Case Study: Frameworks for Nursing Research A group of nurses working in a hospital observed that some patients with chronic renal insufficiency were less cooperative due to being anxious and unable to think clearly. They also noted that these patients were less likely to ambulate and actively participate in their care, unless there was a family member present. Through reviewing the literature, the nurses identified a lack of knowledge about the effects of renal insufficiency on ambulation. The nurses collaborated with an educator with a doctoral degree and developed a proposal for a descriptive, longitudinal study of ambulation and family presence of patients with chronic insufficiency. The framework of the study was synthesized from physiological principles and a theory of family presence and presented as a model. The study was designed to answer the following questions: a. How do renal insufficiency, ambulation, and family presence change over the course of hospitalization? · b. What are the relationships among renal insufficiency, ambulation, and family presence? concern surrounding quality issues in end-of-life
(EOL) nursing home care. In addition, prior
research has failed to emphasize the importance
of state tested nursing aides (STNAs) in
providing this type of care. The purpose of this
study was to examine quality issues in EOL
nursing home care within the context of STNAs’
comfort in providing this care. A convenience
sample of 108 STNAs from four nursing homes
in the Cleveland, Ohio area used PDAs to
provide answers to an audio questionnaire.
Questions included emergent themes from the
literature pertaining to EOL care in nursing
homes, as well as materials from a national
education initiative to improve palliative care.
Findings demonstrated lack of comfort in
discussing death with nursing home residents
and their families and insufficient knowledge
surrounding EOL decisions and certain types
of EOL care. Overall, the level of comfort
providing EOL care was found to be associated
with STNAs’ perceived importance of EOL
care, understanding of hospice, and spiritual
well-being.

On a daily basis, healthcare providers are faced with an array of clinical decisions to be made in an efficient and timely manner. Translating evidence into best practices is one way to achieve this. Without current best evidence, practice is rapidly outdated, often to the detriment of the patient. Evidence based practice is the conscientious use of current best practice in making decisions about patient care (Sackett, Richardson, Rosenberg, & Hayes, 2000). It is important for health care professionals to ask questions about their current clinical practice. In this week’s threaded discussion you will ask that burning question that you ask in your daily care of your patients.
You’ll need to focus on asking the right questions, narrowing the questions to one that is nurse driven and the need for change is evident. The question that you formulate will be the question for your Capstone Project.
The process of reviewing scholarly articles for a change in practice is an important part of the development of any type of research project that can lead to a change in practice. As you are appraising the systematic review and other scholarly articles for your change project, think about areas of the article such as sample size, the population, type of study, discussion and limitations. Critiquing a research article will allow you to evaluate the scientific merit of the study and decide how the results may be useful in practice.

The Effect of Reported Sleep,
Perceived Fatigue, and Sleepiness on
Cognitive Performance in a Sample of
Emergency Nurses

OBJECTIVE: The aim of this study is to explore
the relationship between reported sleep, perceived
fatigue and sleepiness, and cognitive performance.
BACKGROUND: Although evidence suggests that
fatigue and sleepiness affect the provision of care in
inpatient units, there is a lack of research on the sleep
patterns of emergency nurses and the effects of dis-
turbed sleep and fatigue on their cognitive abilities
and susceptibility to medical errors.
METHODS: A quantitative correlational design was
used in this study; in each of 7 different statistical models,
zero-order relationships between predictors and the
dependent variable were examined with appropriate
inferential tests.
RESULTS: Participants reported high levels of
sleepiness and chronic fatigue that impeded full
functioning both at work and at home.
CONCLUSIONS: Although high levels of self-reported
fatigue did not show any effects on cognitive function,
other factors in the environment may contribute to
delayed, missed, or inappropriate care. Further research
is indicated.
Studies of worker fatigue in the military and com-
mercial trucking industries1,2 suggest that long hours,
especially extending into overnight, can be hazardous
in terms of the potential for errors due to sleepiness
and reduced vigilance. Medical providers, both nurses
and physicians, are at risk because of the need for
around-the-clock coverage and the number and type
of decisions they must make in a given time span; in
1989, the Bell Commission Report cited sleep depri-
vation in medical house officers as a major contrib-
utor to the 1984 death of Libby Zion, an 18-year-old
who died at New York Hospital, leading to a restruc-
turing of the hours involved in medical residencies.3
The long (up to 36 hour) shifts of the medical resi-
dents in charge of her care and the consequences of
their fatigue on the decisions that were made were
cited as factors in her death. As a result of their inves-
tigation of the case, the Bell commission recommen-
ded limiting the work hours of medical residents to
less than 80 hours a week and no more than 24 hours
in a row, and subsequent research has led to similar
recommendations for the nursing workforce.4,5
Emergency care settings are chaotic environ-
ments, where there is high patient turnover, constantly
changing priorities and frequent changes in patient

This study evaluated the effects of dance therapy for people with schizophrenia or schizophrenia-like illnesses compared to standard care. A single study with 45 participants found that those receiving dance therapy plus routine care had significantly greater reduction in negative symptoms compared to routine care alone, based on PANSS scores. However, there were no differences between groups in overall PANSS scores, positive symptoms, satisfaction, or quality of life. This case study describes the care of a patient who came into the hospital with sepsis and progressed to acute respiratory and renal failure requiring mechanical ventilation and a tracheostomy. The patient did not respond to any external stimuli except pain. Through researching the patient's conditions and monitoring lab values, the student was able to understand the pathophysiology and appropriately treat the patient's multiple organ dysfunction syndrome. The experience provided an opportunity for the student to learn how to communicate with and provide comfort to patients with limited response abilities.

This is absurd, of course: crime is not a matter of double-entry bookkeeping. You cannot pay a debt by having caused even greater expense, nor can you pay in advance for a bank robbery by offering to serve a prison sentence before you commit it. Perhaps, metaphorically speaking, the slate is wiped clean once a prisoner is released from prison, but the debt is not paid off. It would be just as absurd for me to say, on my imminent retirement after 14 years of my hospital and prison work, that I have paid my debt to society. I had the choice to do something more pleasing if I had wished, and I was paid, if not munificently, at least adequately. I chose the disagreeable neighborhood in which I practiced because, medically speaking, the poor are more interesting, at least to me, than the rich: their pathology is more florid, their need for attention greater. Their dilemmas, if cruder, seem to me more compelling, nearer to the fundamentals of human existence. No doubt I also felt my services would be more valuable there: in other words, that I had some kind of duty to perform. Perhaps for that reason, like the prisoner on his release, I feel I have paid my debt to society. Certainly, the work has taken a toll on me, and it is time to do something else. It would be just as absurd for me to say, on my imminent retirement after 14 years of my hospital and prison work, that I have paid my debt to society. I had the choice to do something more pleasing if I had wished, and I was paid, if not munificently, at least adequately. I chose the disagreeable neighborhood in which I practiced because, medically speaking, the poor are more interesting, at least to me, than the rich: their pathology is more florid, their need for attention greater. Their dilemmas, if cruder, seem to me more compelling, nearer to the fundamentals of human existence. No doubt I also felt my services would be more valuable there: in other words, that I had some kind of duty to perform. Perhaps for that reason, like the prisoner on his release, I feel I have paid my debt to society. Certainly, the work has taken a toll on me, and it is time to do something else. Reveal Topic: A centralized system for strategic resources means that the federal government would represent all individual states and make purchases globally as a single outward-facing entity. Then resources will be coordinated and allocated to individual states.
Credibility: New York State Governor Cuomo states that it is very problematic to keep the current chaotic and unorganized systems. It costs more for taxpayers and could be life-threatening.
Central Idea: US federal government should establish a centralized system for buying, coordinating, and allocating strategic resources such as medical supplies.

FS 1513 Observation Project Details
Time Involved
Students enrolled in FS 1513 are expected to complete at least 15 hours of observations related to human development. This work will involve creating a journal entry for each of their observational sessions. There should be entries for at least 15 hours of observations by the end of the semester.
There should be 1 journal entry for each time you observe—whether it is for 1 hour or more—there should only be 1 entry. So, if you observed 3 times in 1 week for 1 hour each time then you would need 3 journal entries. Alternately, if you observed 1 time for 3 hours then you would need 1 journal entry. However, this means that there will be less information to assess your work (i.e., 1 mistake can cost more points).
Observations: Who and Where
Part of the purpose of this project is to observe differences in development across the life span. So, observations should involve people from different age ranges over the course of the semester. Some entries should focus on development observed during childhood (i.e., observing children from infancy through grade-school), others should involve observing development during adolescence (i.e., observing pubescent and adolescent children), and others should involve should involve observing development during adulthood (i.e., observing adults from early to later in life). The important point is that observations should be completed for at least three age groups for the entire semester.
Observations should take place in naturalistic settings. Naturalistic means that people should be observed in place where they would normally be present (i.e., not in a lab). Ideally, this would mean that they are able to behave as they normally would. The goal, however, is to have enough time to observe a person so that they can demonstrate what they have developed. This means that you will need to be able to watch them behaving naturally for the duration of your observation (i.e., at least 1 hour). Observations that are less than 1 hour will not be counted as complete.
Formatting
Journal entries using the appropriate templates should be completed for each observation session. The templates (available on the course website and below) should contain all of the identifying information. Journal entries that do not use the template will not be graded.
First, each journal entry must contain documentary information. This should cover (1) who did the observations and who was observed, (2) where and when did the observations happen, (3) what were the people being observed generally like, (4) what was the setting like, and (5) keep tr.
