The Department of Education adopts and utilizes modified school forms effective at the end of the 2013-2014 school year. The modified forms underwent review and consultation and align with the K-12 program. The forms replace 16 previous forms and reduce the focus on administrative tasks, allowing teachers to focus on curriculum delivery. By the 2014-2015 school year, all public schools must use the seven new modified school forms which include information on enrollment, attendance, promotions and personnel assignment. The forms are available online and must contain consistent data with the Learner Information System and Enhanced Basic Education Information System.
