The document summarizes the key findings from a benefits realization study conducted by PwC on the use of electronic medical records (EMRs) in primary care settings in Ontario. Through case studies of six high performing clinics, the study found benefits such as 50% faster lab result turnaround times, nearly immediate access to discharge summaries, and referrals sent to specialists in under 1 day. Provider surveys showed strong agreement that EMRs improve areas like chronic disease management, preventative care, and practice efficiencies. The study modeled potential province-wide benefits if all Ontario providers achieved results similar to the case studies, estimating annual financial benefits of $125 million from improved diabetes management alone.
This document discusses the use of electronic diaries (eDiaries) and connected health technologies in clinical trials. It summarizes a 2011 study that found automated email alerts sent to clinicians when cancer patients reported severe symptoms from a mobile eDiary led to an 11% decrease in severe symptoms and improved symptom control compared to a control group without alerts. The study also found high satisfaction rates among clinicians and patients with the automated alert system. It further notes that compliance with eDiary completion was higher when clinicians more frequently reviewed patient-reported data.

This study evaluated the effectiveness of a community-based program for managing severe acute malnutrition (SAM) in Bangladesh. 261 community health workers were trained to screen, assess, and treat children aged 6-36 months with SAM. Preliminary results found high recovery rates and low mortality. Early identification and treatment of SAM and illness likely contributed to the positive outcomes. Coverage was also high due to the decentralized network of health workers and community mobilization around SAM. While facility-based care had low uptake and high default rates, community-based management integrating outpatient and inpatient care was an effective strategy for ensuring treatment of SAM cases.

The pathway was adapted from a Canadian hospital and implemented in 2011. Key results included an increase in patient satisfaction from 56% to 77% and a reduction in average length of stay from 7.94 to 5.78 days over 45 months. Compliance with best practices like surgical site marking and DVT prophylaxis also improved. The pathway was evaluated using a standardized framework and found to improve clinical, service, team, process and financial outcomes for total knee replacement patients.
This document discusses scenario-building methods to define the future of clinical evidence requirements in the US drug development and healthcare systems. It identifies key factors like integration of health systems, use of big data, and patient roles that could impact clinical evidence requirements. Three potential scenarios are described ranging from a status quo system to a fully integrated system. The "moderate integration" scenario is deemed most likely, featuring some increased system integration, standardized electronic health records, growing big data opportunities, and more organized patient groups. Accountable care organizations are also examined, finding they may evaluate drug effectiveness and cost-effectiveness as risk-based payments increase and care pathways are established.
The document reports on lessons learned from implementing a self-administered treatment (SAT) pilot program for drug-resistant tuberculosis (DR-TB) patients in primary health care facilities in Khayelitsha, South Africa, which allowed clinically stable patients to self-administer treatment at home with community support. It details the SAT process, human resource requirements, numbers of patients enrolled and outcomes to date, highlighting successes like improved patient support and high treatment success rates but also challenges of sustained implementation across clinics. The SAT model aims to reduce barriers to adherence and burden on the healthcare system posed by the standard directly observed therapy model for DR-TB patients.
This document discusses remote patient monitoring and how it can help improve patient care while reducing costs. It notes that the remote patient monitoring market is estimated to grow 44% annually and that remote monitoring has been shown to decrease emergency admissions in the UK by 20%. It then describes a proprietary big data technology solution that analyzes digital patient data from remote monitoring devices to provide deeper insights that help doctors and case managers improve decision making and care for patients.

Media Alert: Motus GI® Holdings, Inc. Presents Positive Clinical Data from Pe...Carly Capko
Data show Pure-Vu System significantly increased the number of subjects with an adequate cleansing level to 100% from 19.1% at baseline after use of the Pure-Vu System
Pure-Vu System has potential to improve clinical outcomes while reducing costs and complications associated with conventional purgative-based bowel preps in a market encompassing over 30 million annual procedures worldwide.
This study aimed to identify predictors of 30-day readmission in patients undergoing surgery for gynecologic malignancies. The overall 30-day readmission rate was 7.4%. Variables predicting readmission included experiencing a complication prior to discharge, having widespread cancer, and undergoing a non-laparoscopic surgery. A readmission prediction score was developed based on these factors and validated, identifying patients at high (>15%) risk of readmission. The study concludes this scoring system can help target high-risk patients for interventions to prevent readmissions and reduce healthcare costs.

Antimicrobial stewardship programs (ASP) are an essential practice to prevent increasing
resistance against antibiotics. A successful ASP monitors not only prescribing patterns and
practices but also contributes in minimizing the toxic effects of antibiotics. Moreover, ASP
also facilitates the selection of disease specific antibiotics and enforces rules and regulations to rationalize the use of antibiotics. The aim of the study is to highlight the core
elements of Hospital Antibiotic Stewardship Programs in Karachi. The key elements proposed by center of disease control (CDC) such as; leadership, accountability, drug
expertise, actions to support optimal antibiotic use, tracking (monitoring antibiotic prescribing, use and resistance), reporting information to staff on improving antibiotic use
and resistance and education were evaluated on Yes/No scale. The data was collected
from 44 hospitals of different categories in Karachi and all the major elements were
studied. It was observed that all the hospitals in one setting failed to comply with all the
guidelines. It has been concluded that efforts should be made to design ASP at each
hospital and implemented through suitable policies and procedures.
The document outlines proposed meaningful use criteria and objectives for 2011 that focus on improving quality, safety, efficiency, engaging patients and families, improving care coordination, improving population and public health, and ensuring privacy and security protections. It provides details on specific objectives and associated measures within each category. The summary recommends having a clear understanding of the meaningful use requirements and vendor solutions to meet them.
1. The document proposes a screening program supplier to help address the growing issue of cancer in developed nations.
2. The proposed solution involves establishing a screening program that uses technological solutions from Siemens to provide early detection of cancers like breast and lung cancer.

New technologies have the potential to improve clinical outcomes in toxicology and poisoning situations. The literature review identified several technologies that could help, such as expert systems to manage emergency drug stocks and antidote programs. Studies found that most hospitals have inadequate stocks of important antidotes. Guidelines and collaboration between hospitals, including formal stock sharing agreements, could help address shortages. The implementation of a computerized pharmacy cabinet system in one hospital reduced costs by 75,000 euros over 5 years while still covering 99% of emergency needs. New technologies, if properly managed by clinical staff, may lead to more effective antidote availability and improved patient outcomes in toxicology emergencies.

2) Patients will be randomized into two groups - a control group receiving conventional in-hospital rehabilitation and an intervention group using a mobile app and telemonitoring for 10 months of at-home rehabilitation.
3) The primary outcome is adherence to exercise measured by questionnaires and exercise tests, with secondary outcomes of cardiovascular risk factor control, lifestyle changes, and cost analysis.
This document discusses health challenges related to demographic changes and well-being under the Horizon 2020 program. It provides an overview of the Health challenge including priorities to promote active and healthy aging, a whole life approach, scaling up research, and reducing health inequalities. The document reviews funding rules and evaluation criteria for Horizon 2020. It also provides details on specific calls within the 2016-2017 work program related to personalized medicine, including calls on population cohorts, ICT solutions for aging, robotics, virtual coaching, and in-silico trials.
This document discusses the need for a paradigm shift in using information and communication technologies (ICT) and health information technology (HIT) to improve population health outcomes. It argues that current approaches focusing primarily on improving healthcare delivery have failed to address the "tsunami" of preventable poor health affecting many countries. The document proposes leveraging crowdsourced health data from individuals and communities through non-invasive sensors and other means. Combined with advanced modeling and nudging technologies, this could enable more predictive, preventive, and evidence-based public health policy approaches. The goal is to optimize human performance and health at the population level through ICT-enabled co-production of scientific knowledge, rather than just treating diseases within the existing healthcare system.
1. The study evaluated the efficacy of an adherence strategy using a computer alert system and telephone support to improve adherence to antiretroviral therapy (ART).
2. The rates of dispensed ART, which indirectly indicate adherence, significantly increased from 2006 to 2009 after implementing the intervention. Viral load levels also significantly increased compared to baseline.
3. A computer system was designed to alert staff of delays in medication pickup and trained personnel contacted patients by telephone. This centralized monitoring combined with communication support improved long-term adherence and clinical outcomes.

Delirium Care Pathway MoDelirium Care Pathway Model Design: STOP DELIRIUMdel ...komalicarol
We present a delirium care pathway model that we have dubbed
STOP DELIRIUM. Due to delirium's magnitude and effect in elderly hospitalized patients, we recommend hospitals must have
a delirium care pathway for early identification, prevention, and
delirium management. The protocol STOP DELIRIUM is driven
from evidence-based guidelines to help establish the aim "STOP"
for Spot, Think, Optimize and Prevent delirium. The clinical
pathway model needs to incorporate a clinical information management system and educational materials to increase delirium
awareness. The implementation should be scalable and adaptable
to incorporate other departments.
Diabetes Prediction by Supervised and Unsupervised Approaches with Feature Se...IJARIIT
Two approaches to building models for prediction of the onset of Type diabetes mellitus in juvenile subjects were examined. A set of tests performed immediately before diagnosis was used to build classifiers to predict whether the subject would be diagnosed with juvenile diabetes. A modified training set consisting of differences between test results taken at different times was also used to build classifiers to predict whether a subject would be diagnosed with juvenile diabetes. Supervised were compared with decision trees and unsupervised of both types of classifiers. In this study, the system and the test most likely to confirm a diagnosis based on the pre-test probability computed from the patient's information including symptoms and the results of previous tests. If the patient's disease post-test probability is higher than the treatment threshold, a diagnostic decision will be made, and vice versa. Otherwise, the patient needs more tests to help make a decision. The system will then recommend the next optimal test and repeat the same process. In this thesis find out which approach is better on diabetes dataset in weka framework. Also use feature selection techniques which reduce the features and complexities of process

MAST: a model for HTA-based assessment of telemedicine applicationsHTAi Bilbao 2012
This document describes MAST (Model for Assessment of Telemedicine), a multidisciplinary framework for assessing telemedicine applications. It was developed through a comprehensive process and aims to describe effectiveness and quality of care impacts of telemedicine. MAST consists of 3 stages: setting context, multidisciplinary assessment across clinical, economic, organizational and social domains, and assessing transferability. It is being empirically tested through large European projects and has supported over 25 trials. While time-consuming, MAST provides a standardized approach and ensures comparability. Feedback has been positive and several regions are adopting it, with revisions planned based on empirical results to establish MAST as a widely accepted methodology.
Clinical Trial Results Transparency and the Changing Paradigm of Medical Comm...ClinTrialsClick
This document discusses how clinical trial transparency requirements are challenging the traditional model of medical communications. It outlines how regulations have evolved over time to require registration and public reporting of clinical trial results. Two examples are given showing how earlier disclosure of trial results on platforms like ClinicalTrials.gov impacted the traditional publication process. The document argues that empowering end users to access and understand complex clinical trial data requires making results more findable, navigable and understandable beyond current formats. It proposes a model of a "Gout Resource Center" to centrally organize gout trial results in a standardized, searchable format with user-friendly tools.
Structure and development of a clinical decision support system: application ...komalicarol
Clinical decision requires to infer great, diverse and not suitably
organized quantity of information and having low time to decide.
The therapeutic choice is fundamental to formulate a strategy to
avoid complications and to achieve favorable results, being more
important in some specialties. In addition, medical decision-makers are overloaded with clinical tasks, have an intense work rate and
are subject to a great demand, and are prone to greater tiredness.
In this sense, computer tools can be extremely useful, as can deal
with a lot of information in much less time than decision-makers.
Thus, the existence of a tool that assists them in decision-making
is of crucial importance

The document discusses challenges in evaluating the effectiveness of diagnostic tests and linking test results to patient outcomes. It notes that tests provide information to guide therapeutic decisions, so test effects are realized in the context of available therapies. Randomized trials of tests alone are difficult to design, but trials can compare strategies incorporating tests. Observational studies using large databases can also assess tests, but have limitations like lacking test findings and clinical context. The National Oncology PET Registry prospectively collected real-world data on how PET impacts management plans and found changes in 36.5% of cases. However, it did not evaluate if changes improved outcomes. Combining registries with claims data can provide more longitudinal information but also has weaknesses. Evaluating newer diagnostics
This document discusses methods for exploring causality using big health data and addressing potential biases. It covers some common biases like regression to the mean, Simpson's paradox, and confounding by indication. Emerging methods discussed to overcome biases include case-only study designs, regression discontinuity designs, Mendelian randomization, and instrumental variable techniques. The document also addresses the challenges of using routine healthcare data and when reliable causality assessment is most needed.

This document discusses a study that used multiple correspondence analysis to help identify health inequities across regions in Portugal. The study analyzed linked electronic health records of adults admitted to hospitals for acute myocardial infarction between 2012-2013. It found disparities between regions in recording health indicators like blood pressure and cholesterol. Regions with higher rates of no recording for indicators also had higher mortality rates. The multivariate analysis generated visual outputs that clearly showed regional inequities in mortality and quality of electronic health records, helping pinpoint areas that need policy action.

LAND USE LAND COVER AND NDVI OF MIRZAPUR DISTRICT, UPRAHUL
This Dissertation explores the particular circumstances of Mirzapur, a region located in the
core of India. Mirzapur, with its varied terrains and abundant biodiversity, offers an optimal
environment for investigating the changes in vegetation cover dynamics. Our study utilizes
advanced technologies such as GIS (Geographic Information Systems) and Remote sensing to
analyze the transformations that have taken place over the course of a decade.
The complex relationship between human activities and the environment has been the focus
of extensive research and worry. As the global community grapples with swift urbanization,
population expansion, and economic progress, the effects on natural ecosystems are becoming
more evident. A crucial element of this impact is the alteration of vegetation cover, which plays a
significant role in maintaining the ecological equilibrium of our planet.Land serves as the foundation for all human activities and provides the necessary materials for
these activities. As the most crucial natural resource, its utilization by humans results in different
'Land uses,' which are determined by both human activities and the physical characteristics of the
land.
The utilization of land is impacted by human needs and environmental factors. In countries
like India, rapid population growth and the emphasis on extensive resource exploitation can lead
to significant land degradation, adversely affecting the region's land cover.
Therefore, human intervention has significantly influenced land use patterns over many
centuries, evolving its structure over time and space. In the present era, these changes have
accelerated due to factors such as agriculture and urbanization. Information regarding land use and
cover is essential for various planning and management tasks related to the Earth's surface,
providing crucial environmental data for scientific, resource management, policy purposes, and
diverse human activities.
Accurate understanding of land use and cover is imperative for the development planning
of any area. Consequently, a wide range of professionals, including earth system scientists, land
and water managers, and urban planners, are interested in obtaining data on land use and cover
changes, conversion trends, and other related patterns. The spatial dimensions of land use and
cover support policymakers and scientists in making well-informed decisions, as alterations in
these patterns indicate shifts in economic and social conditions. Monitoring such changes with the
help of Advanced technologies like Remote Sensing and Geographic Information Systems is
crucial for coordinated efforts across different administrative levels. Advanced technologies like
Remote Sensing and Geographic Information Systems
