Trafford has been working to develop integrated care over the past 3 years to address rising demand from an aging population with long-term conditions. They held a clinical congress in 2008 to engage professionals and have since focused on developing an integrated service system and community-based integrated care organization. Their principles guiding development include patient-centered care, general practice as the locus of services, incorporating specialist expertise and social care, and extended roles for nurses. They have established governance arrangements and clinical panels to design new integrated services.
