Health Information Exchange
Delivered by Craig Brammer at CITIH 2011. Focus on discussion of regional and national initiatives and opportunities for regional partners to leverage them for driving healthcare improvements, public health and research.
This session will provide a broad perspective on the many initiatives related to HIT. Experts from the regional and national level will discuss data models, privacy concerns and adoption strategies from their different perspectives. Also addressed will be planning for NHIN direct adoption as a complimentary strategic to full HIEs.
Direct Boot Camp 2 0 Federal Agency requirements for exchange via direct
The FHA Directed Exchange Workgroup provides updates on efforts to increase the adoption of Direct for health information exchange between federal agencies and non-federal entities. They are educating federal partners on Direct technology and policies, developing common understanding of use cases and security requirements, and identifying baseline documents and FAQs. Establishing a common federal Direct policy would greatly increase its adoption for information exchange. Focused workgroups address Directed exchange, security, and interoperability issues. Risks to Directed exchange between agencies are being identified and recommendations will be provided to ONC.
Sharing EHRs - The Canadian Experience

This presentation provides an overview of HIE and the Meaningful Use requirement related to the exchange of clinical information as well as information about standards of exchange and the recommended "next steps" for providers.

The aim of this seminar is to update the project stakeholders on progress and present the work to the teams’ campus community as well as to development and education actors. It will be delivered by Sudan University Innovation Lab on their work on a Vaccination Monitoring System.
About the Pilot Project and the Seminar:
Innovation Labs are national facilities developed by UNICEF in partnership with universities to support the work of UNICEF and governments by building local technological capacities for humanitarian development. The project involves setting up two innovation lab pilots for a period of 4 months to (1) prototype software solutions for selected priorities at UNICEF Sudan Country Office; and (2) use pilots as capacity building mechanisms for innovation teams at participating universities and discussion tools on future innovation possibilities at UNICEF and R & D and Training facilities at Universities.
In collaboration with colleagues at the Faculty of Mathematical Sciences (University of Khartoum) and the Computer Centre (Sudan University of Science and Technology), the Innovation Labs pilot project started in October 2012. The project is based on UNICEF’s Innovation Lab model and their approach to “Technology for Development” which is situated within the growing worldwide momentum and enthusiasm to capitalize on improvements in telecommunications and the rapid uptake of mobile technologies to support development work. The project teams are utilising RapidSMS which is an open source software to improve information flows and access as well as provide real-time data monitoring capabilities for two development programmes where UNICEF is providing support for government. The team at Khartoum University is developing a system to monitor the distribution of school kits while the team at Sudan University of Science and Technology will support national efforts to improve coverage of routine vaccination. The teams are composed of fresh graduates and final year students who volunteered to take part in this pilot which consists of several phases that started with a knowledge sharing exercise using a guided self-learning approach. With one month left, the teams have made considerable progress to work collaboratively, liaise with their customers and technical support network to specify and design the system. They are currently in the coding phase and will be demonstrating in this seminar their first working system component as well as what they learned so far. The seminar will also include a preliminary evaluation of the group’s experience and ideas for the way forward.

The aim of this seminar is to update the project stakeholders on progress and present the work to the teams’ campus community as well as to development and education actors. It will be delivered by Sudan University Innovation Lab on their work on a Vaccination Monitoring System.
About the Pilot Project and the Seminar:
Innovation Labs are national facilities developed by UNICEF in partnership with universities to support the work of UNICEF and governments by building local technological capacities for humanitarian development. The project involves setting up two innovation lab pilots for a period of 4 months to (1) prototype software solutions for selected priorities at UNICEF Sudan Country Office; and (2) use pilots as capacity building mechanisms for innovation teams at participating universities and discussion tools on future innovation possibilities at UNICEF and R & D and Training facilities at Universities.
In collaboration with colleagues at the Faculty of Mathematical Sciences (University of Khartoum) and the Computer Centre (Sudan University of Science and Technology), the Innovation Labs pilot project started in October 2012. The project is based on UNICEF’s Innovation Lab model and their approach to “Technology for Development” which is situated within the growing worldwide momentum and enthusiasm to capitalize on improvements in telecommunications and the rapid uptake of mobile technologies to support development work. The project teams are utilising RapidSMS which is an open source software to improve information flows and access as well as provide real-time data monitoring capabilities for two development programmes where UNICEF is providing support for government. The team at Khartoum University is developing a system to monitor the distribution of school kits while the team at Sudan University of Science and Technology will support national efforts to improve coverage of routine vaccination. The teams are composed of fresh graduates and final year students who volunteered to take part in this pilot which consists of several phases that started with a knowledge sharing exercise using a guided self-learning approach. With one month left, the teams have made considerable progress to work collaboratively, liaise with their customers and technical support network to specify and design the system. They are currently in the coding phase and will be demonstrating in this seminar their first working system component as well as what they learned so far. The seminar will also include a preliminary evaluation of the group’s experience and ideas for the way forward.

Pg2 Beginning in 1991, the IOM (which stands for the Institute of Medicine of the National Academies) sponsored studies and created reports that led the way toward the concepts we have in place today for electronic health records. Originally, the IOM called them computer-based patient records.1 During their evolution, the EHR have had many other names, including electronic medical records, computerized medical records, longitudinal patient records, and electronic charts. All of these names referred to essentially the same thing, which in 2003, the IOM renamed as the electronic health records, or EHR.

The acronym EHR is commonly used as shorthand for Electronic Health Records, and will be used in the remainder of this book.
Institute of Medicine (IOM)
The IOM report2 put forth a set of eight core functions that an EHR should be capable of performing:
Health information and data
This function provides a defined data set that includes such items as medical and nursing diagnoses, a medication list, allergies, demographics, clinical narratives, and laboratory test results. Further, it provides improved access to information needed by care providers when they need it.
Result management
Computerized results can be accessed more easily (than paper reports) by the provider at the time and place they are needed.
· Reduced lag time allows for quicker recognition and treatment of medical problems.
· The automated display of previous test results makes it possible to reduce redundant and additional testing.
· Having electronic results can allow for better interpretation and for easier detection of abnormalities, thereby ensuring appropriate follow-up.
· Access to electronic consults and patient consents can establish critical links and improve care coordination among multiple providers, as well as between provider and patient
Order management
Computerized provider order entry (CPOE) systems can improve workflow processes by eliminating lost orders and ambiguities caused by illegible handwriting, generating related orders automatically, monitoring for duplicate orders, and reducing the time required to fill orders.
· CPOE systems for medications reduce the number of errors in medication dose and frequency, drug allergies, and drug–drug interactions.
· The use of CPOE, in conjunction with an EHR, also improves clinician productivity.
Decision Support
Computerized decision support systems include prevention, prescribing of drugs, diagnosis and management, and detection of adverse events and disease outbreaks.
· Computer reminders and prompts improve preventive practices in areas such as vaccinations, breast cancer screening, colorectal screening, and cardiovascular risk reduction.
Electronic communication and connectivity
Electronic communication among care partners can enhance patient safety and quality of care, especially for patients who have multiple providers in multiple settings that must coordinate care plans.

Breast cancer: Post menopausal endocrine therapy
Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.

GASTROINTESTINAL INFECTIONS result from the ingestion of pathogens that cause infections at the level of this tract, generally being transmitted by food, water and hands contaminated by microorganisms such as E. coli, Salmonella, Shigella, Vibrio cholerae, Campylobacter, Staphylococcus, Rotavirus among others that are generally contained in feces, thus configuring a FECAL-ORAL type of transmission.
Among the factors that lead to the occurrence of gastrointestinal infections are the hygienic and sanitary deficiencies that characterize our markets and other places where raw or cooked food is sold, poor environmental sanitation in communities, deficiencies in water treatment (or in the process of its plumbing), risky hygienic-sanitary habits (not washing hands after major and/or minor needs), among others.
These are generally consequences (signs and symptoms) resulting from gastrointestinal infections: diarrhea, vomiting, fever and malaise, among others.
The treatment consists of replacing lost liquids and electrolytes (drinking drinking water and other recommended liquids, including consumption of juicy fruits such as papayas, apples, pears, among others that contain water in their composition).
To prevent this, it is necessary to promote health education, improve the hygienic-sanitary conditions of markets and communities in general as a way of promoting, preserving and prolonging PUBLIC HEALTH.
Gastritis and Gastric Health
Gastric Health is one of the most relevant concerns in human health, with gastrointestinal infections being among the main illnesses that affect humans.
Among gastric problems, we have GASTRITIS AND GASTRIC ULCERS as the main public health problems. Gastritis and gastric ulcers normally result from inflammation and corrosion of the walls of the stomach (gastric mucosa) and are generally associated (caused) by the bacterium Helicobacter pylor, which, according to the literature, this bacterium settles on these walls (of the stomach) and starts to release urease that ends up altering the normal pH of the stomach (acid), which leads to inflammation and corrosion of the mucous membranes and consequent gastritis or ulcers, respectively.
In addition to bacterial infections, gastritis and gastric ulcers are associated with several factors, with emphasis on prolonged fasting, chemical substances including drugs, alcohol, foods with strong seasonings including chilli, which ends up causing inflammation of the stomach walls and/or corrosion. of the same, resulting in the appearance of wounds and consequent gastritis or ulcers, respectively.
