Functional electrical stimulation (FES) applied via transcutaneous
electrodes is a common rehabilitation technique for assisting grasp

in patients with central nervous system injury.


It could be considered a bypass of a lost or damaged
communication channel between the peripheral and central
nervous system (CNS).



The bypass is provided by a stimulator, which sends bursts of
electrical charge pulses that replace the missing drives to the
muscles or sensory information to the CNS.

Intelligent Functional Electrical Stimulation (INTFES)multiplexes
charge pulses from a single pulse train to different conductive
pads using a fast switching system that is synchronized with
stimulation pulses.



There are seven total leads :

o

2 for I2C protocol

o

2 for power supply

o

sensory output

o

stimulation and enable

The multi-pad electrode has an arbitrary number
and shape of pads.

The INTFES multi-pad electrode described here is custom-made
for the activation of the forearm muscles responsible for
grasping.The stimulation electrode structure comprises four layers:

o

a polyester substrate,

o

an Ag/AgCl electrode matrix,

o

an insulation coating,

o

and a conductive hydrogel.
The stimulation electrode is attached to the skin with adhesive
conductive gel .

The stimulator output stage is a single high-voltage stage
with a controlled current. All of the parameters can be programmed
and monitored using a program supplied with the INTFES or with a
custom-made program running on a PC.



Communication with the PC is realized via a Bluetooth
interface. In addition, the stimulus amplitude can be adjusted by
directly using the touch keys on the device.

Stimulator Technical specifications
Microcontroller: 8-bit ATmega1284P
operating at 11 MHz

128 k bytes of In-System Self-Programmable Flash memory

 two wire interface (I2C) modules capable of a data transfer speed of up to
400 kHz
two 16-bit timers and two 8-bit timers
 an SPI interface and

two USARTs. The microcontroller controls all peripherals, including the multipad electrodes, and it acts as a slave device to a host controller (PC,
Smartphone, etc.).

a monitoring chip for battery capacity level.

The output stage limited to 94 V
generated by a step-up DC/DC converter MAX773 .
Analog inputs and multi-pad electrode control: For safety
reasons, analog inputs and multi-pad electrode control logic are

isolated from high voltage circuitry with a digital circuit and a DC/DC
power supply.

Communications: A Bluetooth module maintains communication
between the INTFES device and the host controller.Using this the host can
set all of the stimulation parameters,
 initiate a stimulation protocol,
 activate the pads and
read analog signals and locally calculated parameters.

Control software
 The control software is an application running on a host controller.
 The communication protocol is supported by several programming
languages (C, C#, JAVA, LabView, MATLAB).
 Stimulation can be initiated or terminated promptly using control
software, and it has the highest priority in the INTFES system.

Feedback sensors.
The INTFES stimulator comprises six analog inputs that can be used for
online feedback control of the stimulation parameters.

Preferred electrode selection:The first step in any application of the
INTFES device is tuning the active surface (selecting the number and
position of active pads and setting the current intensities in each active
pad). A look-up table is provided that provides information about pad
number and orientation and current intensities for desired finger

movements.

Important advantages of the new multi-pad electrode for daily use
are:
1)applicability in telerehabilitation, where the therapist can
remotely select active pads with no involvement of the patient.

o

2) delay of muscle fatigue during FES.

o

3) the electrode can be placed randomly and then choose which
pads to activate in order to acquire the best muscle response.This

allows a nonexpert to operate FES effectively, especially in a home
environment.

The INTFES system is operational and is currently being used in
clinical studies at the Dr. Miroslav Zotović Institute for
Rehabilitation, Belgrade, Serbia. The same system can be applied
for other therapeutic modalities like:



1) foot drop correction



2) lower back pain



3) and fitness.

INTFES allows wrist stabilization and selective activation of



muscles that are required for hand opening and closing.
Importantly, the multi-pad electrode application enables



asynchronous operation .


Once the electrode size and shape are determined with respect
to the anatomical features of the forearm, the INTFES system can
be set up on the patient in less than 2 minutes by an
inexperienced user.

Multi pad electrode for effective grasping

  • 3.  Functional electrical stimulation (FES) applied via transcutaneous electrodes is a common rehabilitation technique for assisting grasp in patients with central nervous system injury.  It could be considered a bypass of a lost or damaged communication channel between the peripheral and central nervous system (CNS).  The bypass is provided by a stimulator, which sends bursts of electrical charge pulses that replace the missing drives to the muscles or sensory information to the CNS. 3
  • 4.  Multipad electrode Intelligent Functional Electrical Stimulation (INTFES)multiplexes charge pulses from a single pulse train to different conductive pads using a fast switching system that is synchronized with stimulation pulses.  There are seven total leads : o 2 for I2C protocol o 2 for power supply o sensory output o stimulation and enable 4

  • 6.  The INTFES multi-pad electrode described here is custom-made for the activation of the forearm muscles responsible for grasping.The stimulation electrode structure comprises four layers: o a polyester substrate, o an Ag/AgCl electrode matrix, o an insulation coating, o and a conductive hydrogel. The stimulation electrode is attached to the skin with adhesive conductive gel . 6

  • 8.  The stimulator output stage is a single high-voltage stage with a controlled current. All of the parameters can be programmed and monitored using a program supplied with the INTFES or with a custom-made program running on a PC.  Communication with the PC is realized via a Bluetooth interface. In addition, the stimulus amplitude can be adjusted by directly using the touch keys on the device. 8

Stimulator Technical specifications Microcontroller: 8-bit ATmega1284P operating at 11 MHz 128 k bytes of In-System Self-Programmable Flash memory  two wire interface (I2C) modules capable of a data transfer speed of up to 400 kHz two 16-bit timers and two 8-bit timers  an SPI interface and two USARTs. The microcontroller controls all peripherals, including the multipad electrodes, and it acts as a slave device to a host controller (PC, Smartphone, etc.).  power supply : Li-ion rechargeable 7.4V, 750 mAh battery a monitoring chip for battery capacity level. 11
  • 12. The output stage (current control): The output stage limited to 94 V generated by a step-up DC/DC converter MAX773 . Analog inputs and multi-pad electrode control: For safety reasons, analog inputs and multi-pad electrode control logic are isolated from high voltage circuitry with a digital circuit and a DC/DC power supply. Communications: A Bluetooth module maintains communication between the INTFES device and the host controller.Using this the host can set all of the stimulation parameters,  initiate a stimulation protocol,  activate the pads and read analog signals and locally calculated parameters. 12

Control software  The control software is an application running on a host controller.  The communication protocol is supported by several programming languages (C, C#, JAVA, LabView, MATLAB).  Stimulation can be initiated or terminated promptly using control software, and it has the highest priority in the INTFES system. Feedback sensors. The INTFES stimulator comprises six analog inputs that can be used for online feedback control of the stimulation parameters. 13
  • 14. Preferred electrode selection:The first step in any application of the INTFES device is tuning the active surface (selecting the number and position of active pads and setting the current intensities in each active pad). A look-up table is provided that provides information about pad number and orientation and current intensities for desired finger movements. 14
  • 15.  o Important advantages of the new multi-pad electrode for daily use are: 1)applicability in telerehabilitation, where the therapist can remotely select active pads with no involvement of the patient. o 2) delay of muscle fatigue during FES. o 3) the electrode can be placed randomly and then choose which pads to activate in order to acquire the best muscle response.This allows a nonexpert to operate FES effectively, especially in a home environment. 15
  • 16.  The INTFES system is operational and is currently being used in clinical studies at the Dr. Miroslav Zotović Institute for Rehabilitation, Belgrade, Serbia. The same system can be applied for other therapeutic modalities like:  1) foot drop correction  2) lower back pain  3) and fitness. 16
  • 17. INTFES allows wrist stabilization and selective activation of  muscles that are required for hand opening and closing. Importantly, the multi-pad electrode application enables  asynchronous operation .  Once the electrode size and shape are determined with respect to the anatomical features of the forearm, the INTFES system can be set up on the patient in less than 2 minutes by an inexperienced user. . 17
