This document outlines a public education campaign in California to build support for implementation of national health care reform. It discusses polling that shows initial plurality support for reform that grows when people learn about the law's provisions. The campaign's goal is to educate "persuadable" voters like moderates and those who will directly benefit on popular reforms like prohibiting denial of coverage for pre-existing conditions. The document provides messaging guidance and asks organizations to get involved by discussing reform in their communities.

aspects of earlier debates (key arguments, rhetoric, etc.) have shaped current
controversies on this topic?
The United States currently has many healthcare issues that it is facing. These issues
are the main reason why a large number of people in the country face a lot of health
problems. The country is currently spending the largest amount of money on healthcare
compared to other countries. Due to this reason, many people have provided proposals for
policies that can be utilized to solve these health issues and help individuals in the country
attain a healthcare improvement. An example of the many issues that the country is facing
among others includes shortage of doctors, substance abuse, tobacco, overweight, and
obesity. Access to healthcare is a major issue that many individuals in the United States feel
the need to ensure its existence.
Many people in the United States are interested in the issue of access to healthcare.
Without good access, people do not have the ability to ensure that they will remain healthy.
Access to health care is thus a significantly important aspect that people are highly concerned
with. The patient protection and the affordable care act is one of the main policy that has
attempted to address this issue. Through this act, many individuals have managed to attain

Sutherland Institute Utah Medicare Expansion Survey Summary 091514Magellan Strategies
The survey finds that Utah registered voters are reluctant to favor any of the four Medicaid expansion proposals currently being considered by Governor Herbert and the state legislature. Of the four Medicaid expansion proposals that were tested in the survey, no individual proposal received 50% support among respondents. After explaining the details of each proposal in a stand-alone question format, on average 30% of respondents were unsure if they favored or opposed the proposals. Of the four proposals, the “Do Not Expand Medicaid Right Now” proposal had the highest percentage of voters favoring a proposal with 45% and the lowest percentage opposing a proposal at 26%.
The Healthy Utah Proposal was favored by 32% of respondents, opposed by 40% of respondents, and 28% were unsure or did not have an opinion of the proposal. The Traditional Medicaid Expansion and Partial Medicaid Expansion proposals were the least favored proposals by voters, with only 21% and 19% favoring them respectively. A near majority of 49% and 48% of voters oppose the Traditional and Partial Medicaid Expansion proposals, respectively.
After respondents were informed about the details of each proposal and then asked to choose which one they believed was the best proposal that should be implemented, a plurality of 31% chose the “Do Not Expand Medicaid Right Now” proposal. Seventeen percent chose the Healthy Utah Proposal, 15% chose the Traditional Medicaid Expansion Proposal, and 10% chose the Partial Medicaid Expansion Proposal. One in five respondents, or 20%, were unsure or did not know which proposal was the best and should be implemented, and 7% did not like any of the proposals.

The Affordable Care Act

Public Issues Life Cycle
A public issue is any issue, usually broad and encompassing, that collectively generates concern for an organization and any number of its stakeholders (Lawrence & Weber, 2017). The current healthcare law legally titled the Patient Protection and Affordable Care Act (PPACA) is more commonly referred to as Obamacare is undeniably a public issue. For the foreseeable future, it is an issue that has a life cycle tied to the political party that occupies the White House. Therefore, the management process is currently on either a 4 or 8-year cycle depending upon election results. Regardless of which political party is in the majority, the issue facing our nation over healthcare and how to provide for it will still have the same obstacles facing the American public (Stakeholders in this case), that needs to be solved.
Performance Expectations Gap
The emergence over the last few years of legislation that has required Americans to purchase health insurance has driven the expectations of consumers in an upward trajectory. Setting performance expectations on this issue is mostly political unfortunately. Consequently, the gap in performance is judged through a political lens rather than through the lens of the overall public health of our citizens. Performance Expectations Gap can be defined as the difference in stakeholder (citizen) concerns versus what the organization accomplishes. The larger the gap in performance, the greater the backlash from the adversely affected citizens Fischer, K. (n.d.). In the first attempt at requiring healthcare coverage for every American the expectation was set that the nearly 30 million uninsured Americans would receive coverage. However the first year only saw an enrollment number of approximately 9 million which included Americans that were moved from a pre-ACA plan to a compliant ACA plan (Kowalski, 2014). By 2016 these enrollment numbers had risen to 12.1 million Americans (Blumberg & Holahan, 2016). The expectation was for 30 million Americans to be covered for healthcare but the latest verifiable data shows that only about half of those intended to be covered are actually covered. This has created a performance expectations gap for the PPACA.
Stakeholder Engagement
No organization can purposefully plan nor survive without being attentive to its stakeholders and responding to their legitimate needs and concerns. Stakeholders are constituents with a vested interest in the affairs and actions of the organization. They are individuals, groups, or organizations affected by the organization and who may seek to influence it (Rakich, & Feit, 2001). Therefore, going forward, the leadership of the United States (our organization) must be attentive to what works in the healthcare industry if th ...

Advance Care Planning and Analysis
Democrats’ Plans for Healthcare
I totally agree with Sarah Palin’s sentiments on the Democrats plan for healthcare. I think what the Democrats are pushing for claiming to reduce healthcare is downright discriminatory and it will do more harm than good in the long run. The truth of the matter is, some people need more care than others. For a fact, such a system will not favor vulnerable populations like those with chronic illness, the elderly and the disabled. There are currently more than half a million people in America living with various forms of disabilities. Such people continue to remain in waiting lists to receive services due to the fact that Obamacare urges states to expand Medicaid coverage to able-bodied adults instead of prioritizing the services to those who need them the most (Hilberg, 2015).
My judgment is informed by a commitment to advance care of planning. For instance, recent estimates from the Urban Institute suggest that a big percentage of the individuals eligible to Obamacare Medicaid expansion are adults without children in their prime working years. This means that Obamacare does not provide medical services to the most vulnerable. Another report published by the Foundation for Government Accountability showcases how Medicaid rolls have erupted under Obamacare and also how individuals with disabilities have suffered owing to this.
The Role of Nurses in this Plight
It is important to note that nurses are always in the frontline in boardrooms and in leading healthcare organizations. Given their high level of education and their experience, they have the intelligence to observe, validate and the network to transmit messages that advocate for healthcare reform values. Taking this into consideration, it is not right for them to endorse a legislation even when it is not perfect. If they leverage their numbers and use their influence efficiently, they will be able to support only the legislative changes that strengthen the system instead of those that weaken it (Béland, Rocco & Waddan, 2016).
The above also applies to the recently failed American Care Act. They must rigorously oppose legislation that could undermine or compromise the goals of the healthcare system. Nurses must, therefore, make an extra effort to communicate, listen and collaborate with the new congressional leaders. I believe that by putting their voices together, they can become part of the solution when it comes to directing policy through political goodwill (Béland, Rocco & Waddan, 2016).

In a May 9, 2024 paper, Juri Opitz from the University of Zurich, along with Shira Wein and Nathan Schneider form Georgetown University, discussed the importance of linguistic expertise in natural language processing (NLP) in an era dominated by large language models (LLMs).
The authors explained that while machine translation (MT) previously relied heavily on linguists, the landscape has shifted. “Linguistics is no longer front and center in the way we build NLP systems,” they said. With the emergence of LLMs, which can generate fluent text without the need for specialized modules to handle grammar or semantic coherence, the need for linguistic expertise in NLP is being questioned.

Editor's Notes
Californians learned about health reform through the news and it influenced their views in a very partisan way.
To see how Californians feel about reform, the CA Endowment completed a poll in mid April. The poll was conducted by Fairbank, Maslin, Maullin, Metz & Associates (FM3). The survey sample was 800 voters with an oversample of 200 Decline to State Voters.
Now let’s look at who moves to support after hearing more information and examine why they move. There are two basic categories of positive movers:Those without a strong ideological predisposition to support or oppose reform – notably independents and moderates. Those with some specific likelihood to benefit from reform once they understand its provisions – including voters with pre-existing conditions, those who lack coverage, seniors, and some communities of color.
In the survey, people were read a list of 19 provisions of the new health law in 4 categories, those that relate to expanding access to services, cost & quality of care, insurance company reform, and how health reform will be funded.
These are worded in a neutral way, simply explaining what the law does..these are the top rated provisions that received over 60% support.
Messages stressing the broad distribution of benefits and insurance company reforms are generally the most compelling. The most important provisions to people depend on their circumstances but it’s also good to let people know about the ones that are most popular so that they get repeated..the key to marketing.
What do we want to do?This moment is unique; when was the last time we had such a major reform? There is a wide base of supporters – and the potential for many moreHealth care has now moved from a cloudy problem to a political issueHealth reform will touch everyone. Potential to create a strong constituency empowered to push for more and better health care.Health that expands access to services – has better quality of care – and honestly invests in prevention. Our goal for public education: Leverage this unique moment of health care reform to build a broader and stronger constituency for health, committed to building a health system that improves access and quality of care, and invests in prevention.
With the passage of health reform, this high profile moment presents a key opportunity to establish a stronger base of support for health issues and raise the bar for consumers’ expectations of their health care. The key to better healthcare is a more empowered consumer who understands what they have and begins to make demands for higher quality and better value. To change the culture from consumer passivity to active involvement begins with an understanding of the new law. That understanding will be beneficial for all that we want to do. And as implementation rolls out, public education must continue. This year is critical because there are many who want to weaken, repeal and replace the law and we need to meet them and beat them in their attempts to take these benefits away from people. If reform becomes known to people as coverage and benefits rather than framed as a mandate and taxes—each step of the way will become easier for reformers.Empowered consumerscan create a game-changing new direction for health in our state.
What can you/your organization do to support health care reform in California?
Let’s talk about how you/your organization would like to participate. Who are you constituents or members? How would you approach them?
