The IHI module QI 202: Addressing Small Problems to Build Safer, More Reliable Systems, reflects on the presence of small problems in healthcare systems and how to overcome these small problems to build safer systems. Steve Spear used various examples to make the readers understand the concept of small loopholes and workarounds which distract the employees resulting in mistakes and sometimes these mistakes become catastrophes(Institute for Healthcare Improvement, 2022b). Also, he suggested ways to address them and make healthcare safer. My DPI project which is fall prevention among older adults achieves clinical improvement by building stronger techniques for addressing the reasons and suggesting ways to improve strength, gait, and balance using tai chi as the intervention. Currently, at the practice site, there is only a shift-to-shift report as the standard criteria for fall reporting. Many times people forget to bring to the provider's notice that they experience a fall in the past 3 months. And sometimes if falls are reported, the shift duty nurses forget to enter them into the EHR system. Also, there is currently no special protocol for fall screening. Screening and assessment can help providers know about the population prone to falling and then they can work towards achieving the aim of fall prevention by devising various patient-centered interventions, My DPI project will streamline the process of screening all the older adults above the age of 65 years with CDC STEADI Stay Independent questionnaire (Centers for Disease Control and Prevention.,2020). Those screened at risk will undergo assessment for fall risk factors using a Timed up and go, 30-second chair rise and balance test, with identification of medicines as per Beers criteria, measurement of orthostatic blood pressure, and asking about potential home hazards. Those having poor balance gait and strength will be enrolled in a 12-week tai chi exercise program as the intervention. In order to achieve widespread change, teamwork and collaboration are needed at the levels of the project. Potential sources of mistakes will be addressed in the first place, the commonest of all being linking the fall risk assessment to the patient's e-chart. If my DPI project were implemented in a country with universal healthcare, the widespread change of the project will be affected in multiple ways. Health awareness also significant to economic progress, as healthy population live longer and will be increasingly productivity. Positive mental and physical health is a state of well-being in which the individual realizes his or her own abilities, can cope with the normal stresses of life, can work productively and fruitfully and can contribute to his or her community. In addition, our results reveal that there is a strong correlation between the view count of a video and the average view count of its top referrer videos. The aim of this study are to know the impacts of YouTube as a health awareness video to traveler from Malaysia, and also to find out factors of usage of YouTube rather than other medium applications . Besides that, the factors and the impact of usage is influencing the YouTube as a main website popular and the most websites trending in the worldwide. Health awareness is very crucial information that each person needs to know to ensure that they can maintain a better and healthier life. This study conducted out of 68 respondents where is 35 of female and 33 of male. Most of respondents are in range 15 to 45 years old that are users of new media platforms especially YouTube and interact frequently on these platforms. The questionnaire is made up of three major sections which collect data on the respondents’ demographic profile, their exposure to YouTube, and their knowledge about health awareness video. As the result, this study showed that YouTube as a main platform to find any precautions about health awareness video or materials. In the meanwhile, the researcher agrees personally that YouTube is the best place to look for any videos or materials on health awareness precautions. With this study, it is made evident that YouTube's effects as a health awareness film for travelers from Malaysia altered a respondent's willingness to agree with the study's objectives

The Clinical Practice Guidelines produced by the departments of Defense and Veterans Affairs provide a framework for ensuring evidence-based care for patients with mTBI. This webinar will demonstrate two mobile applications produced by the National Center for Telehealth & Technology that offer providers evidence-informed tools for the treatment and engagement in clinical care of patients with mTBI.
Disparities in health outcomes are a result of a myriad of socio-ecological factors that are linked to education, employment, income, discrimination based on race/ethnicity, gender, religion, sexual orientation, geographic location, mental health and/or disability. These factors are commonly referred to as social determinants of health (SDOH). The World Health Organization defines SDOH as the conditions in which people are born, grow up, work and live and the structures and systems that shape the daily conditions of life. There has been a great deal of research focused on SDOH in the past decade that is critical to informing policy and practice necessary to promote health equity. However, it is also important to acknowledge that this concept is not new. Unacceptable health disparities remain despite substantial evidence, over the past century, which shows SDOH are at the root cause of health disparities.

Background Environmental health the branch of public health concerned with monitoring or mitigating those factors in the environment that affect human health and disease. Or other words the condition of the environment in a particular region, especially as regards ecological diversity or pollution. Objectives The aim of this study was to assess effectiveness of planned teaching programme on environmental health among the community people. Methodology The research approach adopted for this study is a Quantitative research approach. The research design was pre test and post test design. The pilot study was conducted at Rural Area Gandhi Nagar Bhopal. A Convenient sampling technique was used. Structured knowledge questionnaire was used to assessing the environmental Health. The final study was conducted with 50 sample in schools was given followed by post test after 7 days using the same pre test tools. The data collected was analyzed using inferential statistics. Results Indicated overall pre test and post test mean knowledge scores on environmental health. Depicted mean post test score 24.95 is higher than mean pre test score of 16.825. The actual gain knowledge score is 8.125 and post test SD =3.25, pre test SD=4.50 and computed paired t test 9.3235 p= 2.04 at the level of 0.05. Thus, data showed higher than the tabled value t test = 2.18 at the level of 0.05 thus indicated significant difference and effectiveness of planned teaching program, in increasing the knowledge of Community people regarding environmental health. The computed ”˜t’ value t=9.3235 was higher than the table value t=2.04 at 0.05 level of significance. Hence, the research hypothesis H1 was accepted. Conclusion The study concluded that planned teaching program was effective in increasing the knowledge score of Community people regarding environmental health. (HP 2010) goals and objectives build on the previous
two decades’ accomplishments and set national targets
for reducing disease and disability and promoting
healthier, longer lives (DHHS, 2000b). Led by the U.S.
DHHS, the overarching goals of the current initiative
are to increase quality and years of life and to eliminate
health disparities (Davis, 2000; DHHS, 2003). DHHS
(2001) encourages working through communities and
local organizations to influence individual behavior
and the promotion and maintenance of environments
conducive to healthier lifestyles.

Building on the health promotion premise that orga-
nizations and communities are instruments of change
(McLeroy, Bibeau, Steckler, & Glanz, 1988; Stokols,
1992), the DHHS Office of Disease Prevention and
Health Promotion (ODPHP) initiated a pilot study in
October 2001 to develop a national model for engaging
community organizations in health promotion and dis-
ease prevention activities that reflect the national HP

The U.S. health care system faces significant challenges that clearly indicate the urgent need for reform.
There is broad evidence that Americans often do not get the care they need even though the United States spends more money per person on health care than any other nation in the world.
Preventive care is underutilized, resulting in higher spending on complex, advanced diseases.
Practical Policy for Preventive Services
Patients with chronic diseases too often do not receive proven and effective treatments such as drug therapies or self management services to help them more effectively manage their conditions.
These problems are exacerbated by a lack of coordination of care for patients with chronic diseases.
Reforming our health care delivery system to improve the quality and value of care is essential to address escalating costs, poor quality, and increasing numbers of Americans without health insurance coverage.

This webinar will demonstrate two mobile applications produced by the National Center for Telehealth & Technology that offer providers evidence-informed tools for the treatment and engagement in clinical care of patients with mTBI.
Disparities in health outcomes are a result of a myriad of socio-ecological factors that are linked to education, employment, income, discrimination based on race/ethnicity, gender, religion, sexual orientation, geographic location, mental health and/or disability. These factors are commonly referred to as social determinants of health (SDOH). The World Health Organization defines SDOH as the conditions in which people are born, grow up, work and live and the structures and systems that shape the daily conditions of life. There has been a great deal of research focused on SDOH in the past decade that is critical to informing policy and practice necessary to promote health equity. However, it is also important to acknowledge that this concept is not new. Unacceptable health disparities remain despite substantial evidence, over the past century, which shows SDOH are at the root cause of health disparities.

A Study to Assess the Effectiveness of Planned Teaching Programme on Environm...ijtsrd
Background Environmental health the branch of public health concerned with monitoring or mitigating those factors in the environment that affect human health and disease. Or other words the condition of the environment in a particular region, especially as regards ecological diversity or pollution. Objectives The aim of this study was to assess effectiveness of planned teaching programme on environmental health among the community people. Methodology The research approach adopted for this study is a Quantitative research approach. The research design was pre test and post test design. The pilot study was conducted at Rural Area Gandhi Nagar Bhopal. A Convenient sampling technique was used. Structured knowledge questionnaire was used to assessing the environmental Health. The final study was conducted with 50 sample in schools was given followed by post test after 7 days using the same pre test tools. The data collected was analyzed using inferential statistics. Results Indicated overall pre test and post test mean knowledge scores on environmental health. Depicted mean post test score 24.95 is higher than mean pre test score of 16.825. The actual gain knowledge score is 8.125 and post test SD =3.25, pre test SD=4.50 and computed paired t test 9.3235 p= 2.04 at the level of 0.05. Thus, data showed higher than the tabled value t test = 2.18 at the level of 0.05 thus indicated significant difference and effectiveness of planned teaching program, in increasing the knowledge of Community people regarding environmental health. The computed ”˜t’ value t=9.3235 was higher than the table value t=2.04 at 0.05 level of significance. Hence, the research hypothesis H1 was accepted. Conclusion The study concluded that planned teaching program was effective in increasing the knowledge score of Community people regarding environmental health. Services [DHHS], 2000a). Today’s Healthy People 2010
(HP 2010) goals and objectives build on the previous
two decades’ accomplishments and set national targets
for reducing disease and disability and promoting
healthier, longer lives (DHHS, 2000b). Led by the U.S.
DHHS, the overarching goals of the current initiative
are to increase quality and years of life and to eliminate
health disparities (Davis, 2000; DHHS, 2003). DHHS
(2001) encourages working through communities and
local organizations to influence individual behavior
and the promotion and maintenance of environments
conducive to healthier lifestyles.

Building on the health promotion premise that orga-
nizations and communities are instruments of change
(McLeroy, Bibeau, Steckler, & Glanz, 1988; Stokols,
1992), the DHHS Office of Disease Prevention and
Health Promotion (ODPHP) initiated a pilot study in
October 2001 to develop a national model for engaging
community organizations in health promotion and dis-

Healthy People was a call to action and an attempt to set health goals for the United States for the next 10 years.
Healthy People 2000 established 3 general goals:

Reduce health disparities.

Practical Policy for Preventive Services
The U.S. health care system faces significant challenges that clearly indicate the urgent need for reform.
There is broad evidence that Americans often do not get the care they need even though the United States spends more money per person on health care than any other nation in the world.
Preventive care is underutilized, resulting in higher spending on complex, advanced diseases.
Practical Policy for Preventive Services
Patients with chronic diseases too often do not receive proven and effective treatments such as drug therapies or self management services to help them more effectively manage their conditions.
These problems are exacerbated by a lack of coordination of care for patients with chronic diseases.
Reforming our health care delivery system to improve the quality and value of care is essential to address escalating costs, poor quality, and increasing numbers of Americans without health insurance coverage.
Why policies need to be developed?
Basic needs are not being met (e.g., People are not receiving the health care they need)
People are not being treated fairly (e.g., People with disabilities do not have access to public places)
Resources are distributed unfairly (e.g., Educational services are more limited in neighborhoods of concentrated poverty)
