The accreditation is designed to serve as a platform for continuous quality improvement as a department must continue to improve even after 5 year accreditation status has been achieved. With the current economic climate and limited funds and resources, accreditation and quality improvement can make current departments functions more efficient while maintaining the integrity of the work.
Winter of 2013, the PHAB board approved a modified logic model
Within the required documentation, there are several major plans and systems a PH department must possess.
Measures, standards, and domains are related. Don’t get caught in the weeds of a measure. Step back to see if your documentation for the measure is demonstrating the standard AND is performing the service of the domain.
Have a way for “clean eyes” to view your documentation.
