 MUS Wellness envisions our benefit plan members engaged in the
ownership and betterment of their health and well-being in order to
maximize quality of life.
 Our Mission: to help our plan members achieve health, by providing
and incentivizing preventive health screenings, healthy lifestyle
education and support, and disease prevention and management
programs

Purposes of Online Incentive “Platform”
 Continue to raise employee awareness of health status and health risk.
 House all employee health data and resources in one place.
 Link employee to appropriate resources/education that fit individual goals
and needs.
 Reward employees for healthy behaviors.
Confidentiality
 Participation in the MUS Wellness Incentive Program is completely confidential
and secure. MUS is committed to protecting individual personal health
information. Limeade is bound by legal and contractual obligations to ensure
the confidentiality of information provided through the program, and does
not rent, share, or sell participants information. MUS will receive
summarized, aggregate reports that show the overall health of all participants
in the program.

 Website goes live Tuesday, July 15th.
 Incentive program will run on the calendar year, and incentivize the following
fiscal year.
 For example, incentives earned by December 31st, 2014, will effect Employee
Benefits in FY 2015.
 Currently, only MUS Employees on Benefit Plan are eligible for online Login
through Limeade. However, one plan member age 18 or older enrolled in a
MUS medical plan is also eligible for a $250 incentive account contribution
with Wellcheck participation.
 All MUS Wellness Programs are still, as always, available and free to spouses and
adult dependents.
