This Vendor Consent/Invoice can either be signed and brought with you the date of performance or
it can be signed and emailed to Accounts Payable (ap@cedarsinaipark.org) for payment to be mailed
to you after the date of your performance (i.e., when services are rendered). Your payment will be
processed and completed within 30 days. We also need you to send with your form or bring with
you a completed W-9 form. Annually, we are required to file an information return with the IRS
disclosing all reportable payments made to each of our vendors during the year. Form W-9 is the
prescribed form for requesting a taxpayer identification number. This will enable us to determine
whether we are required to complete an information return with regard to the payments we have
made to you during the year.

Directions to campus: Arrive on campus via 62nd Street to Boundary Street. Parking is available near
both buildings.
Entrance: Enter the front door of the facility you are performing in and sign in with the front desk
receptionist. They will be happy to direct you to the location of your performance and to the load-in
area.
Setup Time: Allow at least a half an hour of preparation before a performance to allow for
contingencies particularly when working with our in house AV equipment.
Cancellation: Notify at least one week in advance the Life Enrichment staff member who scheduled
you. This allows us time to find a replacement for your performance.
Personal Liability: You are responsible for your own equipment and any personal and professional
items that you bring with you. Please check your performance area before you leave to ensure you
have all your items with you.
HIPPA Laws: There can be no video or audio recording or photographs without prior approval from
Cedar Sinai Park Life Enrichment staff.
Please be aware that we are a Jewish facility when choosing material to present to our community.

Dr. Ride Supervisor and Culture Change Director position.
