Position Title: Reentry Support Specialist
Organization/ Program: Wellness Connections
Reports To: Program Manager
Position Purpose:
Looking for highly motivated team player who embraces recovery and enjoys working with people.
This position will work as an integral member of our wellness program team, providing support and
rehabilitation services to adults in recovery with mental health and/or substance use disabilities.
Primary participants will be individuals who are re-entering the community following incarceration in
jail or prison. Emphasis is on providing the encouragement, support and resources members need to
accomplish their individualized goals for greater independence and enhanced quality of life. Job will
entail the delivery of recovery-based services to include life skills classes,support groups, health
promotion, pre-vocational training, personal assistance and transportation in a 1:1 and group setting.
Peer Support Specialists serve in the capacity of coaches and mentors, modeling wellness and
recovery practices and principles.
Required Qualifications:
Must have a demonstrated commitment to a recovery philosophy.
Must have personal mental health and/or substance use recovery experience.
Ability to interact effectively and positively with a diverse population.
Strong interpersonal, verbal and written communication skills.
Computer literacy skills.
Excellent organizational skills.
Ability to maintain confidential information, both oral and written.
Ability to work as a team member.
High school diploma or equivalent.
Valid driver’s license or state issued ID card.
Ability to become CPR and First Aid certified, and pass TB testing.
Reliable transportation to and from work.

Preferred Qualifications:
Associates or Bachelor’s degree in behavioral health related field;
Knowledge of the AZ Behavioral Health System and community resources
Certification as a Peer Support Specialist (CPSS) or completion of DBHS approved Peer
Support Training;
Bilingual Spanish speaker;
Ability to lift up to 45 lbs.
Primary Duties and Responsibilities:
Help program participants identify their strengths and abilities;
Assist program participants in attaining their recovery goals and resolve personal and
environmental obstacles.
Facilitate 1:1 and group peer support, skills training, health education and other recovery
activities.
Work collaboratively with participants, staff and volunteers to ensure members’ needs are
being met according to Program standards and State and RBHA requirements.
Assist with outings and events;
Transport program participants as necessary;
Assist with meal preparation and kitchen maintenance activities;
Be aware of community resources; link members as appropriate to needed services and
supports;
Complete participant progress notes on a daily basis, and other documentation as required;
Assist in the daily operation and maintenance of the center;
Attend/complete required trainings;
Other duties as assigned.
