4. • Mortality is 1-5 % without treatment.
• Dengue is endemic in more than 110 countries
• Infection rate range b/w 50-100 million people/ year
• Approx - 12 -25000 death's / year.
• It's a global problem since 2nd world War.
• Dengue fever virus ( DENV ) is RNA Virus family - flaviviridia, genus - flavivirus
• Most are transmitted by arthropods ( mosquito/ticks)
• 4 types of virus
EPIDEMIOLOGY
Serotype
• DENV 1
• DENV 2
• DENV
3
• DENV 4
5. TRANSMISSION
• Transmitted by several species of mosquito.
• Dengue virus is primarily transmitted by Aedes mosquito particularly A.aegypti
(they bite during day mostly)
• some other species
( A.albopictus, A.polynesiensis & A.scutellars
• Humans are primary host of virus
• Singal bite is enough tonacquire this virus.
Do you know female?
Mosquito that takes blood meal from person infected with this fever Becomes itself
infected with virus.
8-10 days later virus spread to other tissue, including mosquitoes salivary gland.
6. PATHOPHYSIOLOGY
• Mosquito carrying dengue virus bits person
• Virus enters skin together with Mosquito saliva
• It binds & enters WBC & reproduces inside cells while they move throughout
body
• In Severe infection, vimy production inside body is increases & other organs
can be affected & fluid from bloodstream leaks through wall of small blood
vessels into body cavities.
• This results , Reduce blood flood in vessels & BD decreases it can't Supply
blood to vital organ .
• Dysfunction of bone marrow leads to reduce number of platelets necessary for
effective blood clotting , this increases risk of bleeding.
7. SIGNS & SYMPTOMS
• Fever (more than 105)
• Headache
• Incubation period 3-14 days but often it is 4-7 days
• Muscle & Joint pains
• Nausea
• Pain behind eyes
In small porportion of causes disese develop into life-threating dengue hemorrhagic fever.
8. • Fever More than 105
• Headache
• Incubation period 3-14 days but often it is 4-7 days
• Muscle & Joint pains
• Nausea
• Pain behind eyes
• Vomitting
In small poportion of cases discase develop into life-threating ' dengue
hemorrhagic fever '
WARNING SIGNS
11. FEBRILE PHASE
• Headache
• Generalised
• High fever
• Rashes (1/2 days of symptoms as flushed skin)
• Petechiae Csmall red spot)
12. CRITICAL
PHASE
• High Fever
• Fluid accumulation in chest & abdominal cavity due to increased capillary
permeability
• Leads to depletion of fluid from Circulation & to Blood supply to vital organ
• Organ dysfunction & severe bleeding
• Shock & hemorrhage occur in less than 5% of all dengue Cases
13. RECOVERY PHASES
• resorption of leaked fluid into bloodstream
• last's 2-3 days
• Itching & Decrease HR
• Fluid overload occurs if it affected brain, causes decrease Consciousness /
Seizures
14. DIAGNOSIS
• Clinically done
• Changes detected on lab investigation ( decrease WBC, decrease platelets)
• probable diagnosis based on finding oof fever + nemsea, rash, pain, etc
