SUBJECTIVE
Patient reports “fantastic” mood. Reports that she sleeps about 5 hours/night to which she adds “I hate sleep, it’s no fun.”
You reviewed her hospital records and find that she has been medically worked up by a physician who reported her to be in overall good health. Lab studies were all within normal limits. You find that the patient had genetic testing in the hospital (specifically GeneSight testing) as none of the medications that they were treating her with seemed to work.
Genetic testing reveals that she is positive for CYP2D6*10 allele.
Patient confesses that she stopped taking her lithium (which was prescribed in the hospital) since she was discharged two weeks ago.
MENTAL STATUS EXAM
The patient is alert, oriented to person, place, time, and event. She is dressed quite oddly- wearing what appears to be an evening gown to her appointment. Speech is rapid, pressured, tangential. Self-reported mood is euthymic. Affect broad. Patient denies visual or auditory hallucinations, no overt delusional or paranoid thought processes readily apparent. Judgment is grossly intact, but insight is clearly impaired. She is currently denying suicidal or homicidal ideation.

LGBT college students in Pennsylvania experience significantly higher rates of mental health conditions, self-harm, and suicidality compared to heterosexual and cisgender peers and the general college population. Over half of LGBT students surveyed had been diagnosed with a mental health condition. Barriers to accessing mental health care included cost, lack of time, and health insurance issues. Minority stress from discrimination, family rejection, and internalized homophobia/transphobia likely contribute to poorer mental health outcomes for LGBT students.

Informatics meshes technology and information into something useful. In nursing, informatics uses media such as health portals, mobile apps, social networking, and telehealth to aide nurses, patients and other stakeholders in decision making. When utilizing informatics our knowledge increases, we communicate better, we become more efficient with our job, and we provide better patient care.
As a hospice nurse, I often admit patients with very little to no medical history. My scenario would be to have a database that shares all health information for patients. If this would be available, decisions could be made that is in the best interest of the patient immediately and not have to wait for information to be sent. When having to wait, it impedes patient care and often has to backtrack and change the plan of care. Many times there is vital medical history missed because the patient/family member is a poor historian due to disease process or stress of the situation. If there is one database for all information potential medication errors could be avoided because you would know information such as allergies, current and previous medications prescribed.
This technology is already available to some extent in the prescription pain medicine world. The prescription drug monitoring program (PDMP) is a database that keeps track of controlled substances being prescribed. This is used in the fight against the opioid epidemic occurring today.
This idea is not new but has not come to fruition yet. If this was to occur, HIPPA would have to be strictly maintained. This could happen by having the patient sign an agreement allowing their information to be in the database and every provider having access to it. Many people travel even state to state and if there was a national database, all health records would be available at the fingertips of the provider no matter where the patient was allowing efficient and knowledgeable patient care.

LGBT college students in Pennsylvania experience significantly higher rates of mental health conditions, self-harm, and suicidality compared to heterosexual and cisgender peers and the general college population. In nursing, informatics uses media such as health portals, mobile apps, social networking, and telehealth to aide nurses, patients and other stakeholders in decision making. When utilizing informatics our knowledge increases, we communicate better, we become more efficient with our job, and we provide better patient care.
As a hospice nurse, I often admit patients with very little to no medical history. My scenario would be to have a database that shares all health information for patients. If this would be available, decisions could be made that is in the best interest of the patient immediately and not have to wait for information to be sent. When having to wait, it impedes patient care and often has to backtrack and change the plan of care. Many times there is vital medical history missed because the patient/family member is a poor historian due to disease process or stress of the situation. If there is one database for all information potential medication errors could be avoided because you would know information such as allergies, current and previous medications prescribed.
This technology is already available to some extent in the prescription pain medicine world. The prescription drug monitoring program (PDMP) is a database that keeps track of controlled substances being prescribed. This is used in the fight against the opioid epidemic occurring today.
This idea is not new but has not come to fruition yet. If this was to occur, HIPPA would have to be strictly maintained. This could happen by having the patient sign an agreement allowing their information to be in the database and every provider having access to it. Many people travel even state to state and if there was a national database, all health records would be available at the fingertips of the provider no matter where the patient was allowing efficient and knowledgeable patient care.

CDHPs are geared to encourage participants to enroll in some type of wellness program and improve their lifestyles. Specific types of CDHPs are health reimbursement accounts (HRA), flexible spending accounts (FSA), and health savings accounts (HSA).
However, there are concerns about CDHPs. The consumer may neither understand nor have the education and the tools to manage his or her own healthcare appropriately. This may have long-term ramifications on the whole healthcare system and whether CDHPs can be successful for the consumer, the employer, the physician, and the healthcare facilities, as well as the insurers. Answer the following questions in regard to this development:
Are CDHPs more geared toward the healthier and younger population?
Are they effective for patients with chronic illnesses?
Will they discourage the use of preventative care and cause increased healthcare costs in the future?

Case Study, Mohr CHAPTER 38, Homeless Clients In completing the case study, students will be addressing the following learning objectives: Discuss factors that contribute to homelessness in people with mental illness. Discuss barriers that prevent homeless people with mental illness from receiving care measures to promote access. Kevin, a 39-year-old unemployed homeless male who has paranoid schizophrenia, was brought to the psychiatric hospital by the police. Citizens called the police because Kevin was in the street directing pedestrians and traffic in opposition to the traffic lights and verbally abusing everyone who did not follow his directions. Kevin is known to the police since he is often homeless, and states that his family does not want him. Kevin also has a history of poly substance abuse with alcohol, heroin, and crack cocaine, and he has been jailed for public intoxication several times. The nursing assessment reveals that Kevin has not been taking his prescribed psychotropic medications for 3 weeks. Kevin states that he does not have any money, and he does not remember where to go for mental health care (Learning Objectives: 2) a. What are the major factors that contribute to Kevin’s frequent homelessness? b. What barriers does Kevin face in the receiving treatment? How can these barriers be addressed?
