LOCK OUT/TAG OUT
Roles and Responsibilities
 Approving Authority – Site Supervisor
 Issuing Authority – PTW Coordinator
 Performing Authority – Foreman or Supervisor
 Electrical Isolation Authority
 Mechanical Isolation Authority
The Approving Authority must visit the location where the task is
to take place prior to approving and signing the permit
PROCEDURE
The procedure specifies methods of controlling
hazardous energy sources during construction and
maintenance activities on:
 Electrical services
 Facilities
 Shop equipment
 Pressurized pipe and
 Systems used as
service lines for
construction
 Electrical
 Generated
 Static
 Mechanical
 Transitional
 Rotational
 Thermal
 Machines or Equipment
 Chemical Reactions
 Potential Pressure
 Hydraulic
 Pneumatic
 Vacuum
Procedure (cont’d)
The equipment to be worked is isolated from all
sources of energy:
PROCEDURE (CONT’D)
 A standard “DANGER – DO NOT OPERATE” tag and
individually keyed locks will be used on the Project
 All tags shall be dated, signed and a description of the work
being performed shown on the tag and securely attached to the
equipment / lock
 Tags must never be re-used,
but destroyed immediately
upon removal.
No alterations to a tag are permitted!

PROCEDURE (CONT’D)
Tags required beyond one shift shall be replaced by
the oncoming shift.
In no case will locks and tags be permitted to remain
more than 7 days without re-inspection, signed and

All sources of energy such as
 pipelines
 valves
 pumps
shall be blanked off and otherwise secured
to prevent charging, energizing or creating
any type of hazards to persons working on
systems or inside a confined space
ELECTRICAL OPERATED SYSTEMS
When work on electrical equipment or services is
scheduled, the power source shall be de-energized using
one or more of the following means:
Main Isolator
Remove Fuses
Local Isolator
ELECTRICAL OPERATED SYSTEMS
(CONT’D)
To lock out electrically powered shop equipment to
ensure maximum employee safety:
A power disconnect switch shall be installed between the
main power supply panel and each piece of electrically driven
equipment which directly connected to the power panel box
The disconnect switch should be located close to the
equipment and properly identified
Pushbuttons or butterfly
controls of equipment should
not be locked out
ELECTRICAL OPERATED SYSTEMS

Completed Danger Tag(s) and locks shall be attached to the
controlling device by:
 Person responsible for de-energizing the system
 The foreman in charge of the work on the system
 All craft workers required to work on the system
Where exposed electrical circuits are to be worked on, the de-
energized equipment or systems shall be tested using
a voltmeter on the down stream
side of the tagged our
controlling device to ensure
there is an effective isolation
of the circuit

 The lockout log shall list all personnel applying a lock and
tag on any power panel; distribution panel or equipment-
controlling device.
 Each person shall detail in the lockout log the time of day he
applied the lock, the necessity for the lockout, the area or
machine where the work is being performed and
the approximate length of time
that the power panel, distribution
panel or equipment controller will
be de-energized

 When the work is completed, the employees will report this
information to the Electrical Superintendent before removing
his lock. The employees will log the time the work was
completed in the lockout log and the time he removed his
lock
 Whenever maintenance of shop equipment is required, such
as changing the blade of a radial arm saw, the craft operator
shall verify that the controlling device (power disconnect) has
been tagged and locked out before maintenance work begins
OPERATING FACILITIES AND
EQUIPMENT
 Electrical Isolation Authority or his designee de-
energizes the system, checks it and places his lock
and tag on the lockout device
 Other employees working on the system places
their locks and tags on the device and complete the
lockout log
 Upon completion of the work, the Electrical
Isolation Authority or his designee is the last
person to remove his lock
and tag
MECHANICAL WORK
Whenever work is scheduled on mechanical
equipment or pressurized systems controlling
devices such as:
 circuit breakers
 switches
 valve handles
 and other operating mechanisms
shall be locked and tagged out to prevent their
manipulation or operation
MECHANICAL WORK (CONT’D)
 Where possible the system shall be disconnected
from all sources of pressure or hazardous
substance
 If appropriate on tanks and pressure vessels, the
system shall be emptied, depressurized and
blanked off before work
starts
 Appropriate gas tests
shall be conducted
TYPES OF LOCKOUT DEVICES
 Plug Locks
 Ball Valve Lockout
 Gate Valve Lockout
 Group Lockout Hasp
 Electrical
 Hydraulic, pneumatic,
and other pressurized
systems
EMERGENCY REMOVAL OF
EMPLOYEES LOCKOUT LOCK
 Each lock-out / tag-out device shall be removed from each
energy-isolating device by the employee who applied the
device
 The supervisor in charge of the work and the Electrical
Isolation Authority shall
complete an “emergency lock-out
Device Removal” form and
present it to the Construction
Manager and the senior
Safety Officer for an
approval to remove the lock

PROCEDURE (CONT’D)  A standard “DANGER – DO NOT OPERATE” tag and individually keyed locks will be used on the Project  All tags shall be dated, signed and a description of the work being performed shown on the tag and securely attached to the equipment / lock  Tags must never be re-used, but destroyed immediately upon removal. No alterations to a tag are permitted! DO NOT OPERATE
  • 6. PROCEDURE (CONT’D) Tags required beyond one shift shall be replaced by the oncoming shift. In no case will locks and tags be permitted to remain more than 7 days without re-inspection, signed and Dated
  • 7. ISOLATION CONFORMATION CERTIFICATE (ICC) All sources of energy such as  pipelines  valves  pumps shall be blanked off and otherwise secured to prevent charging, energizing or creating any type of hazards to persons working on systems or inside a confined space
  • 8. ELECTRICAL OPERATED SYSTEMS When work on electrical equipment or services is scheduled, the power source shall be de-energized using one or more of the following means: Main Isolator Remove Fuses Local Isolator

ELECTRICAL OPERATED SYSTEMS (CONT’D) To lock out electrically powered shop equipment to ensure maximum employee safety: A power disconnect switch shall be installed between the main power supply panel and each piece of electrically driven equipment which directly connected to the power panel box The disconnect switch should be located close to the equipment and properly identified Pushbuttons or butterfly controls of equipment should not be locked out

ELECTRICAL OPERATED SYSTEMS (CONT’D) Completed Danger Tag(s) and locks shall be attached to the controlling device by:  Person responsible for de-energizing the system  The foreman in charge of the work on the system  All craft workers required to work on the system Where exposed electrical circuits are to be worked on, the de- energized equipment or systems shall be tested using a voltmeter on the down stream side of the tagged our controlling device to ensure there is an effective isolation of the circuit

ELECTRICAL OPERATED SYSTEMS (CONT’D)  The lockout log shall list all personnel applying a lock and tag on any power panel; distribution panel or equipment- controlling device.  Each person shall detail in the lockout log the time of day he applied the lock, the necessity for the lockout, the area or machine where the work is being performed and the approximate length of time that the power panel, distribution panel or equipment controller will be de-energized
  • 12. ELECTRICAL OPERATED SYSTEMS (CONT’D)  When the work is completed, the employees will report this information to the Electrical Superintendent before removing his lock. The employees will log the time the work was completed in the lockout log and the time he removed his lock  Whenever maintenance of shop equipment is required, such as changing the blade of a radial arm saw, the craft operator shall verify that the controlling device (power disconnect) has been tagged and locked out before maintenance work begins

OPERATING FACILITIES AND EQUIPMENT  Electrical Isolation Authority or his designee de- energizes the system, checks it and places his lock and tag on the lockout device  Other employees working on the system places their locks and tags on the device and complete the lockout log  Upon completion of the work, the Electrical Isolation Authority or his designee is the last person to remove his lock and tag
  • 14. MECHANICAL WORK Whenever work is scheduled on mechanical equipment or pressurized systems controlling devices such as:  circuit breakers  switches  valve handles  and other operating mechanisms shall be locked and tagged out to prevent their manipulation or operation

MECHANICAL WORK (CONT’D)  Where possible the system shall be disconnected from all sources of pressure or hazardous substance  If appropriate on tanks and pressure vessels, the system shall be emptied, depressurized and blanked off before work starts  Appropriate gas tests shall be conducted
  • 16. TYPES OF LOCKOUT DEVICES  Plug Locks  Ball Valve Lockout  Gate Valve Lockout  Group Lockout Hasp  Electrical  Hydraulic, pneumatic, and other pressurized systems

EMERGENCY REMOVAL OF EMPLOYEES LOCKOUT LOCK  Each lock-out / tag-out device shall be removed from each energy-isolating device by the employee who applied the device  The supervisor in charge of the work and the Electrical Isolation Authority shall complete an “emergency lock-out Device Removal” form and present it to the Construction Manager and the senior Safety Officer for an approval to remove the lock
