Simon Duffy spoke to disabled people, families, professionals and policy makers in Finland at the beginning of 2016. he explored the potential benefits of introducing personal budgets to the health and social care reforms in Finland [SOTE] and what has been learned from 50 years of international experience.

5. We will talk about what we think are the best international examples and

oikeus josta henkilö on vastuussa
It is a different way of thinking about
and organising support - support is not a gift
but a right that the person controls
9.
10. People take control and so can become full
citizens playing and active part in community life
Ihmiset ottavat vastuuta ja siten voivat toimia täysivaltaisina
kansalaisina. Heillä on aktiivinen rooli omassa yhteisössään.
11. • Today in Finland there are
going to be big changes in
the organisation of health
and social care.
• The policy is to increase
choice for citizens.
• But there are different
ways of giving people
choice.
• Some choices are better
than others.
• Personal budgeting
should be a big part of the
future system.
• Suomessa ollaan
toteuttamassa suurta
sosiaali- ja terveyshuollon
palvelurakennusuudistusta.
• Tavoitteena on kasvattaa
ihmisten vapautta valita.
• On kuitenkin erilaisia tapoja
antaa ihmisille
valinnanvapaus.
• Jotkut vaihtoehdot ovat
parempia kuin toiset.
• Henkilökohtaisen
budjetoinnin mallin tulisi olla
suuresti esillä sote-
uudistuksesta puhuttaessa.

13. 1. How to agree the amount
of the personal budget
2. How to help people make
plans
3. How to enable people to
take control
4. How to learn and innovate
5. How to change things
Henkilökohtaisen budjetoinnin järjestelmän luomisessa on monia
tärkeitä huomioitavia asioita:
We explore some of these matters in detail in the sections below
1. Miten muodostaa ja sopia
henkilökohtainen budjetti
2. Miten auttaa ihmisiä
tekemään suunnitelmia
3. Miten mahdollistaa ihmisille
vastuunotto
4. Miten oppia ja kehittää uutta
5. Miten muuttaa asioita
There are many important details to consider when designing a
system of personal budgeting these include:
Tutkimme näitä yksityiskohtia tarkemmin myöhemmissä
osioissa.

the person
• Article 19 - Living independently
and being included in the
community
• Article 23 - Respect for home and
the family
• Article 24 - Education
• Article 27 - Work and employment

inviolability of human dignity and the freedom
and rights of the individual and promote justice

17. 7 Keys to Citizenship
7 avainta kansalaisuuteen
20. There have been lots of
reports in different
countries showing that
personal budgets (in
many different forms)
leads to positive
improvements in people’s
lives. There are no
reports of it harming
people or making things
worse.
Moni raportti eri maista
on osoittanut, että
henkilökohtainen
budjetointi (monenlaisina
eri järjestelminä) johtaa
positiivisiin muutoksiin
ihmisten elämissä.
Mikään raportti ei ole
osoittanut sen olevan
vahingollista tai tekevän
asioita huonommiksi.
21. For example, in England,
In Control helped people
with intellectual disabilities
to get personal budgets. It
found that these budgets
really helped people to
advance their citizenship.
Esimerkiksi Englannissa In
Control avusti
kehitysvammaisia ihmisiä
aloittamaan
henkilökohtaisen
budjetoinnin. Havaittiin,
että henkilökohtainen
budjetointi auttoi ihmisiä
edistämään
kansalaisuuttaan.
22.
23. In recent research a
service provider
converted a contract for
services into personal
budgets for people with
intellectual disabilities
and saw big
improvements.
Viimeaikaisessa
tutkimuksessa
palveluntuottaja muutti
kehitysvammaisten
henkilöiden
palvelusopimukset
henkilökohtaisiksi
budjeteiksi ja saavutti
suuria parannuksia.
24.
25. Personal budgets also
seem to make people
safer. Research has
shown that people feel
safer and that there is
less abuse. This can be
compared with
significant risk in more
institutional services.
Henkilökohtaiset budjetit
vaikuttavat myös
turvallisuuteen. Tutkimus
osoittaa, että ihmiset
tuntevat olonsa
turvallisemmaksi ja
kokevat vähemmän
hyväksikäyttöä
verrattuna laitosmaisissa
olosuhteissa esiintyviin
merkittäviin riskeihin.

had harmed themselves
• 21.0% of inpatients had suffered an
accident in the last 3 months
• 22.2% of people had suffered physical
assault in the last 3 months
• Physical restraint had been used 34.2% of
people in the last 3 months 11.4% had
suffered seclusion in the last 3 months
• 56.6% of people had been the subject of at
least one incident involving self harm, an
accident, physical assault against them,
hands-on restraint or seclusion during the
last three months
• Antipsychotic medication used regularly or
at least once in the last 28 days for 68.3%
of the people in the units
Data from a recent UK Government report on institutional
services for people with intellectual disabilities
• Viimeisen 3 kuukauden aikana 25,5 %
asukkaista oli vahingoittanut itseään
• 21,0 % asukkaista oli kohdannut tapaturman
viimeisten 3 kuukauden aikana
• 22,2 % asukkaista joutunut fyysisen väkivallan
kohteeksi viimeisten 3 kuukauden aikana
• Fyysistä rajoittamista oli käytetty 34,2%
ihmisistä viimeisten 3 kuukauden aikana. 11,4
% ihmisistä oli pidetty eristyksissä viimeisten 3
kuukauden aikana.
• 56,6 %:lle ihmisistä tapahtui viimeisten 3
kuukauden vähintään yksi seuraavista
tapauksista: itsensä vahingoittaminen,
tapaturma, fyysisen väkivallan kohtaaminen,
rajoittamistoimenpiteiden kohteeksi
joutuminen tai eristäminen
• 68,3 % asukkaista käytti säännöllisesti tai
vähintään kerran antipsykoottista lääkitystä
viimeisen 28 päivän aikana.

30. When people have
control they spend
more of their budget on
being part of ordinary
life and getting support
from people they trust
and want to spend time
with.
Kun ihmisillä on
vapautta ja vastuuta, he
käyttävät budjettiaan
tavanomaisiin asioihin
elämässä ja saavat
tukea henkilöiltä joihin
he luottavat ja joiden
kanssa he haluavat
viettää aikaa.
31. If budgets can be used flexibly then people use money differently and
connect to community
Jos budjettia voi käyttää joustavasti, silloin ihmiset käyttävät rahaa eri
tavoin ja kuuluakseen yhteisöön.
32. Here are 2 examples
of how things can be
much better for
people when people
have more control
and flexibility and
support is designed
around the person’s
needs.
Tässä on kaksi
esimerkkiä, miten
asiat voivat olla paljon
paremmin kun
ihmisillä on enemmän
vastuuta, joustavuutta
ja yksilöllisesti
suunniteltua tukea.
36. All systems of personal
budgets seem to
improve people’s lives.
However systems vary in
how much they cost and
the impact they have on
costs over time. What
really matter is how the
system is designed.
Kaikki henkilökohtaisen
budjetoinnin järjestämisen
tavat näyttävät parantavan
ihmisten elämää. On
kuitenkin eri asia, kuinka
paljon ne maksavat ja mitkä
ovat niiden vaikutukset
kuluihin ajan kuluessa.
Merkittävää on, kuinka
henkilökohtaisen
budjetoinnin malli on
suunniteltu.
37. 4 toimivaa periaatetta
1. Tapoja, jolla olemassa olevaa rahoitusta
käytetään tulee muuttaa - ei kannata
käyttää uutta rahaa ja jättää vanhaa
järjestelmää paikalleen
2. On varmistettava, että rahoitus
mahdollistaa oikeuksien hyödyntämisen
- rahaa tulee delegoida paikallisille
alueille niin paljon kuin mahdollista
3. Ei kannata rajoittaa liikaa sitä, kuka voi
tuottaa tukea tai auttaa ihmisiä
hallinnoimaan budjettejaan -
järjestelmän on hyvä olla avoin
4. On mahdollistettava, että ihmiset voivat
käyttää henkilökohtaisia budjettejaan
joustavasti, jotta he voivat rakentaa sille,
joka heidän elämässään jo toimii
1. Transform how existing funding
is used - don’t spend new money
while leaving the old system in
place.
2. Make sure there is funding in
place to make rights real -
delegate money to local areas as
much as possible.
3. Don’t restrict too much who can
provide support or help people
manage their budgets - keep the
system open.
4. Make sure people can use money
flexibly, so that they can build on
what is already good in their life.

Scotland - Glasgow 12 -44.0%
USA - Denver - Disabled Children - -34.0%

39.
40. Personal budgets work
best when we
remember that money
is not the most
important thing in life.
Henkilökohtainen
budjetointi toimii
parhaiten, kun
muistamme ettei raha ole
tärkein asia elämässä.
41.
42.
43.
44. Compare this with
how the old system
worked
Vertailua siihen, miten
aiempi järjestelmä toimi
45.
46. We give people
services because they
have needs.
But services on their
own can stop people
flourishing.
It is human rights, not
services, that are
important.
Me annamme ihmisille
palveluita, koska heillä
on tarpeita.
Mutta palvelut
itsessään voivat estää
ihmisiä kukoistamasta.
Palvelut itsessään eivät
ole tärkeitä, vaan
ihmisoikeudet.
47. It is is important to
meet people’s needs.
But many needs
cannot be met unless
the person meets their
own needs.
Tarpeet ovat tärkeitä.
Mutta monet tarpeista
eivät tule tyydytetyksi,
elleivät ihmiset itse
pysty vastaamaan
tarpeisiinsa
51. Personal budgets give
people much more
flexibility about what
money can be spent on.
It should be anything
that is not criminal.
People also need lots of
options for managing
their personal budget.
Henkilökohtainen
budjetointi antaa ihmisille
enemmän joustavuutta
siihen miten rahat voidaan
käyttää.
Käyttötarkoituksen ei
kuitenkaan pitäisi olla
rikollista.
Ihmiset tarvitsevat myös
erilaisia mahdollisuuksia
henkilökohtaisen budjetin
hallinnointiin.
52.
53. For example, in Scotland
we developed a system
called Individual Service
Funds that allowed
support organisations to
manage personal
budgets.
Esimerkiksi Skotlannissa
kehitimme ’yksityiseksi
palvelurahastoksi’
kutsutun järjestelmän,
jolla mahdollistettiin
henkilökohtaisten
budjettien hallinta
organisaatioille.

60. Everything can be individualised and made more personal.
Everything can be individualised and made more personal.
61. Joudumme alistumaan vahvempien tahtoon. Isot yritykset, kaupungit
ja kunnat päättävät puolestamme mikä on meille parasta. Tässä on
kysymys vallasta. Miksi minusta tuntuu, ettei minulla ole valtaa
omassa elämässäni? Sami Helle, Euroopan Parlamentissa
62. Until recently, the Dutch welfare-to-work system was dominated by
a small number of very large private providers. In 2004, 47
providers had contracts with the national benefits agency, 10 of
them responsible for 75% of the market. In response, in 2004 the
national benefits agency adopted a new system for commissioning
welfare-to-work providers: the individual welfare-to-work contract
(individuele reintegratie overeenkomst, or IRO). Under this system,
an individual jobseeker chooses a welfare-to-work provider, and
with them draws up a plan for returning to work….
…proliferation of providers: the 47 providers contracted by the
benefits agency in 2004 had become 2,400 by 2007, many of
them small and specialist – though provider numbers are now
contracting.

67. You can change when budgets need reviewing
Henkilökohtaista budjettia voi tarkistaa kun muutokselle on tarve.

73.
74. • Sosiaali- ja terveydenhuollon
kustannusten nousu on
kansainvälinen ongelma
• Kalliimmat, keskittyneemmät ja
laitosvaltaisemmat palvelut
lisäävät kustannuksia, mutta eivät
muodostu vaikuttavammiksi
• Suurempi arvo on
kansalaisuuden, perheiden ja
lähiyhteisöjen tukemisessa, mutta
näihin investointi vähenee.
• Miten voimme siirtää resursseja
vastavirtaan, jotta voimme
ratkaista ongelmia aiemmin ja
vaikuttavammin?
• The growth in the economic cost
of health and social services is
an international problem.
• More expensive, more
centralised and more institutional
services grow in cost, but do not
become more efficient.
• But greater value lies in
supporting citizenship, families
and local communities, but
investment in this is reducing.
• How can we move resources
upstream to solve problems
earlier and more efficiently?
75.
76. • Henkilökohtainen budjetointi on
yksi tehokas tapa siirtää
resursseja vääränlaisista
palveluista takaisin kansalaisille
ja lähiyhteisöille
• Henkilökohtaiset budjetit ovat
joustavia ja mahdollistavat
luovuuden ja resurssien
hyödyntämisen lähiyhteisöissä
• Henkilökohtaiset budjetit
auttavat paikallisia toimijoita
rahoituksen saamiseen suoraan
ihmisiltä - erillään
kilpailutusjärjestelmistä, jotka
usein johtavat keskitettyihin
palveluihin
• Personal budgets is one
powerful way of moving
resources out of the wrong
services and back to citizens
and local communities.
• Personal budgets are flexible
and they let people be
creative and use resources in
their local community.
• Personal budgets help
community organisations to
get funding from people -
outside the procurement
systems that often leads to
centralisation.
77. • Henkilökohtainen budjetointi
eroaa palvelusetelijärjestelmästä,
rahallisista etuuksista ja muista
"asiakasvalintajärjestelmistä",
kuten "raha seuraa potilasta" -
mallista
• Henkilökohtaiset budjetit voivat
olla henkilön itsensä, perheen,
paikallisorganisaatioiden tai
kuntien ammattilaisten
hallinnoitavissa
• Henkilökohtainen budjetointi on
johtanut merkittäviin parannuksiin
elämässä, tyytyväisyydessä ja
tehokkuudessa.
• Personal budgets are
different to vouchers,
benefits or other systems
of ‘customer choice’ or
‘money follows the patient’.
• They can be controlled by
the person, family,
community organisations
or government
professionals.
• They have led to significant
improvements in life,
satisfaction and efficiency.
