There are numerous changes taking place in South Africa, in the economy, politics and health. All these are interdependent and embedded in a social milieu which brings a number of pressures on health services and systems. The major event in the medium to long term is the impact of the National Health Insurance. Other contextual factors of importance include the range of social determinants of health and disease, with the provision of water, sanitation, electricity and housing being the key services. South Africa will also be influenced in the future by the major diseases it harbours at present. This seminar provided some insight into how these factors will impact on the South African Health Services.

The majority of these jobs will be most prevalent in the industries of hospitals, offices of health practitioners, nursing and residential care facilities, home healthcare services, and outpatient, laboratory, and other ambulatory services (Torpey). Industrial growth within the U.S. healthcare system will be a direct byproduct of macro-trends emerging in the U.S. healthcare system, such as those related to the overall economy, morphing demographics, personal lifestyles and behaviors, emerging technologies, and evolving federal and state government policies. As a result, this growing, heavily diverse healthcare industry will present an inordinate amount of career opportunities for healthcare managers in the next 10 years.
Dobson DaVanzo Structural Changes Drive Health Care Spending Slowdown
If recent trends in health care spending growth continue, Medicare spending could be $2.6 trillion lower than 2008 projections through 2023, representing a 26% savings. Lower projections are due to structural changes in health care delivery that are unprecedented in scale and scope. These include risk-based payments, care coordination, and reduced benefits, which are causing behavioral changes among providers, payers and patients. If structural changes become further entrenched, they could produce long-term reductions in health care cost growth.
Kano State Health Profile - Nigeria

Dobson DaVanzo Structural Changes Drive Health Care Spending Slowdown
If recent trends in health care spending growth continue, Medicare spending could be $2.6 trillion lower than 2008 projections through 2023, representing a 26% savings. Lower projections are due to structural changes in health care delivery that are unprecedented in scale and scope. These include risk-based payments, care coordination, and reduced benefits, which are causing behavioral changes among providers, payers and patients. If structural changes become further entrenched, they could produce long-term reductions in health care cost growth.

The nonprofit organization shares many services with the local government. Health is among the priorities of any government hence it is intimately involved in any activities on health. ASC informs the society about cancer and creates awareness on the necessity of having a cancer-free society (Davidson et al., 2015). Also, the organization partners with companies and other organizations to fund for cancers related activities such as research on best practices in cancer issues. Besides, the organization provides the local government with the human power to engage in delivering different cancer-related activities. ASC is helped the society to reduce cancer deaths considerably in the United States. In enhancing effective communication with external and internal audiences, the organization should apply the following strategies. Firstly, it should raise awareness and also equip employees with information and skills. There should include the formation of a liaison committee to ensure all stakeholders are involved in the program. Lastly, there should be a clear communication plan in and out of the organization.

The creation of community health promotion and education programs takes into consideration several agencies or parties who help in the achievement of the desired health goals. Each of the partners will address its roles using different approaches depending on their area of expertise. This is an important factor to consider as different institutions address health promotion using different approaches and perspectives. The overall outcome from the contribution of every partner should be able to restore and promote the physical, emotional, spiritual, psychological, and social wellness of the community in relation to the health issue being suffered (Minelli, & Breckon, 2009). Chronic diseases are currently the leading causes of death in the community due to their complexity and the severe effects on human health. The community health promotion and education program will be provided by the ‘Health Concerns Coalition’ which will be made up of the following partners; community religious groups, Cancer Supportive Care Foundation, an association of cancer-survivor patients, nutritional organizations, and the local authority.
1. Cancer Supportive Care Foundation – This is an important part of the coalition as it will offer technical expertise in education and diagnosis of chronic diseases. The foundation team will include medical experts who will diagnose the community members of any chronic illnesses. Examinations for diseases such as breast cancer, prostate cancer, diabetes and blood pressure will be conducted by this partner as they will provide modern machines needed for the diagnosis of chronic illnesses.

Cell Therapy Expansion and Challenges in Autoimmune Disease
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
