Mr. Santosh Kumar, son of Prahalad Singh and born on 29 March 1972, underwent safety induction training on 22 January 2016 at Anant Electrical Store in Sunkota, Shajapur district of Madhya Pradesh as a cook/helper. He is valid for three months from the date of issue and is responsible for renewing the pass before expiry, only performing the cleared task, keeping the pass on hand while working, and informing safety officers if lost.

2. 1. It is the personal responsibility of the pass holder to
get the pass renewed before the date of expiry.
2. The individual shall not carry out any other task
other than the one he has been cleared to carry out.
3. This pass is to be compulsorily kept handy while
working at site. Person without the pass will be liable
to be removed from the site.
4. If lost, immediately inform the IWISL safety officer /
Site Engineer.
