The inclusion of evidence-based practice provides nurses with the scientific research and experience to make a comprehensive decision. The practice enables the nurses to re-evaluate the risks and only adopt the best mechanism to ensure an improved patient outcome. Patients are also able to receive the best available outcomes. It is very advisable to move the nursing practice to be evidence-based to ensure that there is patient-centered care that is safe, inclusive, and effective. However, there have been barriers towards this progress since only 15% of U.S practice is evidence-based. One of the barriers which have led to lagging behind in adopting evidence-based practice is nurse shortage. Evidence-based practice requires massive documentation and research together with increased testing and experience. This requires a large human resource which is not available due to nurse shortage across the united states (Stavor et al., 2017). This has acted as a barrier towards the goal of moving practice to evidence-based. The government should employ more nurses and also dedicate some of the workforces specifically to matters to do with shifting traditional caregiving to EBP.
The second barrier is unsupportive administration. Research indicates that over 70% of nurses know about evidence-based practice, but the barriers to the practice in a clinical setting make it hard for them to adopt it. To move practice to EBP requires active collaboration from all stakeholders and more so from the administration of the healthcare setting. However, most administrations have been termed as unsupportive for the move due to the challenges of resources involved in the move. EPB presents a huge cost in the beginning due to its data requirements. However, it is able to reduce the cost of healthcare by 35% after its implementation. Lack of support from the management makes it hard to move nursing practice to EBP in a clinical setting since it’s a collaborative activity that requires dedicated and goal-oriented leadership (Duncombe, 2018). Policies and regulations should be created which force the push to enable the administration of various healthcare to have no otherwise but to comply in the shift.

Whether you are talking about intrapersonal problems or patient care problems, no matter the setting, there will always be something that can be improved. In my facility, I am determined to address the problem that is heavily influencing hospitals: hospital-acquired diseases. This is a serious issue, since a large percentage of patients (over 3%) acquire a hospital-acquired disease at some point during their stay at a given healthcare facility. This can be caused by a variety of reasons, but the most common of which is the absence of proper sterility. By having tools, supplies, and healthcare providers with little or incorrect sterilization techniques, then there is an indeterminate amount of diseases that a patient with a likely already compromised immune system. Furthermore, these hospital-acquired diseases can also affect the healthcare professionals transmitting them, since the providers themselves are the vector for the disease. The project would then consist of a new set of policies that would require more intense analysis of sterilization techniques, including actions before sterilization, during sterilization, and after sterilization (transportation, use, etc.).
By localizing the area, or areas, in which sterilization protocol fails, we will be able to successfully reduce the amount of hospital-acquired diseases an individual patient will experience. Maximum minimization of these diseases is essential to provide a healthcare environment where patient care flourishes, but is also efficient in its usage of funds and time spent by professionals. For example, if a patient receives strep from a medical professional, that patient will require further care; also, the provider will possibly also suffer from the transmitted disease, meaning that person may not be able to practice and therefore put a dent in the hospital’s means. In order to prevent this, implementation of the aforementioned guidelines must take place, since these will allow for a much more strict view of the sterilization techniques. However, a complete rehaul of the methods of sterilization will require time, funds, and strong interprofessional communication to make sure there are no lapses at any point the renewed process. Departmental and funding approving is required, but I believe that this problem is essential enough to solve that it will result in quick approval.

Nursing is among the fastest-growing occupations in the United States through the nation still faces a significant shortfall of qualified nurses. The shortage may
continue to be highly essential due to the aging population. The U.S. is speculated to go through a shortage of Registered Nurses which is expected to maximize as
baby boomers age and the need for health care maximizes. The Registered Nursing workforce is speculated to grow from 2 million in 2019 to 3 million in 2029, a rise
of 220,000 and 7%. Impact of Nursing Shortages in an Organization In 2014, a study was conducted in American hospitals regarding the nursing shortage. They found
that a rise in nurses; workload by one patient maximized the possibility of dying in 30 days of admission by 7%. The researchers as well found out that each 10% rise
in bachelor’s degree nurses was related to a drop in patient death by 7% (Haddad, Annamaraju, & Toney-Butler, 2020). Insufficient nurse staffing is as well associated
with high patient mortality rates. In facilities with a limited number of nurses, the mortality risk for patients tends to be about 6% more on units that were under-
staffed in comparison to the well-staffed units. When a nurse’s workload is high due to high patient turnover, mortality risk as well tends to rise. Based on a study con-
ducted by the American Association of Colleges of Nursing (2020), there tends to be 4.9 fewer death in 10000 patients on intensive care units that are staffed with a

Coordination of patient care plays an important role in positive health outcomes; however, it is becoming more of a challenge as the United States’ population is aging, diseases and treatments are becoming more complex and technological costs are rising (Seyedamini, et al., 2019, p. 6). This coordination of care is known as clinical integration, which can be defined as “the coordination of patient care across providers, settings and and time to achieve safe, effective, efficient and patient-focused care” (Regis College, n.d). Clinical integration has the potential to lower health care costs, improve efficiency and quality of care (Seyedamini, et al., 2019, p. 7).
However, in order to facilitate efficient clinical integration, providers need proper financing, cultural adaptation and supportive resources (Seyedamini, et al., 2019, p. 7). Challenges that providers and patients face with clinical integration include continuously updating of medical techniques, equipment and computer-based information systems, increased complexity of care making communication between providers more difficult, as well as increased insurance coverage under the Affordable Care Act causing a higher demand for providers (Shi & Singh, 2019, p. 138). According to Haughom (2017), our modern health care system requires “comprehensive care management systems” (para. 8). He states in addition to electronic medical records, providers need a system that contains the five core competencies of care management including, 1) data integration, 2) patient stratification and intake, 3) care coordination, 4) patient engagement, and 5) performance measurement.
Currently, there is a comprehensive smartphone system being developed that has yet to be fully launched known as FHIR (Fast Healthcare Interoperability Resources). This system would allow providers, researchers and patients to store and share medical information all in one place (Rae-Dupree, 2019, para. 3). This system would support clinical integration and simplify the current complex process of sharing data between providers and patients, which in turn could result in greater efficiency in healthcare delivery (Regis College, n.d). However, this system is still working on gaining support from providers and institutions. They are also facing challenges with how to properly protect patient information in the cyber world (Rae-Dupree, 2019, para. 24).
With newly developing technologies to improve clinical integration, such as FHIR, health care professionals must be properly trained and adhere to new systems to achieve the intended outcomes of improved quality of care and reduced costs. New delivery models being used to replac.

Nearly fifteen years ago, the Institute of Medicine published the “To Err Is Human” report, which exposed the substantial impact of medical errors in the US healthcare system and called for a dramatic system change, including an improved understanding of those errors (McCarthy, Tuiskula, Driscoll, & Davis, 2017). Medical errors are considered to be failure to achieve the original goal or plan of action, and these errors may range from a patient falls to a mistake in the operating room. Not only do medical errors cause harm to the patient and jeopardize the patient’s trust, but they also cause a financial strain for the health system (“To Err is Human,” 1999). One of the contributing factors to medical errors is the lack of effective communication between doctors who are treating the same patient. This results in healthcare providers overprescribing medications for patients as well as increases the possibility of a patient having unnecessary tests or procedures performed. The report’s four-tiered approach includes:
· Focusing on creating a stronger foundation of education on patient safety
· Mandating a nationwide reporting system to encourage timely reporting of errors
· Increasing the standards of performance for healthcare providers
· Taking advantage of the security that safety systems offer (“To Err is Human,” 1999)
Creating a strong educational foundation for patient safety is most important. Healthcare personnel are much more likely to actively participate in reporting systems, encourage one another to perform at a higher level, and take advantage of safety systems when they are well educated on patient safety and the implications of medical errors. The reporting system seems to provide the least amount of impact on patient safety as they can result in losing patient trust in certain healthcare systems. The healthcare system as a whole has made progress in establishing a safe environment for patients when they are in need of care.

Despite continuing evidence of problems in patient safety and gaps between the care that patients receive and the evidence about what they should receive, efforts to improve quality in healthcare show mostly inconsistent and patchy results.

This always takes much more time and energy than anyone anticipates. It is worth investing heavily in data from the outset. Assess local systems, train people, and have quality assurance.

My change proposal is being implemented in the ICU. The intervention is implementing communication tools and processes that are evidence based to improve nursing sensitive indicators in the ICU. The internal stakeholders are the ICU staff and the hospital. The external stakeholders are the community that is served by the hospital. Our hospital works off of a relationship-based care (RBC) model. RBC is a culture transformation model and an operational framework that improves safety, quality, patient satisfaction, and staff satisfaction by improving every relationship within an organization (Gallison & Kester, 2018). The core of workforce engagement is the reignighting of joy and meaning for nurses. The joy and satisfaction in having a sense of accomplishment and significance in the work through processes leading to successful outcomes. RBC speaks to how we treat patients, family, and each other. Internal stakeholders all work off this model in this organization. However, covid greatly challenged relationship based care principles by limiting how we interact with each other and our patient families. That in person piece is missing for many patients still. In this organizational transition back to pre-covid practices, meetings, and policies staff are looking for guidance to unify and strengthen the workforce. It is a good segway into external stakeholders. Our nurses and other staff are also members of the community served by the hospital. Therefore, the internal stakeholders all face the real fact that they too receive their care here and have an interest in the quality of care provided. This community funded hospital has been influenced by local donors, architects and artists. Donors play a large role in celebrating the staff and creating this sense of meaning and significance for hospital staff. In a relationship based care model, these gestures serve a huge purpose and allow the hospital to recognize staff in unique ways. The positive factor here is that the nurses have come out of this powerless feeling covid left them with. Small gestures that build trust between nursing and management create a more productive work environment. This is done through clear concise communication, open discussion, and acting on feedback from staff.

Abstract
This study evaluated the correlation of an emergency department embedded care coordinator
with access to community and medical records in decreasing hospital and emergency
department use in patients with behavioral health issues. This retrospective cohort study
presents a 6-month pre-post analysis on patients seen by the care coordinator (n=524). Looking
at all-cause healthcare utilization, care coordination was associated with a significant median
decrease of one emergency department visit per patient (p G 0.001) and a decrease of 9.5 h in
emergency department length of stay per average visit per patient (pG0.001). There was no
significant effect on the number of hospitalizations or hospital length of stay. This intervention
demonstrated a correlation with reducing emergency department use in patients with behavioral
health issues, but no correlation with reducing hospital utilization. This under-researched
approach of integrating medical records at point-of-care could serve as a model for better
emergency department management of behavioral health patients.

likely to be frequent users of health services.1–7 Brennan et al. found that patients with at least one
primary psychiatric visit to the emergency department (ED) were 4.6 times more likely than those

Nontraditional nursing roles have emerged due to technological growth, healthcare reform, and demand for preventative and community-based care. Areas such as patient safety, quality improvement, health informatics, behavioral health, and care coordination have become important domains for nursing. The roles of occupational health nurses, case managers, HEDIS nurses, quality assurance nurses, and nurse educators are growing due to a focus on wellness, chronic disease management, and reducing healthcare costs. These nontraditional nursing roles offer salaries comparable to registered nurses and are projected to be in high demand over the next decade, especially in large cities such as Houston, Chicago, and Los Angeles.

Change Proposal: Reimplementation of a Bedside Shift Report
The identified clinical problem that is relevant to nurses in the workplace is the bedside shift report (BSR). The bedside shift report may be used to help the development of teamwork, ownership, and accountability thus increasing the satisfaction of the nurses. The reason for selecting the reimplementation of a bedside shift report is because; at a targeted facility the implementation of the bedside shift report is becoming poor. This has led to the poor compliance with the standards of practice, and increase costs in the facility. Also, to have a reduced cost by removing overtime pay and scrubbing off legal errors which result in legal fees (Clark et al., 2020). (*Re-phrase this sentence, avoid slang).
The reimplementation of a bedside shift report is important for the nurse practicing in the organization. Bedside shift report assists in improving the experience of the client and improving the handoff care between the nurses through including the family and client in the delivery of care. Bedside shift report is leading to the introduction of a change in the nursing culture i.e. introduction of various methods of communicating patient-specific care at a shift transformation. Shift reports given at the bedside is helping in changing the manner in which nurses are communicating and practicing (Clark et al., 2020).
At the targeted facility, the execution of the bedside shift report is poor and this is associated to the lack of knowledge concerning the role of bedside shift reporting on the patient outcome. The healthcare providers at the facility are not directly engaged in the provision of healthcare. There is poor communication of the goals of the bedside shift reporting and their importance about the positive outcomes and benefits that are involved in the change (Clark et al., 2020).
According to the study by Gregory et al., (2014), the process of assessing the transformation in practice that involves transitioning of the bedside nurse shift handoffs is important. The evaluation process is helpful in the re-implementation plan to help in improving the areas of weaknesses. The evaluation process of the success in the implementation in the practice is assessing the perception of the nurses about the proposed changes i.e. the re-implementation plans. There is evidence showing the positive impacts of the bedside shift report in the safety of the patient, the control of the pain, the satisfaction of the patients, and improvement in the nurse to patient or nurse to nurse communication (Gregory et al., 2014).

Whether you are talking about intrapersonal problems or patient care problems, no matter the setting, there will always be something that can be improved. In my facility, I am determined to address the problem that is heavily influencing hospitals: hospital-acquired diseases. This is a serious issue, since a large percentage of patients (over 3%) acquire a hospital-acquired disease at some point during their stay at a given healthcare facility. This can be caused by a variety of reasons, but the most common of which is the absence of proper sterility. By having tools, supplies, and healthcare providers with little or incorrect sterilization techniques, then there is an indeterminate amount of diseases that a patient with a likely already compromised immune system. Furthermore, these hospital-acquired diseases can also affect the healthcare professionals transmitting them, since the providers themselves are the vector for the disease. The project would then consist of a new set of policies that would require more intense analysis of sterilization techniques, including actions before sterilization, during sterilization, and after sterilization (transportation, use, etc.).
By localizing the area, or areas, in which sterilization protocol fails, we will be able to successfully reduce the amount of hospital-acquired diseases an individual patient will experience. Maximum minimization of these diseases is essential to provide a healthcare environment where patient care flourishes, but is also efficient in its usage of funds and time spent by professionals. For example, if a patient receives strep from a medical professional, that patient will require further care; also, the provider will possibly also suffer from the transmitted disease, meaning that person may not be able to practice and therefore put a dent in the hospital’s means. In order to prevent this, implementation of the aforementioned guidelines must take place, since these will allow for a much more strict view of the sterilization techniques. However, a complete rehaul of the methods of sterilization will require time, funds, and strong interprofessional communication to make sure there are no lapses at any point the renewed process. Departmental and funding approving is required, but I believe that this problem is essential enough to solve that it will result in quick approval.

baby boomers age and the need for health care maximizes. The Registered Nursing workforce is speculated to grow from 2 million in 2019 to 3 million in 2029, a rise
of 220,000 and 7%. Impact of Nursing Shortages in an Organization In 2014, a study was conducted in American hospitals regarding the nursing shortage. They found
that a rise in nurses; workload by one patient maximized the possibility of dying in 30 days of admission by 7%. The researchers as well found out that each 10% rise
in bachelor’s degree nurses was related to a drop in patient death by 7% (Haddad, Annamaraju, & Toney-Butler, 2020). Insufficient nurse staffing is as well associated
with high patient mortality rates. In facilities with a limited number of nurses, the mortality risk for patients tends to be about 6% more on units that were under-
staffed in comparison to the well-staffed units. When a nurse’s workload is high due to high patient turnover, mortality risk as well tends to rise. Based on a study con-
ducted by the American Association of Colleges of Nursing (2020), there tends to be 4.9 fewer death in 10000 patients on intensive care units that are staffed with a

Coordination of patient care plays an important role in positive health outcomes; however, it is becoming more of a challenge as the United States’ population is aging, diseases and treatments are becoming more complex and technological costs are rising (Seyedamini, et al., 2019, p. 6). This coordination of care is known as clinical integration, which can be defined as “the coordination of patient care across providers, settings and and time to achieve safe, effective, efficient and patient-focused care” (Regis College, n.d). Clinical integration has the potential to lower health care costs, improve efficiency and quality of care (Seyedamini, et al., 2019, p. 7).
However, in order to facilitate efficient clinical integration, providers need proper financing, cultural adaptation and supportive resources (Seyedamini, et al., 2019, p. 7). Challenges that providers and patients face with clinical integration include continuously updating of medical techniques, equipment and computer-based information systems, increased complexity of care making communication between providers more difficult, as well as increased insurance coverage under the Affordable Care Act causing a higher demand for providers (Shi & Singh, 2019, p. 138). According to Haughom (2017), our modern health care system requires “comprehensive care management systems” (para. 8). He states in addition to electronic medical records, providers need a system that contains the five core competencies of care management including, 1) data integration, 2) patient stratification and intake, 3) care coordination, 4) patient engagement, and 5) performance measurement.
Currently, there is a comprehensive smartphone system being developed that has yet to be fully launched known as FHIR (Fast Healthcare Interoperability Resources). This system would allow providers, researchers and patients to store and share medical information all in one place (Rae-Dupree, 2019, para. 3). This system would support clinical integration and simplify the current complex process of sharing data between providers and patients, which in turn could result in greater efficiency in healthcare delivery (Regis College, n.d). However, this system is still working on gaining support from providers and institutions. They are also facing challenges with how to properly protect patient information in the cyber world (Rae-Dupree, 2019, para. 24).

Nearly fifteen years ago, the Institute of Medicine published the “To Err Is Human” report, which exposed the substantial impact of medical errors in the US healthcare system and called for a dramatic system change, including an improved understanding of those errors (McCarthy, Tuiskula, Driscoll, & Davis, 2017). Medical errors are considered to be failure to achieve the original goal or plan of action, and these errors may range from a patient falls to a mistake in the operating room. Not only do medical errors cause harm to the patient and jeopardize the patient’s trust, but they also cause a financial strain for the health system (“To Err is Human,” 1999). One of the contributing factors to medical errors is the lack of effective communication between doctors who are treating the same patient. This results in healthcare providers overprescribing medications for patients as well as increases the possibility of a patient having unnecessary tests or procedures performed. The report’s four-tiered approach includes:
· Focusing on creating a stronger foundation of education on patient safety
· Mandating a nationwide reporting system to encourage timely reporting of errors
· Increasing the standards of performance for healthcare providers
· Taking advantage of the security that safety systems offer (“To Err is Human,” 1999)
Creating a strong educational foundation for patient safety is most important. Healthcare personnel are much more likely to actively participate in reporting systems, encourage one another to perform at a higher level, and take advantage of safety systems when they are well educated on patient safety and the implications of medical errors. The reporting system seems to provide the least amount of impact on patient safety as they can result in losing patient trust in certain healthcare systems. The healthcare system as a whole has made progress in establishing a safe environment for patients when they are in need of care.
Challenges for Patient Safety and Steps for Improvement
Despite continuing evidence of problems in patient safety and gaps between the care that patients receive and the evidence about what they should receive, efforts to improve quality in healthcare show mostly inconsistent and patchy results.

Data Collection and Monitoring Systems
This always takes much more time and energy than anyone anticipates. It is worth investing heavily in data from the outset. Assess local systems, train people, and have quality assurance.
Tribalism and Lack of Staff Engagement
Overcoming a perceived lack of ownership and professional or disciplinary boundaries can be very difficult. Clarify who owns the problem and solution, agree roles and responsibilities at the outset, work to common goals, and use shared language.

My change proposal is being implemented in the ICU. The intervention is implementing communication tools and processes that are evidence based to improve nursing sensitive indicators in the ICU. The internal stakeholders are the ICU staff and the hospital. The external stakeholders are the community that is served by the hospital. Our hospital works off of a relationship-based care (RBC) model. RBC is a culture transformation model and an operational framework that improves safety, quality, patient satisfaction, and staff satisfaction by improving every relationship within an organization (Gallison & Kester, 2018). The core of workforce engagement is the reignighting of joy and meaning for nurses. The joy and satisfaction in having a sense of accomplishment and significance in the work through processes leading to successful outcomes. RBC speaks to how we treat patients, family, and each other. Internal stakeholders all work off this model in this organization. However, covid greatly challenged relationship based care principles by limiting how we interact with each other and our patient families. That in person piece is missing for many patients still. In this organizational transition back to pre-covid practices, meetings, and policies staff are looking for guidance to unify and strengthen the workforce. It is a good segway into external stakeholders. Our nurses and other staff are also members of the community served by the hospital. Therefore, the internal stakeholders all face the real fact that they too receive their care here and have an interest in the quality of care provided. This community funded hospital has been influenced by local donors, architects and artists. Donors play a large role in celebrating the staff and creating this sense of meaning and significance for hospital staff. In a relationship based care model, these gestures serve a huge purpose and allow the hospital to recognize staff in unique ways. The positive factor here is that the nurses have come out of this powerless feeling covid left them with. Small gestures that build trust between nursing and management create a more productive work environment. This is done through clear concise communication, open discussion, and acting on feedback from staff.

Abstract
This study evaluated the correlation of an emergency department embedded care coordinator
with access to community and medical records in decreasing hospital and emergency
department use in patients with behavioral health issues. This retrospective cohort study
presents a 6-month pre-post analysis on patients seen by the care coordinator (n=524). Looking
at all-cause healthcare utilization, care coordination was associated with a significant median
decrease of one emergency department visit per patient (p G 0.001) and a decrease of 9.5 h in
emergency department length of stay per average visit per patient (pG0.001). There was no
significant effect on the number of hospitalizations or hospital length of stay. This intervention
demonstrated a correlation with reducing emergency department use in patients with behavioral
health issues, but no correlation with reducing hospital utilization. This under-researched
approach of integrating medical records at point-of-care could serve as a model for better
emergency department management of behavioral health patients.

Patients with behavioral health issues often require more resource-intensive care and are more
likely to be frequent users of health services.1–7 Brennan et al. found that patients with at least one
primary psychiatric visit to the emergency department (ED) were 4.6 times more likely than those
without a primary psychiatric visit to be classified as high utilizers of health services overall, and
that on average, high utilizers with a primary psychiatric visit had a significantly higher number of
ED visits than non-psychiatric high utilizers.7

Nontraditional nursing roles have emerged due to technological growth, healthcare reform, and demand for preventative and community-based care. Areas such as patient safety, quality improvement, health informatics, behavioral health, and care coordination have become important domains for nursing. The roles of occupational health nurses, case managers, HEDIS nurses, quality assurance nurses, and nurse educators are growing due to a focus on wellness, chronic disease management, and reducing healthcare costs. These nontraditional nursing roles offer salaries comparable to registered nurses and are projected to be in high demand over the next decade, especially in large cities such as Houston, Chicago, and Los Angeles.

Change Proposal: Reimplementation of a Bedside Shift Report
The identified clinical problem that is relevant to nurses in the workplace is the bedside shift report (BSR). The bedside shift report may be used to help the development of teamwork, ownership, and accountability thus increasing the satisfaction of the nurses. The reason for selecting the reimplementation of a bedside shift report is because; at a targeted facility the implementation of the bedside shift report is becoming poor. This has led to the poor compliance with the standards of practice, and increase costs in the facility. Also, to have a reduced cost by removing overtime pay and scrubbing off legal errors which result in legal fees (Clark et al., 2020). (*Re-phrase this sentence, avoid slang).
The reimplementation of a bedside shift report is important for the nurse practicing in the organization. Bedside shift report assists in improving the experience of the client and improving the handoff care between the nurses through including the family and client in the delivery of care. Bedside shift report is leading to the introduction of a change in the nursing culture i.e. introduction of various methods of communicating patient-specific care at a shift transformation. Shift reports given at the bedside is helping in changing the manner in which nurses are communicating and practicing (Clark et al., 2020).
At the targeted facility, the execution of the bedside shift report is poor and this is associated to the lack of knowledge concerning the role of bedside shift reporting on the patient outcome. The healthcare providers at the facility are not directly engaged in the provision of healthcare. There is poor communication of the goals of the bedside shift reporting and their importance about the positive outcomes and benefits that are involved in the change (Clark et al., 2020).
According to the study by Gregory et al., (2014), the process of assessing the transformation in practice that involves transitioning of the bedside nurse shift handoffs is important. The evaluation process is helpful in the re-implementation plan to help in improving the areas of weaknesses. The evaluation process of the success in the implementation in the practice is assessing the perception of the nurses about the proposed changes i.e. the re-implementation plans. There is evidence showing the positive impacts of the bedside shift report in the safety of the patient, the control of the pain, the satisfaction of the patients, and improvement in the nurse to patient or nurse to nurse communication (Gregory et al., 2014).

NURS 4220 Residents with Alzheimer Disease NURS 4220 Residents with Alzheimer DiseaseNURS 4220 Residents with Alzheimer DiseaseScenario one in this week’s discussion topic is centered around sending many nursing home residents with Alzheimer’s disease, predominantly with Medicare and Medicaid, to the ED and may not need to be seen. With this scenario, both leadership styles can be incorporated. However, the transformational leadership style will be discussed. All team members will need to be involved, and I would share the concerns and review the issues with the nurse manager. The process would begin with an analysis of nurse to patient ratios, as many ED visits can be prevented with better nurse staffing level at the nursing home. Inadequate staffing can lead to limited time to provide proper medical interventions. Therefore, the ED is used to provide medical coverage that is lacking at the nursing home.A staff survey would allow feedback as to why too many patients are being sent to the emergency department, and at the same time, this will encourage staff to present their ideas and give them a sense of empowerment (Yoder-Wise, 2015). Another plan of action would be to petition the ED and encourage an ED utilization review to reduce the unnecessary visits to the ED and learn why the hospital is inquiring about the misuse of the ED visits in question and validate or rescind the problem in question according to data (Spath, 2018).A more precise understanding of the driving factor associated with inappropriate transferring of nursing home residents to the ED may result in better nursing home practice recommendations and allow for the implementation of quality improvement interventions (Perrin et al., 2017). Once the data from the Ed and staff surveys are validated, a protocol/ map of when a patient would need to be transferred would be implemented. Educating staff on all levels would help to identify the protocol for transferring patients to the ED. Nursing staff would first need to evaluate the patient, having responsibility, and ownership. Guidelines would help the nursing staff complete a conclusive assessment of the medical complaint presented. Determining what an emergent is and what medical complaint can be achieved at the nursing home is vital. The nursing home is part of a more extensive system that includes a major medical network that can be utilized as a part of an integrated network. The nursing staff would need to collaborate with other medical providers and determine the needs of the patient.
