6. REQUIRED REPORTS – Original
Copies
A. Copy of the electronically encoded Cooperative Annual Progress Report
(CAPR) with the following attachment:
1. Social Audit Report including its program of activities pursuant to its socio-civic goals;
2. Performance Audit Report, including copies of the semi annual Report on mediation
and conciliation as received by the Authority pursuant to Rule 7 of the Revised IRR;
3. Financial Statements audited by an External Auditor accredited by the CDA; and
4 List of Officers and Mandatory Trainings Undertaken / Completed.
B. Certification fee of P100.00
7. REQUIRED REPORTS –

8. REQUIRED REPORTS – AFS
• Statement of Financial Condition
• Statement of Operation
• Statement of Changes in Members Equity
• Statement of Cash flows
• Notes to financial Statement
10. REQUIRED REPORTS
Shall be submitted at Regional Office Level duly received by the
officer-of-the-day.
No required reports shall be accepted when there is a deficiency.
All reports received by the provincial offices are not valid unless
forwarded immediately to Regional office level on or before the
deadline or else the cooperative shall incur delay in accordance with
the provision of the law and/or memorandum circular issued.
11. REQUIRED REPORTS - Delay
Delay commence when, after 120 days of end of fiscal year
Subject to monetary penalty of P100 per day of delay except for micro
cooperatives.
12. Reminders:
• Partial submission of the required reports = noncompliance and
subject to appropriate legal action.
• Reports Submitted with lack or nor signatures will be considered as
noncompliance
• Reports submitted with material discrepancy in the AFS will not be
issued COC
13. Certificate of Compliance
Shall be released at REGIONAL OFFICE LEVEL upon completion of the
required reports within the prescribe period as indicated in the
corresponding claim stub.
COC shall be released only to the officers of the cooperative or any of
its authorized representative.
