Reports that she sleeps about 5 hours/night to which she adds “I hate sleep, it’s no fun.” You reviewed her hospital records and find that she has been medically worked up by a physician who reported her to be in overall good health. Lab studies were all within normal limits. You find that the patient had genetic testing in the hospital (specifically GeneSight testing) as none of the medications that they were treating her with seemed to work. Genetic testing reveals that she is positive for CYP2D6*10 allele. Patient confesses that she stopped taking her lithium (which was prescribed in the hospital) since she was discharged two weeks ago. MENTAL STATUS EXAM The patient is alert, oriented to person, place, time, and event. She is dressed quite oddly- wearing what appears to be an evening gown to her appointment. Speech is rapid, pressured, tangential. Self-reported mood is euthymic. Affect broad. Patient denies visual or auditory hallucinations, no overt delusional or paranoid thought processes readily apparent. Judgment is grossly intact, but insight is clearly impaired. Informatics meshes technology and information into something useful. In nursing, informatics uses media such as health portals, mobile apps, social networking, and telehealth to aide nurses, patients and other stakeholders in decision making. When utilizing informatics our knowledge increases, we communicate better, we become more efficient with our job, and we provide better patient care. As a hospice nurse, I often admit patients with very little to no medical history. My scenario would be to have a database that shares all health information for patients. If this would be available, decisions could be made that is in the best interest of the patient immediately and not have to wait for information to be sent. When having to wait, it impedes patient care and often has to backtrack and change the plan of care. Many times there is vital medical history missed because the patient/family member is a poor historian due to disease process or stress of the situation. If there is one database for all information potential medication errors could be avoided because you would know information such as allergies, current and previous medications prescribed. This technology is already available to some extent in the prescription pain medicine world. The prescription drug monitoring program (PDMP) is a database that keeps track of controlled substances being prescribed. This is used in the fight against the opioid epidemic occurring today. This idea is not new but has not come to fruition yet. If this was to occur, HIPPA would have to be strictly maintained. This could happen by having the patient sign an agreement allowing their information to be in the database and every provider having access to it. Many people travel even state to state and if there was a national database, all health records would be available at the fingertips of the provider no matter where the patient was allowing efficient and knowledgeable patient care. The state of our Prisons Presentation Introduction Research has found out that state prisons tend to hold more mentally ill individuals than psychiatric hospitals The number of mentally ill prisoners held by the individuals is said to be more than ten times. The result of this has been a change in prison warders duties to become caretakers of these ill individuals. This has generally led to poor services being delivered as prisons are less equipped with very poorly trained individuals. Recent research has found out that a good number of state prisons are holding more mentally ill individuals than psychiatric hospitals do. The number of mentally ill prisoners is said to be ten times more than in the psychiatric hospitals. The result of this has been poor service delivery 2 Continuation Current problem State prisons have thus been converted to asylums for several individuals suffering from mental illnesses in the United States. Population The population targeted is made up of the inmates Thesis statement Although prison is the appropriate place for individuals who may cause harm to others or to themselves, correctional facilities are not the right place for mentally unstable convicts (Fuller, 2015). The problem being experienced currently is that of having state prisons acting as asylums for mentally ill individuals. In this, it is important to note that despite prisons being the best facilities to correct individuals, they are actually not the right places to put mentally ill convicts. 3 Background Mentally unstable patients in the past were being locked up in dungeons and isolated from the rest of the world. Over 75%cases of mental illnesses are recorded in prisons and jails. There is little information concerning the efforts made to minimize the increase of mental disorders in prisons. People who are mentally ill have in the past been isolated from the others. The current statistics indicate that a good number of mentally ill individuals are in prisons and jails and there is inadequate information to know more about their health conditions. 4 Major Points Prisoners are at a higher risk of getting psychological disorders than the general public (Wolfe et al. 2014). Mental disorders more common among females and young prisoners. Homelessness for prisoners suffering from mental illness History of physical or sexual abuse among inmates with mental health problems It has been noted that a good number of prisoners happen to be exposed to a higher risk of acquiring psychological disorders than the general public does. Most of these disorders are always common with young female prisoners and escalated by issues like homelessness and having a history of either a sexual or physical abuse. 5 Continuation There is little to no access of treatment for inmates. All of the contractor workers completed the refinery orientation training. Work for the contractor crews is assigned/scheduled each morning. On the day of the incident, the day-shift (6 am to 6 pm) crew had been tasked with isolating the acid gas feed stream for the Claus unit. Due to other work priorities, the crew did not isolate the line as planned. A shift turnover for the night contractor crew did not happen due to mandatory safety training that delayed their arrival at the worksite. Upon their arrival at the work site, the night crew held a job safety analysis (JSA) review of the scheduled task (line breaking of the acid gas feed line to replace a segment) to be performed and the hazards present. No pressure gauges or monitoring was present to indicate that the acid gas feed line was operational. The crew initiated the line breaking activity (open the line to the atmosphere) at approximately 7:45 pm under self-contained breathing apparatus (SCBA), which almost immediately resulted in the uncontrolled release of acid gas. Discuss barriers that prevent homeless people with mental illness from receiving care measures to promote access. Kevin, a 39-year-old unemployed homeless male who has paranoid schizophrenia, was brought to the psychiatric hospital by the police. Citizens called the police because Kevin was in the street directing pedestrians and traffic in opposition to the traffic lights and verbally abusing everyone who did not follow his directions. Kevin is known to the police since he is often homeless, and states that his family does not want him. Kevin also has a history of poly substance abuse with alcohol, heroin, and crack cocaine, and he has been jailed for public intoxication several times. The nursing assessment reveals that Kevin has not been taking his prescribed psychotropic medications for 3 weeks. Kevin states that he does not have any money, and he does not remember where to go for mental health care (Learning Objectives: 2) a. What are the major factors that contribute to Kevin’s frequent homelessness? b. What barriers does Kevin face in the receiving treatment? How can these barriers be addressed?
