This document discusses a proposed mixed methods study to examine challenges in transitional care. The study would involve observing the admission and discharge processes of elderly patients between hospitals, nursing homes, and home care. Both qualitative and quantitative data would be collected through participant observation, open-ended conversations with patients and caregivers, and semi-structured interviews with nursing leaders. The mixed methods approach would allow the qualitative data to provide context and explanations for the quantitative results. The goal is to better understand transitional care challenges and improve quality of care.

Standardized care plans can be described as the pre-determined menu of interventions which are used for different patient situations (Monsen, Swenson & Kerr, 2016). Evidence-based care is the conscientious use of the most recent evidence to make decisions on the care of individual patients or in the delivery of health care services (Murdaugh, Parsons & Pender, 2018). The current best evidence is the most recent information which has been obtained from valid and relevant research about the effects of different types of healthcare, the accuracy of diagnostic tests, the potential for harm from exposure to different agents, or predictive power of prognostic factor (Schmidt & Brown, 2017). Standardized care plans form the main basis for the implementation of evidence-based care directly in practice and for the improvement of patient outcomes (Nussbaum et al., 2015; Yehuda & Hoge, 2016). A health care facility recently transitioned to the use of a new and better electronic health record system. The facility also purchased standardized care plans to increase efficiency in their operations. However, the compliance with using the standardized care plans was only 40 percent among the inpatient nurses. This presents several specific problems in the healthcare facility. There is poor compliance from the nurses concerning the addition of standardized care plans to the charts of patients based on their diagnosis or primary problem(s). The system which the facility invested in was not being used for the improvement of patient outcomes and quality of care delivered. The focus of this project is the failure of inpatient nurses to make use of standardized care plans. The gap in nursing is the failure of delivery of evidence-based practice using the standardized care plans which result in poor patient outcomes and quality of life. One of the areas of knowledge that has not yet been explored is the cause of low rates of adoption of standardized care plans by nurses. Nursing shortage and turnover is a serious problem experienced in the United States of America (USA) and the world at large. Many states in the USA have inadequate nurses which end up impairing the quality of service given to the patients especially in-patients. The few nurses available cannot completely meet the needs of the patients especially those who critically needs total care and attention. The few nurses available are usually stressed and in a hurry trying to care for a high number of patients which is usually impossible. This situations adds stress to them and increase their chances of quitting the job. Nurses play a pivotal role in caring for these patients especially the ones admitted in long term acute care facilities (LTAC) since they continuously monitor the patients in critical conditions and provide necessary treatments and medicine to save the lives of the patients (Stone et al., 2006). There are various reasons which can be associated with the nurses' shortage and turnover and these include inadequate nursing educators, inequitable distribution of nurses, high turnover rate and aging workforce to name but a few. These problems can be solved by investing in the development of nurses to make them happy and well equipped with the knowledge and skills required to handle their tasks. This can be through training them regularly, giving them a chance to further their education as they work, paying them well and giving them appropriate compensations among other key strategies.

The patients in acute long term care units require total attention and monitoring than any other patients requiring basic care. This is to ensure that their conditions are kept in control. In fact, one nurse should only handle three or four patients maximum because of the criticality of these patients. the Problem The case of failure in transitional care for the elderly and the chronically ill
patients shows the challenges in transitional care. Transitional nursing is the continuity of
treatment of the patients during the movement from one health center to another or home.
The incidence rehospitalization of people who have been released from the medical
facilities to go home for recovery is high. According to Ortiz research, in 2015, 35.6% of
hospital readmission within 30days of discharge is of elderly patients of age 75- 84. The
authors indicate that this is due to the patients worsening instead of getting better.
Caregivers report that they do not know how to handle the patients that are left in their
care. Storm and Aase (2014) elaborate that physicians say that elderly patients who have
acute conditions are in vulnerable positions of breakdown if the proper transition is absent.
This is after evaluating that some of the deaths reported of elderly patients in home-based
and community- based care be caused by poor. Ye and Quan (2016) point out that in 2012,
China had the practice of the medical practitioners discontinuing patientâs care once the
patient is discharged. The recovery care task was left with caregivers. This practice led to 30
percent of patient declining in their well-being and regular functioning after been released
from the hospital especially the elderly. In the homebased care, they lacked professional
monitoring. In addition to the issue in the transition of the elderly from the hospital to their
home, the other failure is the transitional care within the medical setting. Ortiz (2019)

specialized nursing is the cause for the challenges in transition care. It directly concerns the
nursing profession. It raises awareness of the weakness of nursing in transitional care. It
identifies that nursing service in transitional care is poor. Patients are sent off without
continuity care from professionals till they fully recover. Moreover, the wrong care is given
to the patients when they are moved within hospitals from one setting to entity as nurses
commonly provide the wrong medication. The problem also informs the nurses their
weakness in communication. It implies that unspecialized nurse is not keen in providing
timely and complete information to facilitate continuity care. It mainly raises concerns in
nurses in units where that transfer takes place. The problem as well illuminates to nurses
that poor professional coordination between them is resulting in transitional care
challenges since it hinders consultation and provision of information. It similarly notifies
the nurses their shortfall in performing specific crucial nursing roles. The first one is
ensuring the patient with no caregiver receives transitional care from them. Secondly, it is

Design Phase 2 Methodology and Design of the Study The study of transitions included the
acute admissions of elderly individuals to the hospitals up to the nursing homes of from the
home to a home with a home-based care services. Additionally, it included the discharge
transitions of elderly from hospitals to a nursing home or home also to home-based care
services. Therefore, the methodology included participant observation of admission and
discharge transition process. This involved an open-ended conversation with the patients
and also with the patientâs next of kinâs while involving the health care personnel (Design
and Methodology of a Mixed Methods Follow-up Study to the 2014 Ghana Demographic and
Health Survey, 2017). The observation also entails a collection of data which is guided by a
thematic guide which is adapted to the admission and discharge transition in the health

what the interviews should cover and how it should be carried out. It starts with the
general, discussion of transition processes and the patients experienced pertaining to
transitions received at homecare. The participants in this regards will focus on all the
experiences on the transition from the hospital to their homes, and DESIGN PHASE 2 4 then
researchers will decide on what next (Johnson, Pennell, Stoop, & Dorer, 2018). The guide for
