I collaborate and Children with Diabetes Research Foundation paid for us to create a website and FB group (slides)
- Wife and I own digital marketing agency
- 750 members (slide)
Community is super grateful for the info
