This document outlines a proposed mentorship program called "Vision for Boys to Men" aimed at reducing discipline issues and improving academic performance among 50 male students at a school. It details current student data showing high referral and low reading rates. The program goals are to cut referrals 20%, increase reading scores, and lower absences. Mentors are asked to commit one day weekly and quarterly off-campus rewards. Activities include violence prevention and social skills programs, team sports, service projects and team building. Resources, training and regular evaluation of discipline, test scores and attendance data are discussed. Feedback from potential mentors is requested within seven days including number of volunteers and preferred attendance days.
