3. Tahap Pemilihan Obat
Fungsi dari pemilihan atau penyeleksian obat adalah untuk
menentukan apakah obat benar-benar diperlukan dan sesuai
dengan jumlah penduduk serta pola penyakit. Untuk
mendapatkan pengadaan obat yang baik, sebaiknya diawali
dengan dasar-dasar seleksi kebutuhan obat yaitu meliputi
a) Obat merupakan kebutuhan untuk sebagian besar populasi
penyakit;
b) Obat memiliki keamanan, kemanjuran yang didukung
dengan bukti ilmiah;
c) Obat memiliki manfaat yang maksimal dengan risiko yang
minimal;
d) Obat mempunyai mutu yang terjamin baik ditinjau dari segi
stabilitas maupun bioavaibilitasnya;
e) Biaya pengobatan mempunyai rasio antara manfaat
dengan biaya yang baik;
f) Apabila pilihan lebih dari satu, maka dipilih yang paling
baik, banyak diketahui dan farmakokinetiknya yang paling
menguntungkan;
g) Mudah diperoleh dengan harga terjangkau;
h) Obat sedapat mungkin merupakan sediaan tunggal.

4. Pada tahap seleksi obat harus pula dipertimbangkan antara lain seperti ;
dampak administratif, biaya yang ditimbulkan, kemudahan dalam
mendapatkan obat, kemudahan obat dalam penyimpanan, kemudahan
obat untuk di distribusikan, dosis obat sesuai dengan kebutuhan terapi,
obat yang dipilih sesuai dengan standar mutu yang terjamin. Sedangkan
untuk menghindari risiko yang dapat terjadi harus pula
mempertimbangkan kontra indikasi, peringatan dan perhatian juga efek
samping obat.

5. Tahap Kompilasi Pemakaian Obat
Kompilasi pemakaian obat berfungsi untuk mengetahui pemakaian
bulanan tiap-tiap jenis obat selama setahun dan sebagai data
pembanding bagi stok optimum.
Informasi yang didapatkan dari kompilasi pemakaian obat adalah :
• Jumlah pemakaian tiap jenis obat pada tiap Unit Pelayanan Kesehatan
di Puskesmas
• Persentase pemakaian tiap jenis obat terhadap total pemakaian
setahun seluruh Unit Pelayanan Kesehatan.
• Pemakaian rata-rata untuk setiap jenis obat di Puskesmas

6. METODE
PERHITUNGAN
KEBUTUHAN
01 Metode Konsumsi
Metode konsumsi didasarkan pada data konsumsi sediaan farmasi. Metode ini sering dijadikan
perkiraan yang paling tepat dalam perencanaan sediaan farmasi. Rumah Sakit yang sudah mapan
biasanya menggunakan metode konsumsi. Metode konsumsi menggunakan data dari konsumsi periode
sebelumnya dengan penyesuaian yang dibutuhkan. Perhitungan dengan metode konsumsi didasarkan
atas analisa data konsumsi sediaan farmasi periode sebelumnya ditambah stok penyangga (buffer
stock), stok waktu tunggu (lead time) dan memperhatikan sisa stok. Buffer stock dapat
mempertimbangkan kemungkinan perubahan pola penyakit dan kenaikan jumlah kunjungan (misal:
adanya Kejadian Luar Biasa). Jumlah buffer stock bervariasi antara 10% sampai 20% dari kebutuhan
atau tergantung kebijakan Rumah Sakit. Sedangkan stok lead time adalah stok Obat yang dibutuhkan
selama waktu tunggu sejak Obat dipesan sampai Obat diterima.
Untuk menghitung jumlah obat yang dibutuhkan berdasarkan metode konsumsi, perlu diperhatikan hal-
hal sebagai berikut:
a) Pengumpulan dan pengolahan data
b) Analisis data untuk informasi dan evaluasi
c) Perhitungan perkiraan kebutuhan obat
d) Penyesuaian jumlah kebutuhan Sediaan Farmasi dengan alokasi dana

7. LANJUTAN
Data yang perlu dipersiapkan untuk
perhitungan metode konsumsi adalah:
• Daftar nama obat
• Stok awal
• Penerimaan
• Pengeluaran
• Sisa stok
• Daftar obat hilang, rusak,
kedaluwarsa
• Kekosongan obat
• Pemakaian rata-rata obat satu
periode
• Waktu tunggu sejak obat dipesan
sampai diterima (lead time)
• Stok pengaman (buffer stock)
• Pola kunjungan
Rumus : A= (B+C+D) - E
A = Rencana Kebutuhan
B = Stok Kerja (Pemakaian rata-rata x 12 bulan)
C = Buffer stock
D = Lead Time Stock (Lead time x pemakaian rata-rata)
E = Sisa stok
A= (B+C+D) - E
Keterangan :
• Stok Kerja adalah kebutuhan obat untuk pelayanan kefarmasian
selama satu periode.
• Buffer stock adalah stok pengaman
• Lead time stock adalah lamanya waktu antara pemesanan obat
sampai dengan obat diterima.
• Lead stock adalah jumlah obat yang dibutuhkan selama waktu
tunggu (lead time)

02 Metode Morbiditas
Metode morbiditas adalah perhitungan kebutuhan obat berdasarkan pola penyakit.
Metode morbiditas memperkirakan keperluan obat–obat tertentu berdasarkan dari
jumlah obat, dan kejadian penyakit umum, dan mempertimbangkan pola standar
pengobatan untuk penyakit tertentu. Metode ini umumnya dilakukan pada program yang
dinaikkan skalanya (scaling up). Metode ini merupakan metode yang paling rumit dan
memakan waktu yang lama. Hal ini disebabkan karena sulitnya pengumpulan data
morbiditas yang valid terhadap rangkaian penyakit tertentu.
Langkah-langkah dalam perhitungan kebutuhan dengan metode morbiditas:
a. Mengumpulkan data yang diperlukan
Data yang perlu dipersiapkan untuk perhitungan metode morbiditas adalah:
1).Perkiraan jumlah populasi
Komposisi demografi dari populasi yang akan diklasifikasikan berdasarkan jenis kelamin
untuk umur antara:
• 0 s.d. 4 tahun
• 4 s.d. 14 tahun
• 15 s.d. 44 tahun
• >45 tahun
• Atau ditetapkan berdasarkan kelompok dewasa (>12 tahun) dan
anak (1 – 12 tahun)

9. 3). Standar pengobatan
Obat yang masuk dalam rencana
kebutuhan harus disesuaikan dengan
standar pengobatan di rumah sakit.
2). Pola morbiditas penyakit
• Jenis penyakit pertahun untuk seluruh
populasi pada kelompok umur yang ada.
• Frekuensi kejadian masing-masing
penyakit pertahun untuk seluruh
populasi pada kelompok umur yang ada.

10. b. Menghitung kebutuhan jumlah obat, dengan cara
jumlah kasus dikali jumlah obat sesuai pedoman
pengobatan dasar. Jumlah kebutuhan obat yang akan
datang dihitung dengan mempertimbangkan faktor antara
lain pola penyakit, lead time dan buffer stock.

03 Metode Proxy Consumption
Metode proxy consumption dapat digunakan untuk perencanaan
pengadaan di Rumah Sakit baru yang tidak memiliki data konsumsi di
tahun sebelumnya. Selain itu, metode ini juga dapat digunakan di
Rumah Sakit yang sudah berdiri lama apabila data metode konsumsi
dan/atau metode morbiditas tidak dapat dipercaya. Sebagai contoh
terdapat ketidaklengkapan data konsumsi diantara bulan Januari hingga
Desember. Metode proxy consumption adalah metode perhitungan
kebutuhan obat menggunakan data kejadian penyakit, konsumsi obat,
permintaan, atau penggunaan, dan/atau pengeluaran obat dari Rumah
Sakit yang telah memiliki sistem pengelolaan obat dan
mengekstrapolasikan konsumsi atau tingkat kebutuhan berdasarkan
cakupan populasi atau tingkat layanan yang diberikan. Metode ini dapat
digunakan untuk menghasilkan gambaran ketika digunakan pada
fasilitas tertentu dengan fasilitas lain yang memiliki kemiripan profil
masyarakat dan jenis pelayanan. Metode ini juga bermanfaat untuk
gambaran pengecekan silang dengan metode yang lain.

13. Analisis ABC
ABC bukanlah singkatan, melainkan suatu
penamaan yang menunjukkan
peringkat/rangking dimana urutan dimulai
dengan yang terbaik/terbanyak. Analisis ABC
mengelompokkan item obat berdasarkan
kebutuhan dananya, yaitu:
a. Kelompok A:
Adalah kelompok jenis obat dengan jumlah nilai
rencana pengadaannya menunjukkan
penyerapan dana sekitar 70% dari jumlah dana
obat keseluruhan.
b. Kelompok B:
Adalah kelompok jenis obat dengan jumlah nilai
rencana pengadaannya menunjukkan
penyerapan dana sekitar 20%.
c. Kelompok C:
Adalah kelompok jenis obat dengan jumlah nilai
rencana pengadaannya menunjukkan
penyerapan dana sekitar 10% dari jumlah dana
obat keseluruhan..

14. Analisis ABC dapat dibedakan menjadi 2 (dua), yaitu :
a. Analisis ABC
Pemakaian Langkah-langkah yang harus dilakukan, yaitu sebagai berikut :
1. Mengumpulkan daftar jenis obat dalam satu periode.
2. Membuat daftar pemakaian dari masing-masing jenis obat.
3. Jumlah pemakaian masing-masing jenis obat diurutkan berdasarkan
jumlah pemakaian terbanyak ke jumlah pemakaian yang terkecil.
4. Menghitung persentase untuk masing-masing dan persentase
kumulatifnya.
5. Mengelompokkan obat menjadi 3 kelompok berdasarkan persentase
70-20- 10, yaitu :
• 70 % masuk kelompok A
• 20 % masuk kelompok B
• l0 % masuk kelompok C

15. Analisis VEN
Salah satu cara untuk meningkatkan
efisiensi penggunaan dana obat yang
terbatas adalah dengan
mengelompokkan obat yang didasarkan
kepada dampak tiap jenis obat pada
kesehatan. Semua jenis obat yang
tercantum dalam daftar obat
dikelompokkan kedalam 3 kelompok
berikut :

Kelompok V :
Adalah kelompok obat-obatan
yang harus tersedia karena
dipakai untuk tindakan
penyelamatan hidup manusia,
atau untuk pengobatan
penyakit yang menyebabkan
kematian. Obat yang termasuk
dalam kelompok ini antara lain,
life saving drugs, obat untuk
pelayanan kesehatan dasar,
dan obat untuk mengatasi
penyakit-penyakit penyebab
kematian terbesar.
Kelompok E :
Adalah kelompok obat-
obatan esensial yang
banyak digunakan
dalam
tindakan atau dipakai
diseluruh unit di Rumah
Sakit, biasanya
merupakan obat yang
bekerja secara kausal
atau obat yang bekerja
pada sumber penyakit.
Kelompok N :
Merupakan obat-obatan
penunjang atau
pelengkap yaitu obat
yang kerjanya ringan
dan biasa dipergunakan
untuk menimbulkan
kenyamanan atau untuk
mengatasi keluhan
ringan.

17. Penggolongan obat dengan analisis VEN dapat
digunakan :
1. Penyesuaian rencana kebutuhan obat dengan alokasi
dana yang tersedia. Obat-obatan yang perlu ditambah
atau dikurangi dapat didasarkan atas pengelompokkan
obat menurut VEN.
2. 2. Dalam penyusunan rencana kebutuhan obat yang
masuk kelompok V agar diusahakan tidak terjadi
kekosongan obat. Untuk menyusun daftar VEN perlu
ditentukan lebih dahulu kriteria penentuan VEN.
Kriteria sebaiknya disusun oleh suatu Tim. Dalam
menentukan kriteria perlu dipertimbangkan kondisi dan
kebutuhan masing-masing wilayah. Kriteria yang
disusun dapat mencakup berbagai aspek antara lain;
klinis, konsumsi, target kondisi dan biaya (Depkes,
2002).

18. Jenis obat yang termasuk kategori
A dari analisis ABC adalah benar-
benar jenis obat yang diperlukan
untuk penanggulangan penyakit
terbanyak. Dengan kata lain,
statusnya harus E dan sebagian V
dari VEN. Sebaliknya, jenis obat
dengan status N harusnya masuk
kategori C.

Digunakan untuk menetapkan prioritas untuk pengadaan obat dimana anggaran
yang ada tidak sesuai dengan kebutuhan
Metoda gabungan ini digunakan untuk melakukan pengurangan obat. Mekanismenya adalah :
• Obat yang masuk kategori NC menjadi prioritas pertama untuk dikurangi atau dihilangkan dari
rencana kebutuhan, bila dana masih kurang, maka obat kategori NB menjadi prioritas
selanjutnya dan obat yang masuk kategori NA menjadi prioritas berikutnya. Jika setelah
dilakukan dengan pendekatan ini dana yang tersedia masih juga kurang lakukan langkah
selanjutnya.
• Pendekatannya sama dengan pada saat pengurangan obat pada kriteria NC, NB, NA dimulai
dengan pengurangan obat kategori EC, EB dan EA
