Race and Preference for Knee Replacement

Hourly rounding was implemented in a long term care facility to reduce falls among residents. The pilot program involved education of staff on hourly rounding and use of a checklist to document rounds. Initial results showed a 43% reduction in falls during the 30 day pilot period compared to the previous year. However, compliance with hourly rounding decreased due to staffing shortages. Continuous education and supervision are recommended to sustain fall reductions through consistent hourly rounding. Direct observation of staff also improved adherence to the rounding protocol.

HOURLY ROUNDS APRIL 13, 2015
Hourly rounding was implemented in a long term care facility to reduce falls among residents. The pilot program involved education of staff on hourly rounding and use of a checklist to document rounds. Initial results showed a 43% reduction in falls during the 30 day pilot period compared to the previous year. However, compliance with hourly rounding decreased due to staffing shortages. Continuous education and supervision are recommended to sustain fall reductions through consistent hourly rounding. Direct observation of staff also improved adherence to the rounding protocol.

This study assessed the costs and effects of different degrees of task shifting for anti-retroviral therapy (ART) from physicians to other health professionals in Ethiopia. This study examined outcomes of 100 fingertip injuries treated at Bellevue Hospital between January and May 2011. 64% healed without surgery, 18% required surgery, and 18% were lost to follow up. Those requiring surgery tended to have larger defects, volar oblique injuries, exposed bone, and distal phalanx fractures. While treatment algorithms exist, the study found that many fingertip injuries can be treated nonoperatively to achieve good function and sensation and allow for earlier return to work.

Abstract: The rate of unplanned 30-day readmissions to the hospital after discharge is being used as a marker to compare the quality of care across hospitals and to set reimbursement levels for care. While the readmission rate can be reported using administrative data, the accuracy of these data is variable, and defining which readmissions are unplanned and preventable is often difficult. The purpose of this study was to review readmissions to a single orthopedic hospital to identify the causes for readmission and, in particular, which readmissions are planned versus unplanned. Using that hospital's administrative database of patient records from 2007 to 2009, we identified all patients who were readmitted to the hospital within 30 days of a previous hospitalization for a procedure. Readmissions were broadly categorized as planned (a staged or rescheduled procedure or a direct transfer) or unplanned. Unplanned readmissions were defined as either surgical or nonsurgical complications (medical conditions not directly related to the procedure). Almost 30 percent of readmissions were planned. Of the unplanned readmissions, close to 60 percent were triggered by an infection or a concern for an infection. Nonsurgical complications accounted for 18.2 percent of unplanned readmissions. This study highlights the importance of careful data collection and abstraction when calculating early readmission rates. Preventing surgical site infection and better coordinating care between orthopedic surgeons and primary care and medical subspecialty physicians may significantly reduce readmission rates. 1) The study analyzed over 30,000 pediatric trauma patients treated at level I or II adult or pediatric trauma centers to compare the use of whole body CT (WBCT) scans between facilities.
2) It found WBCT scans were used significantly more often on pediatric patients treated at adult trauma centers (31.4%) compared to pediatric centers (17.6%).
3) After adjusting for factors, pediatric patients treated at adult centers were 1.8 times more likely to receive a WBCT, increasing their radiation risk without improving outcomes, as mortality did not differ between the groups. The study concludes guidelines are needed to minimize unnecessary WBCT use across centers.

The Affinity project aims to implement Ireland's National Strategy for preventing falls and fractures in the aging population by developing a governance framework and targeting 15-30% of those aged 65+. As part of the project, St. Columcille's Hospital established a monthly falls clinic that provides same-day access to DXA scans, physiotherapy, occupational therapy and other assessments and interventions. Over a 6 month pilot period, the clinic assessed 24 patients, 67% of whom required DXA scans which found 100% had osteopenia or osteoporosis. The project successfully identified at-risk patients and provided multidisciplinary care to address their needs.

Intervention
The capstone change proposal is effects of disproportionate nurse to patient staffing ratios on the quality of patient care. Patients can be exposed to several safety issues if proper care is not given to them. These problems include falls, hospital-acquired infection due to poor hand hygiene by the healthcare workers, medication administration errors, poor health education to the patients, and negligence in attending to the spiritual needs of the patients. Interventions includes presenting the safety concerns to the management team of the facility to enable them to hire more nurses to deliver adequate care to the patients. In-service training of the nurses on fall prevention, proper application of fall precautions and identification of patients who are at risk of falls are another important intervention. Proper hand hygiene is an intervention that will prevent hospital-acquired infections and nurses should form the culture of doing it (Sands, & Aunger, 2020). Medication errors can lead to complications or death of patients. Nurses should check the medications properly and identify the patients before administration of the medications.
Evidence Based Literature
The articles reviewed have different research aims and questions, but they are all centered into the idea of the effects of nurse-to-patient ratios on patient outcomes. The research questions of these articles are divided into three categories: definition of nursing staffing, effects of nursing-to-patient ratio on patient outcomes and nursing characteristics that hinders the delivery of care. The study by (Cho et al., 2020), defines the term nursing staffing in terms of the nursing care needs of the patients.
Nurses are essential in the provision of quality care in acute units, and their staffing levels have an impact on patient outcomes. (Cho et al., 2015), examine the link between nursing staffing and patient outcomes, specifically the mortality rate. Comparing to (Driscoll et al., 2018) and (Shin et al., 2018), the articles examine the effects of nursing staffing ratios on the patient outcomes in acute specialist units. Besides, (Needleman, 2016) reviews the studies that examine the effects of nursing skill mix on the patient outcomes such as patient ratings of hospitals, mortality, adverse health outcomes, and nurse burnout and dissatisfaction.
Some of the factors such as nursing skills, staffing methods, and working environment affects the nursing staffing ratio, which hinders the quality of care. The article by (Bridges et al., 2019), explores the relationship between nursing staffing skills and the quality and quantity of their interactions with patients in hospital wards. (Olley et al., 2019).

- 2. Background Falling is a common and morbid condition among elderly persons. Effective strategies to prevent falls have been identified but are underutilized.
- 3. Methods Using a nonrandomized design, we compared rates of injuries from falls in a region of Connecticut where clinicians had been exposed to interventions to change clinical practice (intervention region) and in a region where clinicians had not been exposed to such interventions (usual-care region).
- 4. The interventions encouraged primary care clinicians and staff members involved in home care, outpatient rehabilitation, and senior centers to adopt effective risk assessments and strategies for the prevention of falls (e.g., medication reduction and balance and gait training).
- 5. The outcomes were rates of serious fall-related injuries (hip and other fractures, head injuries, and joint dislocations) and fall-related use of medical services per 1000person-years among persons who were 70 years of age or older. The interventions occurred from 2001 to 2004, and the evaluations took place from 2004 to 2006.
- 6. Results Before the interventions, the adjusted rates of serious fall-related injuries (per 1000 person-years) were 31.2 in the usual-care region and 31.9 in the intervention region. During the evaluation period, the adjusted rates were 31.4 and 28.6, respectively (adjusted rate ratio, 0.91; 95% Bayesian credibility interval, 0.88 to 0.94).
- 7. Between the preintervention period and the evaluation period, the rate of fall-related use of medical services increased from 68.1 to 83.3 per 1000 person-years In the usualcare region and from 70.7 to 74.2 In the intervention region (adjusted rate ratio, 0.89; 95% credibility interval, 0.86 to 0.92).
- 8. The percentages of clinicians who received intervention visits ranged from 62% (131 of 212 primary care offices) to 100% (26 of 26 home care agencies).
- 9. Conclusions Dissemination of evidence about fall prevention, coupled with interventions to change clinical practice, may reduce fall related injuries in elderly persons.
- 10. Thanks
