Chronic diseases are the leading cause of death and disability in the United States. Community Health Workers play an important role in preventing and managing chronic diseases through education, encouragement of healthy behaviors, and ensuring patient understanding of their conditions and medications. The presentation will cover the strategies used by Community Health Workers, including motivational interviewing, developing action plans, teaching back to verify understanding, and focusing on patient-centered care as part of the healthcare team.
