Wk 6- Adult vs. Pediatric Emergencies
Adults presenting for psychiatric emergencies are often handled quite different than children. The decision-making rights of an adult is one of the most basic human rights that must be respected whenever possible (Mental Health America [MHA], 2015). However, children and adolescents presenting for psychiatric emergencies also have rights, but these rights are sometimes different because of their inability to make some independent decisions as a result of them not being of age to grant legal consent. Take for example, Sara, a 41-year-old female presenting to the emergency department after a domestic violence dispute with her husband. Sara’s husband has been physically and emotionally abusive for years. The most recent physical assault resulted in her needing stitches in her forehead and multiple bruises are visible on her face and body. Sara was treated for her injuries, a police report was filed, and she was provided information of shelters for victims of domestic violence. However, Sara declined all offers and asked to be released from the hospital so that she could go bail her husband out of jail. This case is unfortunate, but not uncommon, with many women choosing to return to their abusive partners.
In contrast, consider a child presenting to the emergency department with similar injuries inflicted by the parents. However, this case is less obvious with the parents saying the injuries were “an accident”. When the nurse attempts to evaluate the child, without the parent in the room, the parent refuses to leave, thereby making the nurse suspicious of child abuse. This situation is quite different in that the child’s physical injuries and suspected abuse must be reported. The provider that suspects or discovers child abuse is considered a mandatory reporter and are required, by law, to report suspected child abuse (Child Welfare Information Getaway, 2019). Mandatory reporting of suspected child abuse is both a legal and moral requirement for psychiatric providers. We must advocate for our young clients and psychiatric providers have a legal and ethical duty to continually evaluate their safety in the home environment (Sadock et al., 2014). Based on this concern, I would call the police and Child Protective Service (CPS) to assist with evaluating the safety of this child’s home environment.
The child’s safety at home and mandatory reporting is quite different from that of Sara, the adult victim of domestic .

Client Information
INITIAL CASE CONCEPTUALIZATION: PTSD !2
Maria is a 25 year old African American female, with four children under the age of six
years old. To ensure her protection of her identity, I will refer to her as Maria. Maria states that
she is a christian and does not attend church often. Maria reports that she would like to become
more active in church again. Maria says that she is currently in an abusive relationship with her
spouse of seven years. While she is not physically disabled, she mentioned that she is facing var-
ious psychological challenges that have made her live in an inpatient psychological hospital.
Maria says that she identifies as a female who is attracted to the opposite sex. She is currently
working two jobs to sustain her life and the lives of her four children aged six, three, one, and
two months (1boy and 3 girls). Therefore, she is financially and economically challenged; but
she has been doing the best she could to make ends meet despite her circumstances.
Maria has managed to secure a 2-bedroom apartment that she shares with her four chil-
dren. Besides her two jobs, she also seeks welfare support to help her with daycare, medical and
food assistance. Maria also mentioned that her older brother sends her checks from time to time
to help her with the children. Additionally, she makes use of food stamps that have been availed
to her to cut the cost of food. Some non-governmental organization has also volunteered to take
care of educational needs of her two older children. While her emotional and psychological state
seems a bit unstable, Maria is physically healthy, and reports that she takes care of her physical
wellbeing, by walking 3 miles per day, to catch a bus to work.
Maria presenting issues is PTSD, she has been sexually assaulted numerous of times
throughout her childhood. She seems to be trying to forget some issues of her past that bring
back bad memories regarding her sexual abuse. She seems traumatized by her sexual past, which

is apparent in how she disregards any questions about her sexual abusive past. The relevant his-
tory behind her traumatic response to sexual-related questions stems from her experiences as a
young girl. At the time of the abuse, she was living with her grandmother, cousins, uncles, broth-
ers, and aunts. From the ages of four to nine, she was sexually abused by her family members.
The trauma from those incidents has seemingly remained with her in her adult life. It is affecting
her relationship with males.
Theoretical Approach
The theoretical approach I used for Maria was Trauma-Focused Cognitive Behavioral
Therapy (TF-CBT). This is an evidence-based theoretical approach to treatment of traumatized .
Affirming Social Work Practice with Transgender Individuals
This document provides guidance for social workers to effectively work with transgender individuals. It begins with key terms and discusses that gender identity is self-identified. It then estimates the transgender population in the US and barriers to data collection. Guiding assumptions are outlined, including that gender identity is not a pathology. Barriers to healthcare access and high rates of poverty, unemployment, discrimination and suicide among transgender individuals are reviewed. The importance of trans-affirming mental healthcare is discussed, including listening without assumptions and using correct names and pronouns. Voices of transgender individuals describe wanting to be seen as whole people rather than only in terms of their identity.

ious psychological challenges that have made her live in an inpatient psychological hospital.
Maria says that she identifies as a female who is attracted to the opposite sex. She is currently
working two jobs to sustain her life and the lives of her four children aged six, three, one, and
two months (1boy and 3 girls). Therefore, she is financially and economically challenged; but
she has been doing the best she could to make ends meet despite her circumstances.
Maria has managed to secure a 2-bedroom apartment that she shares with her four chil-
dren. Besides her two jobs, she also seeks welfare support to help her with daycare, medical and
food assistance. Maria also mentioned that her older brother sends her checks from time to time
to help her with the children. Additionally, she makes use of food stamps that have been availed
to her to cut the cost of food. Some non-governmental organization has also volunteered to take
care of educational needs of her two older children. While her emotional and psychological state
seems a bit unstable, Maria is physically healthy, and reports that she takes care of her physical
wellbeing, by walking 3 miles per day, to catch a bus to work.
Maria presenting issues is PTSD, she has been sexually assaulted numerous of times
throughout her childhood. She seems to be trying to forget some issues of her past that bring
back bad memories regarding her sexual abuse. She seems traumatized by her sexual past, which

is apparent in how she disregards any questions about her sexual abusive past. The relevant his-
tory behind her traumatic response to sexual-related questions stems from her experiences as a
young girl. At the time of the abuse, she was living with her grandmother, cousins, uncles, broth-
ers, and aunts. From the ages of four to nine, she was sexually abused by her family members.
The trauma from those incidents has seemingly remained with her in her adult life. It is affecting
her relationship with males.
Theoretical Approach
The theoretical approach I used for Maria was Trauma-Focused Cognitive Behavioral
Therapy (TF-CBT). This is an evidence-based theoretical approach to treatment of traumatized .

"Sex is virtually all in the head anyway" (Cloud, 2010). This quote is how chapter six is started, and I find it very suiting for this week's discussion topic. A sex offender definition is someone who has been convicted of a sex crime involving a minor. This offense is severe, and when sentencing at a state or federal level, multiple factors are considered (Sex Offenders and Sex Offenses: Overview 2019). I have heard many people comment that pedophiles should be executed right after the trial, that sexually abusing a child is the unforgivable crime and that a person who commits such a crime has a mind that can not be healed. During Pre-Prac 1, Dr. Gess had stated that whatever population you do not want to work with, that is what you will get. She had shared that her population was pedophiles, and that was her first client. Dr. Gess continued that she ended up discovering that he was abused as a child and did not receive any counseling. He kept the vicious cycle alive, but that she ended up developing a strong rapport with him and learned a lot. Study after study has shown that when an individual experiences childhood trauma, it increases the risk for health and social problems and that it is a common experience for sexual offenders (Levenson, Willis & Prescott, n.d). Reading the synopsis of Josiah's first session, I have a few questions regarding his risky behaviors with alcohol as well as with sex. First, is he having sex while intoxicated or have his sexual encounters been during periods of sobriety? Second, while binge drinking is made common by media and groups of people, it is still not a commonly accepted behavior in our culture... He may have found a niche drinking group of people. Therefore, what does Josiah deem as "binge drinking" if not his behavior with alcohol? Does he understand the impairments in judgement when drinking can inhibit rational decision-making, therefore increasing the likelihood of risky behaviors in sexual encounters? What forms of "sex" is he having? Is it penetrative intercourse, oral, etc? I ask this because forms of sex that are not penetrative carry no concern for unwanted pregnancies, but still are risky in terms of getting STIs.
In conversation with him, I would ask questions, guiding him to conclude that sexual acts involving more than one person, do just that... Involve more than one person. That means the choices he makes in the shared sexual experiences do not affect him alone. They also affect his sexual partner or partners at the time. He needs to understand that the means in which he engages in sex can be considered reckless and thoughtle.

In his initial intake interview, Jerry admitted to being sexually attracted to young girls. He stated he had difficulty controlling his urges, which led to the crime he committed. Jerry has received counseling while in prison, but will still need to undergo treatment when he is released.
The Case of Sal
Sal is a 20-year-old man currently in prison for molesting his 4-year-old cousin, Maria. Sal, who was living with Maria’s family and was often responsible for babysitting her, had been molesting Maria for 6 months. Sal initially enticed Maria through playing a game. When she would lose, he would tell her she could either allow him to touch her, or he would tell her mom she had lost the game and she would get in trouble. Sal also told Maria she needed to keep their game a secret, because he did not want her to get in trouble. Maria broke down in preschool and told her teacher about the incidents of molestation. The teacher immediately called the police, and Sal was arrested.
Sal tried to minimize the situation and his offense by stating “it was only touching.” He also tried to justify the offense by stating Maria enjoyed the game they played. Sal is currently undergoing psychological rehabilitation treatment at the prison.

This study explored self-perception of stigma and discrimination among men who have sex with men (MSM) in Kathmandu Valley, Nepal. It found that MSM experience stigma from families, healthcare providers, workplaces, and law enforcement. Discrimination was particularly faced by "Metis", biologically male MSM who see themselves as feminine. Common forms of discrimination included violence, refusal of services, outing of sexual orientation without consent, and being forced into marriage or sex work. However, some MSM reported family acceptance and decreasing discrimination over time, which they attributed to education and advocacy efforts. The study highlights the need to address stigma in order to improve health outcomes for MSM in Nepal.

Gender Dysphoria
Transgender is the term used to mean that individual sex assigned during birth based on the external genital does not fit their gender identity. These kinds of people usually experience gender dysphoria, which is one of the psychological distresses associated with the incongruence between one's gender identity and the sex they were assigned during birth. Gender dysphoria usually starts at the beginning of childhood, but in some cases, individuals may not experience it until after puberty and even much later in their stages of life. People with transgender may pursue different domains of gender affirmation, which includes: surgical affirmation, medical affirmation, legal affirmation, and social affirmation (Lindley, 2020). Not all people with transgender issues will desire to have all these domains of gender affirmation as these are highly individual and personal decisions.
Medical treatment for gender dysphoria
Medical treatments for this condition may include hormone therapy, such as masculinizing hormone therapy or feminizing hormone therapy. Surgery includes masculinizing surgery or feminizing surgery, which intends to change chest or breasts, internal genitalia, external genital, body contouring, and facial features.
Some people used hormone therapy to seek maximum masculinization or feminization. Other people find relief from gender dysphoria by using hormones to maximize secondary sex characteristics such as facial hair and breasts. These kinds of treatments are based on individuals' goals and the evaluation of the benefits and risks of the medication use, presence of any other conditions, and consideration of individual's economic and social issues (Zucker, 2018). Most of these gender dysphoria individuals find surgery the most effective and necessary procedure that relieves their condition.
The World Professional Association for Transgender Health gives the following procedure for the surgical or hormonal treatment for people with gender dysphoria.
1. Persistent and a well-documented gender dysphoria
2. The capacity to make consent and an informed decision for the treatment
3. The aged majority of a given country and when the patient is young need to follow the standard of care for the adolescents and children.

"Sex is virtually all in the head anyway" (Cloud, 2010). This quote is how chapter six is started, and I find it very suiting for this week's discussion topic. A sex offender definition is someone who has been convicted of a sex crime involving a minor. This offense is severe, and when sentencing at a state or federal level, multiple factors are considered (Sex Offenders and Sex Offenses: Overview 2019). I have heard many people comment that pedophiles should be executed right after the trial, that sexually abusing a child is the unforgivable crime and that a person who commits such a crime has a mind that can not be healed. During Pre-Prac 1, Dr. Gess had stated that whatever population you do not want to work with, that is what you will get. She had shared that her population was pedophiles, and that was her first client. Dr. Gess continued that she ended up discovering that he was abused as a child and did not receive any counseling. He kept the vicious cycle alive, but that she ended up developing a strong rapport with him and learned a lot. Study after study has shown that when an individual experiences childhood trauma, it increases the risk for health and social problems and that it is a common experience for sexual offenders (Levenson, Willis & Prescott, n.d). Reading the synopsis of Josiah's first session, I have a few questions regarding his risky behaviors with alcohol as well as with sex. First, is he having sex while intoxicated or have his sexual encounters been during periods of sobriety? Second, while binge drinking is made common by media and groups of people, it is still not a commonly accepted behavior in our culture... He may have found a niche drinking group of people. Therefore, what does Josiah deem as "binge drinking" if not his behavior with alcohol? Does he understand the impairments in judgement when drinking can inhibit rational decision-making, therefore increasing the likelihood of risky behaviors in sexual encounters? What forms of "sex" is he having? Is it penetrative intercourse, oral, etc? I ask this because forms of sex that are not penetrative carry no concern for unwanted pregnancies, but still are risky in terms of getting STIs.
In conversation with him, I would ask questions, guiding him to conclude that sexual acts involving more than one person, do just that... Involve more than one person. That means the choices he makes in the shared sexual experiences do not affect him alone. They also affect his sexual partner or partners at the time. He needs to understand that the means in which he engages in sex can be considered reckless and thoughtle.

In his initial intake interview, Jerry admitted to being sexually attracted to young girls. He stated he had difficulty controlling his urges, which led to the crime he committed. Jerry has received counseling while in prison, but will still need to undergo treatment when he is released.
The Case of Sal
Sal is a 20-year-old man currently in prison for molesting his 4-year-old cousin, Maria. Sal, who was living with Maria’s family and was often responsible for babysitting her, had been molesting Maria for 6 months. Sal initially enticed Maria through playing a game. When she would lose, he would tell her she could either allow him to touch her, or he would tell her mom she had lost the game and she would get in trouble. Sal also told Maria she needed to keep their game a secret, because he did not want her to get in trouble. Maria broke down in preschool and told her teacher about the incidents of molestation. The teacher immediately called the police, and Sal was arrested.
Sal tried to minimize the situation and his offense by stating “it was only touching.” He also tried to justify the offense by stating Maria enjoyed the game they played. Sal is currently undergoing psychological rehabilitation treatment at the prison.

This study explored self-perception of stigma and discrimination among men who have sex with men (MSM) in Kathmandu Valley, Nepal. It found that MSM experience stigma from families, healthcare providers, workplaces, and law enforcement. Discrimination was particularly faced by "Metis", biologically male MSM who see themselves as feminine. Common forms of discrimination included violence, refusal of services, outing of sexual orientation without consent, and being forced into marriage or sex work. However, some MSM reported family acceptance and decreasing discrimination over time, which they attributed to education and advocacy efforts. The study highlights the need to address stigma in order to improve health outcomes for MSM in Nepal.

Gender Dysphoria
Transgender is the term used to mean that individual sex assigned during birth based on the external genital does not fit their gender identity. These kinds of people usually experience gender dysphoria, which is one of the psychological distresses associated with the incongruence between one's gender identity and the sex they were assigned during birth. Gender dysphoria usually starts at the beginning of childhood, but in some cases, individuals may not experience it until after puberty and even much later in their stages of life. People with transgender may pursue different domains of gender affirmation, which includes: surgical affirmation, medical affirmation, legal affirmation, and social affirmation (Lindley, 2020). Not all people with transgender issues will desire to have all these domains of gender affirmation as these are highly individual and personal decisions.
Medical treatment for gender dysphoria
Medical treatments for this condition may include hormone therapy, such as masculinizing hormone therapy or feminizing hormone therapy. Surgery includes masculinizing surgery or feminizing surgery, which intends to change chest or breasts, internal genitalia, external genital, body contouring, and facial features.
Some people used hormone therapy to seek maximum masculinization or feminization. Other people find relief from gender dysphoria by using hormones to maximize secondary sex characteristics such as facial hair and breasts. These kinds of treatments are based on individuals' goals and the evaluation of the benefits and risks of the medication use, presence of any other conditions, and consideration of individual's economic and social issues (Zucker, 2018). Most of these gender dysphoria individuals find surgery the most effective and necessary procedure that relieves their condition.
The World Professional Association for Transgender Health gives the following procedure for the surgical or hormonal treatment for people with gender dysphoria.
1. Persistent and a well-documented gender dysphoria
2. The capacity to make consent and an informed decision for the treatment
3. The aged majority of a given country and when the patient is young need to follow the standard of care for the adolescents and children.
Behavioral health treatment
This treatment aims to improve the individual's psychological well-being, self-fulfillment, and quality of life. This kind of treatment does not aim to alter an individual's gender identity, but it aims to explore gender concerns and find ways to reduce gender dysphoria. The main goal.
