What Is Client Directed Outcome Informed (CDOI) Clinical Work?  CDOI service contains no fixed techniques and no causal theory regarding the concerns that bring people to therapy or substance abuse treatment. Any interaction with a client can be client-directed and outcome-informed when the clientā€™s voice is privileged as the source of wisdom and solution and helpers purposefully form strong partnerships with clients: (1) to enhance the factors across theories that account for successful outcome; (2) to use the clientā€™s ideas and preferences (theory of change) to guide choice of technique and model; and (3) to inform the work with reliable and valid measures of the clientā€™s experience of the alliance and outcome.  Why Is CDOI Service Delivery a Good Idea?  ,[object Object]
The success rate of mental health and substance abuse services has little or no relationship with professional discipline (e.g., psychiatrist, psychologist, social worker, addiction counselor), level of care (inpatient, residential, outpatient), or type of treatment (cognitive-behavioral, motivational interviewing, medication).
The success rate of mental health and substance abuse services is largely about the client and the quality of the relationship formed with the therapist.
A small percentage of the total number of people treated (approximately 10%) accounts for 60-70% of the expenditures in mental health and substance abuse; By contrast, the average treated person accounts for less than 10% of the cost.
Drop out rates from treatment average 47%; Clinicians frequently fail to identify people at risk for dropping out of care or unsuccessful outcome.

The success rate of mental health and substance abuse services has little or no relationship with professional discipline (e.g., psychiatrist, psychologist, social worker, addiction counselor), level of care (inpatient, residential, outpatient), or type of treatment (cognitive-behavioral, motivational interviewing, medication).
  • 3. The success rate of mental health and substance abuse services is largely about the client and the quality of the relationship formed with the therapist.
  • 4. A small percentage of the total number of people treated (approximately 10%) accounts for 60-70% of the expenditures in mental health and substance abuse; By contrast, the average treated person accounts for less than 10% of the cost.
  • 5. Drop out rates from treatment average 47%; Clinicians frequently fail to identify people at risk for dropping out of care or unsuccessful outcome.

Satisfaction with rendered services has improved dramatically and complaints have significantly reduced resulting in a decrease in liability and risk. ā€¢ Community Health & Counseling Services, Bangor, ME: Using CDOI services since 2003 with clients diagnosed with severe mental illnessesā€”dramatically improved retention and efficiency of services with perhaps the most difficult to serve clients. The analysis comparing before and after implementation will be published in the forthcoming second edition of the American Psychological Associationā€™s best selling Heart and Soul of Change (Duncan, B., Miller, S. & Wampold, B., in press).
