Haemophagocytic Syndrome
A 20-year-old woman presented with fever, fatigue, and weight loss. She had a history of Holt-Oram syndrome and primary EBV infection a year prior. Testing showed pancytopenia, hepatosplenomegaly, and elevated ferritin. A bone marrow biopsy showed hemophagocytosis. She was diagnosed with EBV-associated hemophagocytic syndrome and died despite treatment. Hemophagocytic syndrome is caused by impaired NK and cytotoxic T cell function leading to uncontrolled immune activation and cellular damage. It can be genetic, infection-associated, malignancy-associated, or associated with autoimmune diseases. Treatment involves controlling inflammation, the underlying trigger, and supportive care.
Mixed cryoglobulinemia & HEV infection
1) Mixed cryoglobulinemia has been reported related to chronic hepatitis E virus (HEV) infection, with 9 documented cases described.
2) All cases involved immunosuppressed patients who developed chronic HEV genotype 3 infection.
3) Treatment with antiviral therapy such as pegylated interferon or ribavirin led to viral clearance and resolution of cryoglobulinemia in reported cases.

Deep vein thrombosis is more common in HIV-infected individuals due to factors causing a hypercoagulable state induced by the virus. The most common symptoms were fever, headache, myalgia, and abdominal pain. Bleeding manifestations occurred in 19% of patients, most commonly melena. Laboratory findings included thrombocytopenia in all patients and elevated hematocrit in 23% of patients. 21% of cases had severe dengue hemorrhagic fever/dengue shock syndrome. The study aims to better understand the characteristics of dengue patients to aid in diagnosis and management.
