A Career In Long Term Care Administration
As a recent graduate from the Health Care Administration and Planning (HCAP) program at Tennessee State University my desire
for Health Care Administration has grown tremendously. My passion has always been Long Term Care, from observing family
members being admitted into a facility with high abuse rates, sitting with the elderly at a young age, to completing an internship at
National Healthcare Corporation (NHC) during the summer of 2016 under the Regional Director, Mr. Tim Shelly. I have always
had a love for the aging population and wanted to work in an environment that believed in producing the best quality of care for the

The two types of legislation that may or may not affect the aging population in the United States are; The Patient Protection and
Affordable Care Act of 2010 and the Health Care and Education Affordability Reconciliation Act of 2010 respectively. Population
aging may affect financial markets if individuals tend to amass assets during their working years and spend them during retirement.
When there is a large cohort such as the baby boom, there may be more demand than usual for corporate stock and other assets
while the cohort saves for retirement. This demand for corporate stock and other assets may abate after the cohort retires.

demand for nurses is expected to only rise. Adding an interesting element is the fact that not only are patients aging, nurses are also
aging. Per Auerbach, Buerhaus, and Staiger (2015), 40% of working nurses today are over the age of 40. In the next 20 years, this

As unique as we are throughout our lifetimes, we are just as unique in how we age. Seniors today who are 65–plus have a greater
opportunity of living another thirty or more years, and while some are not healthy in body and/or mind, a great many are living full

justice. Justice is based on the idea to act fairly (Morrison, 2011). Analyze In this case, we will analyze the important questions of
intubation of patients with dementia by applying the four ethical principles. The first principle is autonomy which is the right to

Cognitive Disorders
1. The four major categories of cognitive disorders are: delirium which is an alteration in consciousness that manifest and
progresses over a relatively short span of time where people have a decreased cognizance of their surrounding environment;
dementia is an on–going atrophy of brain function that is pronounced by damage of memory, bewilderment and inability to focus;
amnesia marked by an indicative loss of memory, against no loss of other cognitive functions that appear in dementia; there is also
an inclusion of other cognitive disorders not particularly specified that may otherwise not be categorized properly on their own. A
broad assortment of factors may lead to cognitive disorders, that could also encompass other general medical conditions such as
brain infections or head injuries.
Cognitive disorders are normally assessed after physical and neurological inspection. Examples of these tests would be imaging
exams, blood screening, liver function tests and thyroid analysis. Treatment options vary and depend on what kind of condition
appears. Delirium for instance is normally treated adequately through analysis of the underlying cause, the prognosis however, is
not as positive for patients suffering from dementia or amnesia. Common mental disorders such as depression, affect cognitive

I conducted interviews with individuals from the Department of Health and Human services that focused on the aging population,
vulnerable adults and their residency in long term care facilities. The DHS has different branches that cater to the needs of older
adults as well as regulations and laws aimed at protecting residents in long–term care facilities. The regulations include, the Adult
protection statute, and general statutory guidelines for home and community–based services under Medicaid. Minnesota and the

development including meals on wheels. 50,000 Minnesota older Adults rely on meals on wheels for their daily nutrition. Meals on
wheels allows older and disabled adults to remain in their homes and still get food every day, but this might be in jeopardy because
of the new proposal by the trump administration. Furthermore, the trump administration is planning on decreasing the budget of the
Department of Human Services, which funds the Meals on Wheels program. This reduction will increase the impact on senior

A sample of people were selected and given a juvederm injection one of the nasolabial fold and followed for around 20 weeks. The
same sample was injected with a bovine based collagen on the opposite nasolabial fold and followed for 20 weeks also. Juvederm
Injection site reactions for all the patients were similar and lasted for a week. Swelling occurred in 80% of the patients, bruising in
60% and itching in 35%. The patients who maintained improvement with Juvederm at 4 weeks was 90% compared to the 40% with
the bovine based collagen. The patients at the end of study showed a 90% preference to juvederm compared to a 10% preference to

It is not easy to come up with a working definition of aging; different academics around the world defined 'aging' from different
perspectives. Based on the collected information I have, I define aging –a multidimensional process of physiological, mental and
social changes that occurred over the course of life. Though it is beyond human control, aging experiences may vary between
individuals. "In the developed world, chronological time plays a paramount role. The age of 60 or 65, roughly equivalent to
retirement ages in most developed countries is said to be the beginning of old age. In many parts of the developing world,
chronological time has little or no importance in the meaning of old age. Other socially constructed meanings of age are more
significant such as the roles assigned to older people; in some cases it is the loss of roles accompanying physical decline which is
significant in defining old age. Thus, in contrast to the chronological milestones which mark life stages in the developed world, old

According to the Administration on Aging "elder abuse is a term concerning intentional, or negligent acts by a caregiver or any
other person that causes harm or a serious risk of harm to a vulnerable adult." Physical abuse consists of any physical behavior
concerning an elder that is aggressive towards that person (e.g. assault, battery, unsuitable restraint). Another familiar type of elder
abuse is emotional/psychological abuse. Emotional/psychological abuse includes verbal or nonverbal actions that are considered
violent and are directed toward another individual. According to Administration on Aging, emotional/psychological abuse consists
of "inflicting mental pain, anguish, or distress on an elder person through verbal or nonverbal acts (e.g. humiliating, intimidating or
threatening)." Furthermore sexual abuse of an elder is also a common form of elder abuse. Particularly, sexual abuse is any "non–
consensual sexual contact of any kind," (Administration on Aging). Basically, sexual abuse is the violation or exploitation of
another individual in a sexual manner. Additionally, an elder can also be a victim or financial abuse/exploitation. The type of abuse
consists of "the illegal taking, misuse, or concealment of funds, property, or assets of an elder for someone else's benefit,"
(Administration on Aging). According to the National Institute of Justice (2013), financially exploited elderly people suffered
numerous incidents of financial crimes and misdeeds, comprising theft, fraud and misuse of their assets. In many cases these elders
did not have someone with whom they can consult or who will supervise their financial pursuits (National Institute of Justice,
2013). Lastly, elder abuse also consists of neglect. Neglect is considered "the failure by caregivers to provide food, shelter, health

Even though payments from Social Security and Supplemental Security Income have played a critical part in an attempt to increase
economic security among older adults, poverty still remains on the rise for this population. Many still live on limited incomes and
have the modest savings. In 2013, 50% of people in Medicare possessed incomes of less than $23,000 which corresponds to 200%

In 2014, approximately 5.7 million people over the age of 60 were food insecure which comprises 9% of the entire senior
population. This number is projected to increase by 50% by the time the youngest of the Baby Boom Generation reaches age 60 in
2025. It has been proven that food insecure seniors are at an increased risk for chronic health conditions where 60% of the aging
population experience depression, 53% report a heart attack, 52% develop asthma, and 40% report a congestive health failure
(Feeding America, 2016). Recognizably, for seniors in poverty, protecting themselves from food insecurity and hunger is a lot more
difficult than the general population. For instance, researchers conducted a study that focused on the experience of food insecurity
among the elderly population. They took a sample of 46 elderly households from three large cities in upstate New York utilizing
purposive sampling. Older adults were recruited via subsidized housing programs, churches, congregated and home–delivered
meals programs, and a Latino community worker. The elders varied by age, sex food programs used, spousal status, and mobility.
The final sample comprised of 25 Latino older adults living in one large city, 28 non–Latino elders residing in two large cities.
They found that most of the time, food insecure older adults did not have the means to afford the right foods for health and they did
not have the capability to prepare food (Wolfe et. al, 2003). To slowly combat the issue of this insecurity, Title III Grants to State
Formula of the 2016 OAA Reauthorization Act accounts for geographic changes in the older population. The bill regulates the

Individuals in lower socioeconomic (SES) groups are at a higher risk to experience disability and cognitive impairment at an earlier
age, which makes this group in need for nursing home care (Berthelot, Derksen, Finlayson, & Mustard, 1999). Unfortunately, from
a 2011 statistics, the average cost of nursing home care is $85,775, but these varies by state (Berger, 2014). Medicaid provides the
money for individuals to receiving nursing home care, but not all nursing homes accept medicaid programs (Dickey, n.d.), Nursing
homes that accept medicaid programs are subject to government inspections (Dickey, n.d.). Furthermore, to receive medicaid for
nursing home care, individuals must prove that is "medically necessary," and this decision varies by state (Dickey, n.d.). Higher
quality (non–profit) nursing homes are usually funded by religious organizations (Berger, 2014). Other sources of funding are from
Medicare (not for long term care), Medicaid, and also Veterans Administration Aid and Attendance (for veterans) (Mullin, 2013).

There is not a plethora of research on the transgender inmate population. Brown and McDuffie (2009) report 750 transgender
prisoners were in custody in 2007. The only reason this population is last on the list of importance is because of the limited
population. Transgender inmate population pose one of the most challenging legal questions to the DOC. How far does the DOC
have to go in providing medical, psychiatric, or surgical needs to those inmates who enter the correctional facility as transgender
(Brown & McDuffie, 2009). There has been some successful litigation that has addressed these issues with inmates who have been
diagnosed with gender identity disorders (GID). Brown and McDuffie (2009), suggest California has some of the most
"comprehensive directive" that allows inmates to continue or initiate "cross–sex hormones for appropriately diagnosed inmates"
(p.288). Ultimately, the transgender population pose a real threat to the correctional environment, such as, safety issues and

A method to solve the socioeconomic issue, newer long–term facilities with great qualities, adequate staffing could be opened. The
facilities could seek funds from the state, and from local donations such as churches, in an effort to open a long–term care facility
with high quality but with lower costs. For nursing homes who refuse to admit older African Americans based on their chronic
illness, people could begin to sue the nursing homes for discrimination and maybe that would stop the discrimination in admitting
older African Americans to higher quality care facilities. Another method to lower and stop racial discrimination older African
Americans face in nursing homes is to maybe have a black owned, and all black staff so that there wouldn't be any point for racial
discrimination to occur. For older African Americans who simply opt to check in to nursing homes with lower quality, there isn't
much one can do to change that except talk to them and make them aware that there are other alternatives in seeking long–term care

An aging population is a population with an increased median age due to an increase in life expectancy, accompanied by a decrease
in fertility rates. This phenomena is already occurring in other countries, and the United States is expected to undergo a similar
demographic shift. The population of citizens aged 65 and over is projected to double in the next few decades, from 43.1 million in
2012, to 83.7 million in 2050, and life expectancy is expected to rise at the same time (Ortman, Velkoff, Hogan, & others, 2014).
The most important consequence of this is an increase in the percentage of older people from 13% to 20%. An increase of these
amount while seemingly insignificant is going to pose a significant burden on the societal support systems set in place to help older
individuals. These burden while widespread across different departments, is poised to heavily impact a healthcare climate that is
already stretched to the brink. It is essential for adequate preparation to be made in form of public policies that plan for the future.
The three most important public policy issues facing the United States are reauthorizing the Older Americans Act, ensuring long–
term services and supports, and health coverage for older individuals. The Older Americans Act of 1965 was the first

their likelihood of being taken advantage of and mistreated has risen a great amount. This vulnerable population does not deserve
this kind of treatment. We need to be doing whatever it takes to provide these people with quality health care to ensure safety and
good quality of life. This means increasing funding, continuing education for providers and other related professions, harsher
punishments for violators, and strict laws requiring background checks and health assessments before being allowed to care for an
elderly person. The aging population is at high risk of being taken advantage of and mistreated by a caregiver or family member.
Elders are the most vulnerable group of people and are subject to elder abuse in their later years of life. The Administration of
Aging refers to elder abuse as the knowing, intentional, or negligent act that causes harm or serious risk of harm to a vulnerable
adult. Every year, hundreds of thousands of older adults are abused, neglected, and exploited. Unfortunately, a trusted caregiver or a

With the significant increase of older Americans their likelihood of being taken advantage of and mistreated has risen a great amount. This vulnerable population does not deserve this kind of treatment. We need to be doing whatever it takes to provide these people with quality health care to ensure safety and good quality of life.
