1. Covid was the most collaborative care that come to my mind. Covid came and changed the world and healthcare as we had known it to be forever. Having to provide care to patients that were sick with a disease we knew little about. Starting of Covid it combined every level of healthcare professionals to care for the sick patients. That includes nurses, cna, doctors, dietary, environmental services, spiritual support and family. The care was coordinated as a team. The doctors made the ultimate decisions nurses and cna's did the hard part because the doctors didn't want to go into the rooms to access the patients. The nurses role was heavy. They had to stay up to date on covid numbers, l PPE, crash carts ready, staff properly wearing the protective gear needed, assessing patients, consulting with doctors, respiratory, deal with family and most of all scared patients. We one patient that was a frequent flyer he would come at lease twice a week stay a day and leave AMA. He suffered mental illness. Sadly, he was one that was very friendly. Would always come and make sure he spoke to us all. He was one of the first covid patients to come to the icu. We all worked together to do everything we could. He would spike fevers of 105 and higher, sweating, crying from the pain in his muscles. We as a team was still learning what covid was itself. So we all came together and agreed that we will take turns accessing him. He coded three times over the course of a week. We were down and devastated. We came together and bounced ideas of interventions that can ease the pain. The goal was keep the fever down and monitor very closely. Well we didn't think he would make it but he did. I wouldn't have done anything differently because that showed us that all levels of healthcare are important when making a decision on patient centered care. .

As far as she could tell, everything looked normal. The patient, an avid runner, had been on bed rest for several weeks due to the DVT. He was not to see the specialist until next week, so he and his wife discussed the fact that the woman who gave them the reports interpreted them as normal, and decided it was OK to go running. One mile into the run, the patient developed severe chest pain and shortness of breath. The wife called the ambulance and they transported him to the emergency room at Quinbery General Hospital. Soon after arrival, Bob was pronounced dead. Autopsy showed death due to an embolism. When the ER physician, Dr. Connie Monday, questioned the wife about what happened, she explained that the employee who provided copies of his records told them that the tests appeared normal. Dr. Monday explained that the person who gave them the records was not properly trained or licensed to determine or interpret the test results and they should have waited for the specialist to review the reports. Why is the loss associated with illness especially difficult? Which losses are common with a chronic pain condition? How can we build resilience? These are some of the questions addressed by Deirdre Ryan in this presentation. A loss can be anything related to a person’s life, such as relationships, their role in life, privacy, identity, health, dignity and so on. In learning to live with lingering loss, people frequently move from holding on to and dwelling on that part of themselves which has been lost to letting go. Letting go – rethinking and replanning your life – can be an essential component of grieving. While loss associated with chronic pain can be exhausting and involves continual adjustment and re-adjustment, building emotional resilience can help people deal with their loss. Strategies for building emotional resilience include making connections, looking for self-discovery, being realistic, taking decisions, accepting changes and nurturing a positive view of self. To help deal with lingering loss, Deirdre suggests having compassion for yourself, making small incremental changes, having a plan, avoiding boom-bust and connecting with nature or others. Deirdre Ryan is Chairperson of Chronic pain Ireland and a pre-accredited psychotherapist.

Tonya and Lydia are two senior nursing students assigned to work in the intensive care unit with a critically ill patient. The patient, Mr. Dunn, is an 87-year-old retired ironworker. She thinks about what his quality of life will be after the surgery. She worries about his ability to care for himself and about his state of mind should he be forced to live in a nursing home. She thinks about what she would want for her own father if he were in the same situation. Lydia falls asleep at night pondering these thoughts. She does not know how to go about solving the problem. In psychiatric care, these relationships are often the only way to develop a continuum of care.
