MODULE 3- ACCIDENTS
SAFE COMPANY
ACCIDENTS AND HOW TO
AVOID THEM
An "accident at work" resulting in physical injury. Near miss: neither
injury nor damage.
• Causes: unsafe actions (80% of accidents), which are conscious
employees´ actions that create new risks, and unsafe situations
(20%) are situation in which risks are not under control and that can
lead to an accident
• Corporate culture often plays a major role
• Accidents involving minor injuries occur ten times more frequently
than serious accidents, but these are generally not reported.
Investigation needed.
• Accident pyramid: many minor incidents and less obvious risk
situations ultimately lead to serious accidents.
• Human failure (inattention, carelessness, a lack of experience or
inadequate training) can lead to unsafe actions or situations.
PREVENTION
Following factors have be taken into consideration:
• Employer obligations: working methods, supervision, a good work
description and clear instructions.
• Machinery: safe, covers, guards and other types of protection in
place. User manual. Well-maintained.
• The environment also influences (pressure of work, environmental
noise, temperature, smells or dust and blinding light).
• Immediate reaction in case of an unsafe situation or if someone
undertakes a dangerous action.
ACCIDENT PREVENTION
POLICY
An accident prevention policy contains:
• Features: proactive, all levels of a company, integrated within the
company's general policy.
• Management; responsibility in drawing up and implementing a
prevention policy.
• Delegate tasks and authority, training and instruction and structure
for consultation. Ensuring that the existing rules and regulations are
observed out (regular inspections.)
• Necessary information, report and investigate accidents and perform
evaluations after obstacles arise.
ACCIDENTS: PROCEDURE
Following procedures must take place:
• Every accident must be reported to the supervisor (even minor
accidents and near misses).
• In case of an injury: take care not to endanger yourself and make
sure no one else gets injured.
• In case of serious injury: alert immediately your direct superior and
emergency services.
• If an accident occurs: record it always! Accidents involving injury or
potential claims may need to be notified (national requirements) and
a detailed report.
ACCIDENTS: PROCEDURE II
Following procedures must take place:
• All accidents must be investigated to determine the causes.
Investigation: as soon as possible, with the people involved and at
the site of the accident. Objectively. Interview witnesses.
• Analyzing the results. Final report.
• Appropriate measures to avoid similar accidents in the future.

  • 2. ACCIDENTS AND HOW TO AVOID THEM An "accident at work" resulting in physical injury. Near miss: neither injury nor damage. • Causes: unsafe actions (80% of accidents), which are conscious employees´ actions that create new risks, and unsafe situations (20%) are situation in which risks are not under control and that can lead to an accident • Corporate culture often plays a major role • Accidents involving minor injuries occur ten times more frequently than serious accidents, but these are generally not reported. Investigation needed. • Accident pyramid: many minor incidents and less obvious risk situations ultimately lead to serious accidents. • Human failure (inattention, carelessness, a lack of experience or inadequate training) can lead to unsafe actions or situations.
  • 3. PREVENTION Following factors have be taken into consideration: • Employer obligations: working methods, supervision, a good work description and clear instructions. • Machinery: safe, covers, guards and other types of protection in place. User manual. Well-maintained. • The environment also influences (pressure of work, environmental noise, temperature, smells or dust and blinding light). • Immediate reaction in case of an unsafe situation or if someone undertakes a dangerous action.
  • 4. ACCIDENT PREVENTION POLICY An accident prevention policy contains: • Features: proactive, all levels of a company, integrated within the company's general policy. • Management; responsibility in drawing up and implementing a prevention policy. • Delegate tasks and authority, training and instruction and structure for consultation. Ensuring that the existing rules and regulations are observed out (regular inspections.) • Necessary information, report and investigate accidents and perform evaluations after obstacles arise.
  • 5. ACCIDENTS: PROCEDURE Following procedures must take place: • Every accident must be reported to the supervisor (even minor accidents and near misses). • In case of an injury: take care not to endanger yourself and make sure no one else gets injured. • In case of serious injury: alert immediately your direct superior and emergency services. • If an accident occurs: record it always! Accidents involving injury or potential claims may need to be notified (national requirements) and a detailed report.
  • 6. ACCIDENTS: PROCEDURE II Following procedures must take place: • All accidents must be investigated to determine the causes. Investigation: as soon as possible, with the people involved and at the site of the accident. Objectively. Interview witnesses. • Analyzing the results. Final report. • Appropriate measures to avoid similar accidents in the future.
