- Introduce self – CQI Coordinator at AHCSA Acknowledgement of Country Thanks to NACCHO for providing the opportunity to present to you today about an exciting and important program that AHCSA have developed and implemented to our SA ACCHS -- the South Australian Quality Improvement Data Cycles (commonly known a SQID)
- So… why SQID? AHCSAs role – support, facilitate and foster quality improvement for SA ACCHSs Build on existing quality improvement programs within AHCSA looking at Back in 2014, AHCSA had developed a program called SQID – The SQID Program allowed Member ACCHSs to share secure access to this de-identified nKPI data with AHCSA. The main objectives of the SQID Program are to develop a tool that will compliment current CQI processes being undertaken within our Member Services, as well as to aid in promoting new quality improvement initiatives by; •Providing SQID reports to participating ACCHSs with an overview of their quarterly nKPI numerator and denominator totals, trend lined with previous calendar year quarter totals. •Benchmarking participating ACCHSs with the AHCSA percentage average for each nKPI in order to ascertain how each Member Service is fairing in comparison to the greater SA Community Controlled sector. •Analysing and presenting nKPI data for Member Services that may not have the capacity to undertake these tasks within their respective organisations. •The development and growth of the nKPI evidence base for the SA Community Controlled health sector, which may inform a range of future activities. Because of this program’s successfulness and with a helping hand from our sister organisation, VACCHO, we built on this program to add an additional quality improvement layer which makes up the new program “SQID Cycles”
- The AHCSA SQID cycles is program that was developed based off the VACCHO’s Vic Cycles SQID Cycles is a state-wide quality improvement project focused around a particular topic. The SQID Cycles work directly with participating Member Services in the collection of de-identified baseline health data, followed by webinar a presentation that aims to explore the underpinning processes, procedures, data entry methods etc. associated with the cycle topic. De-identified health data will be collected again after three months and compared with the baseline data to assess health service improvement. It is a three-monthly, interactive quality improvement program that ACCHOs can participate in to improve their systems and processes in a particular area.. It is important to note that, nationally, there are a miriade of data driven quality improvement acitivites that cover a range of various indicators – however, with the SQID Cycles, the aim is to focus on one specific area of health per cycle AND for that ONE specific area to be driven from a grass roots level. VIC cycles As depicted in the picture on the screen, the SQID Cycles works directly with participating Member Services in the collection of de-identified baseline health data, followed by webinar presentations that will aim to explore the underpinning processes, procedures, data entry methods associated with the cycle topic. De- identified health data will be collected again after three months and compared with the baseline data to assess health service improvement Its important to point out that at the center of this program is the AHCSA SQID Cycles - ACCHO Data Working Group. The role of this working group is to: Discuss SQID Cycle topic selection prior to the beginning of each new cycle Provide ongoing evaluation and feedback of content delivery, resources, and level of AHCSA support Support knowledge sharing and ideas for quality improvement
- As the content of the webinar series was being developed and with the “Data Custodianship Agreements” and the “ Data Governance Protocols in place; we worked with participating H/S to collect baseline HbA1c Data to develop dashboards – like the ones you can see on the screen. The data collected and presented was based on the type 2 diabetes current client population residing within the health service area. And we collected this data using either a secure remote access support or via phone with health service staff to run the Communicare reports, which was then securely shared with AHCSA using Own Cloud The HbA1c Dashboards included the following indicators; Proportion of type 2 diabetes clients with an HbA1c test recorded within the previous 6 and 12 months (by sex) Proportion of HbA1c tests recorded by age group within the previous 6 and 12 months Number of clients with an HbA1c test within 6 months, 6-12 months and with no HbA1c recorded SQID ACCHS benchmark for HbA1c tests recorded within the previous 12 months These dashboards were disseminated to participating H/S prior to the first webinar and H/S were encourages to use these dashboards in staff meetings as a conversation starter: Talk about data Talk about what their data looks like and why Discuss possible improvements
- In total, we delivered 4 webinars – 3 of those were over a 6 week period
- HbA1c - an indicator of diabetes management Target for most people is 7% or less SA ACCHS Sector increased HbA1c testing by 5.8% 5 of 7 of ACCHSs participating in SQID cycle 1 improved their overall testing rates Biggest improvement; 18.7%
