ACADEMIC
ELIGIBILITY REQUIREMENTS
o Students must pass at least 5 classes from the
previous semester.
o Students must have a 2.0 GPA from the
previous semester.
o Waivers:
 are only available for the 2.0 GPA
requirement not for the pass 5 classes
requirement.
 may be used one time during middle school
years.
ELIGIBILITY REQUIREMENTS
o Date of Birth: a student cannot reach
his/her 15th birthday on or before August
1st of the current school year.
o Students are only eligible for 6
consecutive semesters, with the 1st
semester beginning when the child enters
the 6th grade.
o Transfer Rule: 6 or 8 games
o $250 fine for giving false information
regarding where you live.
INSURANCE VBCPS offers a supplemental insurance to your current
provider.
 Notify coach immediately if athlete is being seen by a
doctor or emergency room due to accident during practice
or game.
 Claim forms may be issued by the SAC.
 Treatment for any injury must be received and the claim
form filed within 90 days of the date of the injury.
 Plan provides 70% of Usual & Customary coverage.
 $25,000 maximum benefit.
 The deductible is $100 per injury.
 There is a $500 limit on physical therapy claims per injury.

SPORTSMANSHIP
The following are required of all PARENTS/SPECTATORS:
1. Realize you represent Plaza, so encourage through positive
behavior.
2. Being a good sport is more important than victory.
3. Recognize and show appreciation for an outstanding play by
both teams.
4. Be a “team fan” as well as a “my kid” fan.
5. Remember that a ticket to a school event is a privilege to observe
the contest.
6. Treat visiting teams and officials as guests.
7. Be modest in victory and gracious in defeat.
8. Respect the judgment and integrity of officials, realizing that
their decisions are based upon game conditions.
9. Bottom line… be a positive role model 
ATHLETE/PARENT/COACH
COMMUNICATION
Appropriate issues to discuss with the coach:
ways to help your child improve
concerns about your child’s behavior
Inappropriate issues to discuss with the coach:
playing time
starting positions
team strategy
play calling
other athletes
It is NEVER ok to approach a coach after practice or
competition to discuss any of the above. Please refrain
and call the next day.
IMPORTANT INFORMATION
DISCIPLINE:
 First ISS: the student may not stay after school
for practice and may not start the next
competition.
 Second ISS/OSS: the student will be removed
from the team.
GAMES:
 Participants must be in school a minimum of 3
hours in order to participate in practice or a
game.
 We will provide transportation to away games.
 Only a parent/guardian may take a child from an
away site and must sign out with the coach.
IMPORTANT INFORMATION PRACTICES:
• Practices are everyday and are closed to parents. Please be
available by phone while your child is practicing.
• After practice, all team members walk to bus loop to get on the
activity bus or meet their parent. Students will be placed on
their bus if the parent is not here.
 CANCELLATIONS OF GAMES/PRACTICES:
• Made after 2:00 pm. Please refrain from calling.
• There are few dates for make ups, so if at all possible we will
play.
• Be sure coaches have your contact info (cell phone & email
address).
 UNIFORMS:
• Responsibility of team members as soon as they take
possession. They must be returned in the same condition as
they were issued.
• May only be worn for Plaza sponsored events.

 At Plaza everyday; assists team members with various needs
 First Aid Kit:
 Inhalers/epi pens with self carry/administer forms
 Emergency contact/information sheet
 Administers the ImPACT test (online concussion test)
 6th graders - do not take the test
 7th graders - must take the test this year
 8th graders – must take the test if they have not taken it before
(Students were given a permission slip form today if they need to take
the test.)

WHAT HAPPENS IF MY CHILD IS
INJURED AT PRACTICE/GAME?
 Student-athlete needs to report any injury/illness to the
coach and athletic trainer, as soon as possible.
 Athletic Trainer will monitor the student’s injury and notify
the parent/guardian and school nurse, if necessary.
 If student-athlete is referred to physician, or goes on their
own, a note regarding diagnosis and participation status
must be provided to Mrs. Wydra and the school nurse.
 The Athletic Trainer and coach will help the student-athlete
return to play as quickly and safely as possibly.
WHAT HAPPENS IF MY CHILD SUFFERS A
CONCUSSION AT PRACTICE/GAME?
 With a suspected concussion:
 The athlete will not return to play that day and the athletic
trainer will conduct an initial evaluation. The athlete MUST be
picked up by a parent.
 If the athlete is diagnosed of sustaining a concussion, by the
Athletic Trainer and/or doctor, the Concussion Policy (The
Return to Play Progression) will be followed.
 Student-athlete must return doctor note to Mrs. Wydra and the
school nurse after each visit.
 The Return to Play progression will not begin with the athletic
trainer until the athlete is completely symptom free.
GRADUAL RETURN TO PLAY
 Each stage of the Concussion Graduated Return to Play
is a minimum 24 hour period.
Stage 1: No activity
Stage 2: Light Aerobic Exercise
Stage 3: Sport Specific Exercise
Stage 4: Non-Contact Drills
Stage 5: Full Contact
Stage 6: Return to Play
 The final clearance is given by the athletic trainer.
REMEMBER
OThese guidelines and activity restrictions are
for any and all physical activity including:
physical education classes, rec. games and
practices, club games and practices, etc.
ORules are in place to protect the student
athlete.
OYour support is vital to the success of the
program and safety of the students.

  • 3. INTERSCHOLASTIC ATHLETIC MANUAL To find the VBMSL Interscholastic Manual:  Go to vbschools.com  Look under Students, Athletics & Activities, Middle School Sports
  • 4. ACADEMIC ELIGIBILITY REQUIREMENTS o Students must pass at least 5 classes from the previous semester. o Students must have a 2.0 GPA from the previous semester. o Waivers:  are only available for the 2.0 GPA requirement not for the pass 5 classes requirement.  may be used one time during middle school years.
  • 5. ELIGIBILITY REQUIREMENTS o Date of Birth: a student cannot reach his/her 15th birthday on or before August 1st of the current school year. o Students are only eligible for 6 consecutive semesters, with the 1st semester beginning when the child enters the 6th grade. o Transfer Rule: 6 or 8 games o $250 fine for giving false information regarding where you live.

INSURANCE VBCPS offers a supplemental insurance to your current provider.  Notify coach immediately if athlete is being seen by a doctor or emergency room due to accident during practice or game.  Claim forms may be issued by the SAC.  Treatment for any injury must be received and the claim form filed within 90 days of the date of the injury.  Plan provides 70% of Usual & Customary coverage.  $25,000 maximum benefit.  The deductible is $100 per injury.  There is a $500 limit on physical therapy claims per injury.
  • 9. SPORTSMANSHIP The following are required of all PARENTS/SPECTATORS: 1. Realize you represent Plaza, so encourage through positive behavior. 2. Being a good sport is more important than victory. 3. Recognize and show appreciation for an outstanding play by both teams. 4. Be a “team fan” as well as a “my kid” fan. 5. Remember that a ticket to a school event is a privilege to observe the contest. 6. Treat visiting teams and officials as guests. 7. Be modest in victory and gracious in defeat. 8. Respect the judgment and integrity of officials, realizing that their decisions are based upon game conditions. 9. Bottom line… be a positive role model 

ATHLETE/PARENT/COACH COMMUNICATION Appropriate issues to discuss with the coach: ways to help your child improve concerns about your child’s behavior Inappropriate issues to discuss with the coach: playing time starting positions team strategy play calling other athletes It is NEVER ok to approach a coach after practice or competition to discuss any of the above. Please refrain and call the next day.

IMPORTANT INFORMATION DISCIPLINE:  First ISS: the student may not stay after school for practice and may not start the next competition.  Second ISS/OSS: the student will be removed from the team. GAMES:  Participants must be in school a minimum of 3 hours in order to participate in practice or a game.  We will provide transportation to away games.  Only a parent/guardian may take a child from an away site and must sign out with the coach.
  • 12. IMPORTANT INFORMATION PRACTICES: • Practices are everyday and are closed to parents. Please be available by phone while your child is practicing. • After practice, all team members walk to bus loop to get on the activity bus or meet their parent. Students will be placed on their bus if the parent is not here.  CANCELLATIONS OF GAMES/PRACTICES: • Made after 2:00 pm. Please refrain from calling. • There are few dates for make ups, so if at all possible we will play. • Be sure coaches have your contact info (cell phone & email address).  UNIFORMS: • Responsibility of team members as soon as they take possession. They must be returned in the same condition as they were issued. • May only be worn for Plaza sponsored events.

CHKD ATHLETIC TRAINER Mr. Seth Ireland  At Plaza everyday; assists team members with various needs  First Aid Kit:  Inhalers/epi pens with self carry/administer forms  Emergency contact/information sheet  Administers the ImPACT test (online concussion test)  6th graders - do not take the test  7th graders - must take the test this year  8th graders – must take the test if they have not taken it before (Students were given a permission slip form today if they need to take the test.)  Communication is key:  Cell: 757-837-2542 Email: Seth.Ireland@CHKD.org

WHAT HAPPENS IF MY CHILD IS INJURED AT PRACTICE/GAME?  Student-athlete needs to report any injury/illness to the coach and athletic trainer, as soon as possible.  Athletic Trainer will monitor the student’s injury and notify the parent/guardian and school nurse, if necessary.  If student-athlete is referred to physician, or goes on their own, a note regarding diagnosis and participation status must be provided to Mrs. Wydra and the school nurse.  The Athletic Trainer and coach will help the student-athlete return to play as quickly and safely as possibly.
  • 17. WHAT HAPPENS IF MY CHILD SUFFERS A CONCUSSION AT PRACTICE/GAME?  With a suspected concussion:  The athlete will not return to play that day and the athletic trainer will conduct an initial evaluation. The athlete MUST be picked up by a parent.  If the athlete is diagnosed of sustaining a concussion, by the Athletic Trainer and/or doctor, the Concussion Policy (The Return to Play Progression) will be followed.  Student-athlete must return doctor note to Mrs. Wydra and the school nurse after each visit.  The Return to Play progression will not begin with the athletic trainer until the athlete is completely symptom free.
  • 18. GRADUAL RETURN TO PLAY  Each stage of the Concussion Graduated Return to Play is a minimum 24 hour period. Stage 1: No activity Stage 2: Light Aerobic Exercise Stage 3: Sport Specific Exercise Stage 4: Non-Contact Drills Stage 5: Full Contact Stage 6: Return to Play  The final clearance is given by the athletic trainer.

REMEMBER OThese guidelines and activity restrictions are for any and all physical activity including: physical education classes, rec. games and practices, club games and practices, etc. ORules are in place to protect the student athlete. OYour support is vital to the success of the program and safety of the students.
