This document outlines the strategic plan and action plans for Haliburton Highlands Health Services (HHHS) for 2014-2017. The vision is to be leaders in innovative rural health care. The mission is to promote wellness and provide essential, high-quality health services including primary care, hospital care, long-term care, end-of-life care, mental health and addiction services, and community support services. The strategic directions include integrating health systems, improving care coordination, engaging the community, improving employee engagement, identifying service requirements, improving access to services, quality improvement, ensuring sustainability, and developing long-term action plans.
