Menindaklanjuti Surat Edaran Nomor:0975/E5.4/HP/2013 tanggal 12 April 2013 perihal Program Insentif Artikel pada Jurnal Internasional Tahun 2013, dengan hormat kami sampaikan calon penerima program tersebut (terlampir) untuk disampaikan kepada penulis.
Untuk kelancaran pendanaan insentif artikel, maka pencairan dana akan kami sampaikan langsung kepada rekening penulis.
Sebagai persyaratan pencairan insentif, penulis diharuskan:
1. Menandatangani kuitansi 1 (satu) lembar dibubuhi meterai.
2. Menantangani surat Pernyataan Tanggung Jawab Mutlak/SPTJM 1 (satu) lembar dibubuhi meterai.
3. Mengisi data nama dan no NPWP pada form bukti pemotongan pajak.
4. Mengisi dan menyampaikan Daftar Isian, copy halalaman buku tabungan yg memuat nama dan no rekening, dan copy NPWP atas nama pribadi.
